Timothy C. Hain, MD Please read our Return to Index Page last modified: September 4, 2005 Saccades are used to bring the eye rapidly from one point of regard to another. As one does not see during a saccade, it is best to get them over as quickly as possible. Accordingly, saccades typically move at speeds between 200 and 600 degrees/sec for 300 deg/sec, to move gaze 90 degrees, it takes 1/3 second (which still seems like a rather long time not to see). Things that happen very quickly often have a tendency to get out of control. With saccades, most disorders consist of either instability (oscillation, flutter and opsoclonus), or inability to inhibit saccades (square wave jerks, saccadic intrusions).
Recording saccadic nystagmus isn't easy:
Although one might think that recording of a very fast eye movement would be very easy, as it is easy to spot with the naked eye, as of 2005, eye movement recording systems often fail to register saccadic nystagmus. Practically, we have failed to register obvious saccadic nystagmus with infrared reflection recording systems (one from Microguide, with a nominal 100 hz bandwidth), as well as on numerous occasions with an infrared VENG system from Micromedical Technologies . Rather than seeing back-back saccades, saccadic nystagmus looks like square wave jerks (see below).

This article is a condensed version of a paper written in partial fulfillment of the requirements for the class of Scientific Evidence and Opinion Testimony during the Winter Semester of 1999 at the UMKC School of Law. The extensive footnotes have been omitted.
An additional test is the horizontal gaze nystagmus ("HGN"). When used in combination with divided attention tests, HGN helps police officers correctly distinguish suspects who are under the influence of alcohol from those who are not. The test is based on the fact that alcohol affects the automatic tracking mechanisms of the eyes. Nystagmus is defined as "an involuntary rapid movement of the eyeball, which may be horizontal, vertical, rotatory, or mixed." Alcohol slows down the eyes' ability to rapidly track objects and causes to eyes to oscillate, or "jerk", before they normally would in a sober person. Alcohol stimulates the nerve endings, making nystagmus more pronounced in intoxicated persons. As a person's blood alcohol concentration increases, the eyes will "jerk" sooner as they move to the side. The HGN test claims to gauge intoxication by measuring the involuntary oscillation of the eyes. Nystagmus may be caused by many other factors. The court, in

Description:
Nystagmus is the involuntary movement of the eyes. The movements can be from side to side (most common), up and down or in a circular movement. Nystagmus can affect people at birth (early onset, infantile or congenital nystagmus) or later in life (acquired nystagmus). Early onset or congenital nystagmus may be associated with a defect in the eye or in the visual pathway from the eye to the brain. It occurs in conjunction with a number of other eye disorders, such as cataracts, glaucoma, some retinal conditions and albinism. It can also occur in children with multiple disabilities, such as those with Down's syndrome. However, some children with nystagmus have no other health problems and the cause of the nystagmus is unknown: this is called idiopathic nystagmus. Some types of nystagmus can be inherited. Acquired nystagmus, whether in a child or an adult, can be the first sign of a serious eye or brain disorder and therefore people should be referred to an ophthalmologist or a neurologist as soon as the condition is detected. The effects of nystagmus vary from person to person but in all cases it leads to vision becoming blurred. People who develop acquired nystagmus later in life tend to experience a constantly moving image (oscillopsia); people often experience sight loss to varying degrees; perception of depth is often greatly reduced and balance can be affected.

Nystagmus is een ritmisch herhaald onwillekeurig bewegen van de ogen. Dit gebeurt onafhankelijk van de willekeurige oogbewegingen en kan niet bewust worden onderdrukt, de bewegingen zijn niet aan de wil onderworpen. Dit is gewoonlijk met het blote oog waar te nemen door een ander. Nystagmus kan vóórkomen als gevolg van en naast een andere oogaandoening, of als op zichzelf staande oogaandoening.
