Nerve Compression Syndromes of the Lower Extremity Nerve compression syndromes of the lower extremity present a challenge in differential diagnosis. Compression of the common peroneal nerve occurs relatively frequently; compression of the sciatic nerve occurs infrequently. The pattern of weakness helps distinguish lumbar root entrapment from peripheral compression syndromes. Compression syndromes must also be differentiated from diabetic, alcoholic or vasculitic neuropathy. Recovery correlates with the degree and duration of nerve injury. Thus, early diagnosis and treatment are important.

Initial reports described stress fractures in the normal bones of young adults, and most authors identified an association between stress fractures and repeated episodes of increased activity, such as with military training or participation in sports.[3,4,9] These reports led to the concept that stress fractures result from fatigue failure of normal bone matrix, and experimental work has shown that repeated application of nontraumatic loads can cause stress fractures in normal bones.[10] More recent clinical experience with stress fractures has shown that these fractures occur in a variety of age groups, ranging from young children to elderly persons, and that they do not necessarily occur in association with a history of increased activity. In addition, multiple clinical reports have described stress fractures in persons with rheumatoid arthritis, lupus erythematosis, osteoarthritis, pyrophosphate arthropathy, renal disease, osteoporosis and joint replacements, and in older patients who have no apparent musculoskeletal disease.[6,8,11-15] Most of these patients do not have a history of increased activity, but many have decreased bone density. Stress fractures in persons with decreased bone density are referred to as insufficiency fractures.
