Support Organizations Nephrotic syndrome Nephrotic syndrome is a rare kidney disorder that causes high levels of protein in the urine (proteinuria) and low levels of protein in the blood. It may cause high blood pressure, high cholesterol, and swelling around the eyes, arms, legs, and feet. Symptoms of nephrotic syndrome include loss of appetite, unexpected weight gain, abdominal pain, and weakness. Common causes of nephrotic syndrome include kidney diseases as well as other diseases and conditions that affect the kidneys, including diabetes, systemic lupus erythematosus (SLE), and hepatitis C. Allergic reactions to insect bites, poisonous plants, and medications can also lead to nephrotic syndrome. Nephrotic syndrome can affect both adults and children. It can often be cured by removing or treating the cause (for instance, stopping use of a medication). If the cause is not treatable or is not known, medications may be used to reduce the symptoms of nephrotic syndrome. If the problem causing nephrotic syndrome is not corrected, the disorder can progress to kidney failure.

    What are the different types of nephrotic syndrome?
    The type of nephrotic syndrome that is most common in children is called idiopathic nephrotic syndrome. Idiopathic means that a disease occurs with no known cause. The most common type of idiopathic nephrotic syndrome is called minimal-change nephrotic syndrome (MCNS). Idiopathic nephrotic syndrome is more common in boys than girls.
    With MCNS, the child usually has relapses of the illness, but the disease can usually be managed and prognosis is good. In rare cases, a child may develop kidney failure that requires dialysis.
    There is a rare nephrotic syndrome present in the first week of life called "congenital nephrotic syndrome." Congenital nephrotic syndrome is inherited by an autosomal recessive gene, which means that males and females are equally affected, and the child inherited one copy of the gene from each parent, who are carriers. The chance for carrier parents to have a child with congenital nephrotic syndrome is one in four, or 25 percent, with each pregnancy. The outcome for this type of nephrotic syndrome is extremely poor.

    Several illnesses may cause the syndrome Most children make a full recovery Complications can occur These diseases can damage the microscopic filtering units in the kidney called the glomeruli, causing the loss of protein from the body which in turn leads to the symptoms of nephrotic syndrome. The glomeruli clean the blood and make urine. Normally the glomeruli prevent the passage of protein from the blood out into the urine, and in adults less than 150mg of protein is lost each day. But in nephrotic syndrome the glomeruli become leaky to protein and more than 3.5g - that's 25 times the normal amount - is passed out every 24 hours (the equivalent in children is 0.05g/kg body weight/24 hours). Loss of protein leads to:
    • low levels of protein in the blood accumulation of salt and water leading to swelling of the tissues

The main symptom is oedema ('fluid retention') mainly due to the low protein level in the blood. Various kidney disorders can cause nephrotic syndrome, some more serious than others. Treatment and outcome (prognosis) vary, depending on the cause.
    Understanding kidneys and urine The two kidneys lie to the sides of the upper abdomen (the loins), behind the intestines, and either side of the spine. Each kidney is about the size of a large orange, but bean-shaped. A large renal artery takes blood to each kidney. The artery divides into many tiny blood vessels (capillaries) throughout the kidney. In the outer part of the kidneys tiny blood vessels cluster together to form structures called glomeruli. Each glomerulus is like a filter. The structure of the glomerulus allows waste products and some water and salt to pass into a tiny channel called a tubule while keeping blood cells and protein in the bloodstream. Each glomerulus and tubule is called a 'nephron'. There are about one million nephrons in each kidney.
