Benzathine penicillin G in the treatment of neurosyphilis
PG Cuddy The definition, pathogenesis, incidence, diagnosis, and treatment of neurosyphilis are discussed. Controlled trials of benzathine penicillin in the treatment of neurosyphilis are reviewed, as are recent case reports of benzathine penicillin failures. Although few well-controlled studies exist to document conclusively the efficacy of benzathine penicillin in the treatment of neurosyphilis, its use is recommended in selected situations.

It is a tertiary, or third-stage, form of syphilis that affects the brain and spinal cord. What is going on in the body? General paresis is a tertiary form of syphilis. It develops in 5% of the people with untreated syphilis when Treponema pallidum attacks the nervous system. It causes a gradual mental and neurological decline. The symptoms generally appear 20 years or more after the initial infection. What are the causes and risks of the disease? General paresis is a long-term effect of an untreated syphilis infection.

  • 1. Otosyphilis is a rare but very serious cause of hearing loss and dizziness 2. It is diagnosed with a lumbar puncture as it is a form of neurosyphilis 3. It is treated with daily penicillin 4. After treatment, followup lumbar punctures are needed every 3 months until spinal fluid returns to normal.
Syphilis is presently uncommon in the United States although it is making a comeback in some populations (persons with HIV infection). Syphilis is caused by infection with Treponema Pallidum, a spirochete. It usually is spread through contact with infectious lesions or body fluids, and generally through sexual contact.
GENERAL SIGNS AND SYMPTOMS:
About 21 days after exposure, patients develop a skin lesion called a chancre at the site of exposure. These are often on the sexual organs. Syphilitic chancres are typically nontender, hard, non-purulent ulcers. The chancres typically heal without therapy. Secondary syphilis begins 4-10 weeks after the chancre. A rash is the main complaint. It often affects the hands and or soles. The rash is rarely vesicular (blistering). There are many less specific findings such as sore throat, headache and the like. Secondary syphilis can cause neurologic, renal, ophthalmologic, gastrointestinal and hepatic disease. Secondary syphilis again, typically resolves without treatment.

ABSTRACT Background Infection with the human immunodeficiency virus (HIV) may affect both the natural course of syphilis and the response to treatment. We examined the response to treatment with high-dose penicillin G in HIV-infected patients with symptomatic neurosyphilis. Methods Neurosyphilis was defined by reactivity in serum treponemal tests for syphilis, neurologic manifestations consistent with neurosyphilis, and a positive Venereal Disease Research Laboratory (VDRL) test on cerebrospinal fluid. To the Editor: The reports by Gordon et al. and Horowitz et al. (Dec. 1 issue) appropriately question the effectiveness of high-dose penicillin as a treatment for syphilis in persons infected with the human immunodeficiency virus (HIV). However, these reports do not prove that such treatment is ineffective. The polymerase-chain-reaction (PCR) tests used do not necessarily indicate the presence of replicating organisms. In addition, an immunologic marker, especially a nonspecific one such as the Venereal Disease Research Laboratory (VDRL) or rapid plasma reagin titer, cannot be relied on to indicate successful treatment in persons who are immunologically Full Text of this Article References

Die Neurosyphilis hat seit der Einführung des Penicillins in die Therapie der Syphilis eine niedrige Inzidenz . Nachlässigkeiten bei der Behandlung der Syphilis und unbehandelt verschleppte Infektionen mit zunächst mildem Verlauf tragen neben der wieder zunehmenden Rate an Syphilis-Infektionen dazu bei, dass die Neurosyphilis auch heute (2005) noch regelmäßig zu Beschwerden führt.
Klinik
Die klassische Neurosyphilis tritt vor allem in den späten Stadien einer allgemeinen Syphiliserkrankung auf. Affektionen des ZNS können jedoch prinzipiell in jedem Stadium auftreten.
Neurosyphilis früher Stadien

Neurosyphilis
Neurosyphilis refers to the infection of the brain and spinal cord by the syphilis bacteria. This can lead to destruction in many areas of the nervous system , causing loss of function of a person's arms or legs, loss of vision, and altered mental abilities. Neurosyphilis can affect many different body systems and may develop over an extended period of time. Symptoms of neurosyphilis usually include:
  • Personality changes, such as confusion and irritability.
  • Hearing loss.
  • Decreased ability to concentrate.
  • Memory loss.
  • Difficulty speaking or understanding speech.
  • Tremor of the fingers and lips.
  • Mild headaches.
  • Disorderly appearance.
Other symptoms may include:
  • A wide gait.
  • Numbness or tingling of the hands or feet.
  • Muscle pain.
  • Joint destruction because of lack of sensation (Charcot's joint).
  • Inability to control urine or stool (urinary or fecal incontinence).

Of 60 patients with tonic pupils, 29 had serologic tests for syphilis. Five patients had positive blood serology and confirmatory tests; four had other manifestations of neurosyphilis and positive CSF serology. All seropositive patients had bilateral tonic pupils with light-near dissociation and denervation hypersensitivity. Of the 10 tonic pupils, only 1 was miotic and 1 dilated.   • Sichere Neurosyphilis-Diagnostik bei Neugeborenen: Ein Antikörper-Test und ein PCR-Test ermöglichen bereits am ersten Tag nach der Geburt den zuverlässigen Nachweis von Treponema pallidum im Liquor. Die Studie im New England Journal of Medicine (2002; 346: 1792-8) könnte zu einer gezielteren Antibiotikatherapie der Kinder führen. Die kongenitale (angeboren) Neurosyphilis ist zwar selten, aber bedrohlich, weil einige Kinder zunächst klinisch unauffällig sind. Betroffen sind die Kinder von an Syphilis erkrankten Müttern.
