Infections of the central nervous system caused by TREPONEMA PALLIDUM which present with a variety of clinical syndromes. The initial phase of infection usually causes a mild or asymptomatic meningeal reaction. The meningovascular form may present acutely as BRAIN INFARCTION. What is it? Syphilis is a disease caused by the organism Treponema pallidum . It is a sexually transmitted disease (STD), meaning that it is spread through sexual activity with someone infected with the bacterium. The infection usually causes disease over a course of several years. If it is not treated early or correctly, it can lead to serious problems, such as heart problems, mental disorders and neurologic problems (neurosyphilis), blindness, dementia, and death. Syphilis is passed from person to person through direct contact with a syphilis sore or lesion. Sores develop early in the course of syphilis, occurring mainly on the external genitals, vagina, anus, or in the rectum. Sores also can occur on the lips and in the mouth. It's important to note that these sores are not painful, so a person might not notice them. Transmission of the organism occurs during vaginal, anal, or oral sex. Pregnant women with the disease can pass it to the babies they are carrying. Rash-like lesions can appear on any part of the body after the sores heal, and are also infectious. Syphilis cannot be spread by toilet seats, door knobs, swimming pools, hot tubs, bath tubs, shared clothing, or eating utensils.

Testing combo helps diagnose neurosyphilis in HIV patients Last Updated: 2004-07-29 15:13:56 -0400 (Reuters Health) NEW YORK (Reuters Health) - The use of cerebrospinal fluid fluorescent treponemal antibody (CSF-FTA) reactivity testing combined with characterization of CSF lymphocytes can help diagnose neurosyphilis in HIV-infected patients, researchers report in the July 13th issue of Neurology. The diagnosis of neurosyphilis is not excluded by standard nonreactive CSF-Venereal Disease Research Laboratory (VDRL) testing "and in some cases is based solely on an elevated CSF white blood count," lead investigator Dr. Christina Marra of the University of Washington School of Medicine in Seattle told Reuters Health. "Establishing the diagnosis of neurosyphilis can be especially difficult in HIV-infected individuals because HIV itself causes mild elevations in CSF white blood cells that can be indistinguishable from the elevations seen in neurosyphilis." To investigate alternative approaches, the researchers used CSF-FTA testing and measurement of the percentage of CSF lymphocytes that were B cells in fresh and frozen serum samples from 47 HIV patients with syphilis and 26 HIV-infected controls.

A patient with subacute aphasia and hemiparesis was found to have a low density white matter lesion with mass effect on the computed tomographic (CT) scan. Serological examination and biopsy established the diagnosis of paretic neurosyphilis. This CT appearance has not previously been described in cases of neurosyphilis. Neurosyphilis should be considered as a potentially treatable cause of a cerebral mass lesion.

Syphilis and its consequent central nervous system sequelae remain clinical problems, especially when presenting in the unfamiliar acute meningeal forms of syphilitic meningitis and meningovascular syphilis. The diagnosis of neurosyphilis of all types depends on evaluation of spinal fluid reactivity as evidenced by pleocytosis and increased protein content with reactive serology. In a retrospective study encompassing 42 months (January 1984 through July 1987), 12 patients were identified as having both neurosyphilis and acquired immunodeficiency syndrome. These patients constituted 44% of the entire group meeting rigorous diagnostic criteria for neurosyphilis and 1.5% of all patients hospitalized with acquired immunodeficiency syndrome.
