  • Definitions and classification Neuralgia : Pain in the distribution of a nerve or nerves. The term should be used primarily to refer to non paroxysmal pains. Neuropathy : A disturbance of function or a pathologic change in the nerves. mononeuropathy : involving a single nerve mononeuropathy multiplex : involving several nerves Polyneuropathy : involving symmetric or bilateral nerves neuritis : special type of neuropathy with an inflammatory process.
    Classification can be by presumed cause (diabetic, entrapment), or by anatomic site (intercostal neuralgia)
  • Clinical features May include weakness, muscle atrophy, fasciculations, cramps, and loss of tendon reflexes. Sensory changes may involve all sensations, or be selective. Confirmation of diagnosis is best achieved using nerve conduction studies or electromyography.

    Neuralgia means burning, numbness and pain that extends along one or more nerves. A nerve is a cord-like bundle of fibers through which sense and motor impulses pass between the brain and the body. Neuralgia may occur in any of these nerves. There are several disorders in which this kind of pain keeps coming back. These major disorders include:
    * cluster headaches,
    * trigeminal neuralgia of the face,
    * pain at the back of head, in the feet, and in the throat, tongue, or jaw, and
    * sciatica, which affects the lower back, buttocks, and legs. Some of the possible causes of neuralgia include:
    * vitamin B-12 deficiency,
    * chronic diseases such as diabetes and multiple sclerosis,
    * infectious diseases such as herpes, malaria and syphilis, and tumors. Attacks of pain often occur suddenly. There may be long periods between attacks. A person may suffer from anxiety and fear of the next attack. Attacks may be brought on by minor things, such as:
    * irritation of the area

    Neuralgias
    Overview Symptoms Treatment Prevention Alternative Names: Nerve pain; Postherpetic neuralgia Treatment: Treatment of neuralgias is aimed at reversing or controlling the cause of the nerve problem (if it is identified) as well as providing pain relief. Therefore, the treatment varies depending on the cause, location of the pain, severity of the pain and other factors. Even if the cause of the neuralgia is never identified, the condition may improve spontaneously or disappear with time.
    The cause (if known) should be treated. This may include surgical removal of tumors, or surgical separation of the nerve from blood vessels or other structures that compress it.
    Mild over-the-counter analgesics such as aspirin, acetaminophen, or ibuprofen may be helpful for mild pain. Narcotic analgesics such as codeine may be needed for a short time to control severe pain. These traditional pain killers, however, often have disappointing results.

    Central nervous system Definition Pains which follow the paths of specific nerves. Alternative Names Nerve pain; Postherpetic neuralgia Causes And Risk The causes of neuralgias are varied. Chemicals can cause nerve irritation. Inflammation, trauma (including surgery), compression by adjacent structures (tumors or inflamed tissues), and infections can all lead to neuralgias. In many cases, however, the cause is unknown or unidentifiable. Neuralgias are most common in elderly persons, but they can occur at any age.
    Trigeminal neuralgia
    is the most common form of neuralgia. It affects the main sensory nerve of the face, the trigeminal nerve ("trigeminal" literally means "three origins", referring to the division of the nerve into three branches). This condition involves sudden and short attacks of severe pain on one side of the face, along one of the areas supplied by the trigeminal nerve. The pain attacks may be severe enough to cause a facial grimace, which is classically referred to as a painful tic (tic douloureux).

Control of any underlying condition (such as diabetes) will help prevent progression. Drugs to prevent flare-ups may be helpful. When medical measures fail, surgery may be considered to remove the root of the nerve. These include electrosurgery, microsurgery, and radio wave surgery.

    Fundación Valle del Lili
    KRA 98 No. 18-49 (cons. 211)
    Cali, Colombia.

    Annales de Gastroenterologie et d'Hepatologie 1985 Dec, 21:6, 393-6 Suduca P Abstract Ano-rectal neuralgia consists of pain in the ano-rectal region without any underlying organic proctological disease. Classically, three major clinical groups are defined, although associated, successive and intermediary forms may also exist. Proctalgia fugax is the most distinctive condition, consisting of brief episodes of nocturnal pain recurring at very variable intervals. In coccygodynia, the pain is postural and reproduced by pressure and mobilisation of the coccyx. The other forms of ano-rectal pain are grouped under the heading ano-rectal neuralgia or neurosis, which are characterised by the imprecision and the variability of the symptoms described. The pathogenesis of these forms of pain is unknown. Psychiatric elements are generally involved in these conditions, especially in the third clinical group. The treatment suffers from this uncertainty about the pathogenesis. The patient should be reassured by means of the least aggressive proctological procedures.
