Reports on Neurosyphilis
Managing syphilis in patients with HIV remains controversial. These two reports highlight the difficulties of diagnosing and monitoring a response to therapy for patients co-infected with HIV and syphilis. Gordon, et al. report the outcome of 11 patients with neurosyphilis (defined by reactivity in serum treponemal tests, a reactive cerebrospinal fluid VDRL, and the presence of neurologic symptoms). The patients were treated with conventional doses of intravenous penicillin G (18 to 24 million units per day) for ten days. Five patients had prior therapy for syphilis that apparently failed to prevent the development of neurosyphilis. Six weeks after therapy, serum RPR titers fell by two or more dilutions in four patients, and CSF VDRL decreased by two or more dilutions in seven patients. By 24 weeks, serum RPR fell in four of seven evaluable patients and CFS VDRL decreased or was nonreactive in four patients. Two patients had no improvement in serum or CSF titers or in cerebrospinal fluid profiles.

She had right sixth nerve palsy, ataxia, and decreased patellar and Achilles reflexes. The cerebrospinal fluid protein content was 0.86 g/L, and she had pleocytosis. One year later, re-evaluation revealed active neurosyphilis in the freshly drawn serum and cerebrospinal fluid samples. Clinical findings were consistent with tabes dorsalis. Key words: Neurosyphilis; Case report Introduction Syphilis is an infectious disease caused by Treponema (T.) pallidum , which is a natural pathogen only in humans. The infection is mainly acquired by sexual intercourse with an infected individual, but it can also be acquired by vertical transmission at delivery, by laboratory accidents, or through blood transfusion Four stages are distinguished in the natural course of syphilis: primary, secondary or disseminated, latent, and tertiary syphilis. Syphilis is classified as ’early’ up to the first year of latency, whereafter it is called ’late’ syphilis. Neurosyphilis can develop as a manifestation of both early and late syphilis. Neurosyphilitic syndromes in secondary syphilis include meningitis and cranial nerve palsies. In tertiary syphilis, three distinct clinical syndromes have been described: meningovascular syphilis, parenchymatous syphilis, and tabes dorsalis

Neurosyphilis
Neurosyphilis refers to the infection of the brain and spinal cord by the syphilis bacteria. This can lead to destruction in many areas of the nervous system , causing loss of function of a person's arms or legs, loss of vision, and altered mental abilities. Neurosyphilis can affect many different body systems and may develop over an extended period of time. Symptoms of neurosyphilis usually include:
  • Personality changes, such as confusion and irritability. Hearing loss. Decreased ability to concentrate. Memory loss. Difficulty speaking or understanding speech. Tremor of the fingers and lips. Mild headaches. Disorderly appearance.
Other symptoms may include:
  • A wide gait. Numbness or tingling of the hands or feet. Muscle pain. Joint destruction because of lack of sensation (Charcot's joint). Inability to control urine or stool (urinary or fecal incontinence).
Neurosyphilis is divided into two major categories based on the areas of the nervous system affected.

    Central nervous system Definition Neurosyphilis is a slowly progressive and destructive infection of the brain or spinal cord. It occurs in untreated syphilis many years after the primary infection. There are 4 different forms of neurosyphilis: asymptomatic , meningovascular, tabes dorsalis , and general paresis
    Asymptomatic neurosyphilis precedes symptomatic syphilis and is present in 15% of those with latent (hidden) syphilis. In this case, abnormalities may be present in the cerebrospinal fluid, but no symptoms are present.
    In meningovascular neurosyphilis, cranial nerve palsies and pupil abnormalities may be among a wide variety of symptoms. This may also cause damage to blood vessels resulting in stroke.
    In tabes dorsalis, progressive degeneration of the spinal cord occurs causing an inability to walk.

    In tabes dorsalis, progressive degeneration of the spinal cord occurs causing inability to walk. Incidence of tabes dorsalis is 6 out of 1 million.
    In general paresis, paralysis tremors seizures , and mental decline occur as a result of damage to brain cells. The incidence of general paresis is 1 out of 1 million people.
    Stroke secondary to syphilis
    affects 2 out of 1 million people.

  • Positiver Ausfall des TPHA-(oder TPPA-)Tests und des FTA-Abs-Test im Serum. Ein Patient leidet sicher an einer Neurosyphilis, wenn eine lokale treponemenspezifische Antikörperreaktion, messbar über einen spezifischen Antikörper-Index (empfohlen: ITpA-Test), vorliegt. Das Erscheinungsbild des syphilitischen ZNS-Befalls hängt ab vom Krankheitsstadium, in dem es auftritt. Im Sekundärstadium sind leichte Meningitis, Hirnnervenläsionen (N. VIII, VII und III), Polyradikulitis und selten vaskuläre Hirnstammsyndrome möglich. Während der Spätlatenz (>1 Jahr nach Infektion) werden die „asymptomatische“ Neurosyphilis (entzündlicher Liquor ohne klinisches Korrelat) wie auch die unklassifizierbare Neurosyphilis beobachtet. Letztere bietet eine vielfältige, aber nur gering ausgeprägte Symptomatik bei entzündlichem Liquorbefund.
