Mary Rourke, a 55-year old teacher from Salem, N.H., used to nod off all the time as a child, but people just shrugged and said, “Oh, she must be very tired,” she recalls. Then, as an adult, she began having attacks in which her muscles would lose tone and she’d fall– every time she laughed or felt any strong emotion. “I was constantly falling,” she says. “If you told me a joke, I’d flip. I couldn’t be around people.” In Massachusetts, a 49-year old nurse from Norwood who asked that her name not be published also used to crash helplessly to the floor whenever she laughed or got angry, a fact her children quickly learned to exploit. “I couldn’t yell at my kids when they were younger because if I got too mad, I couldn’t stand up,” she says. Things were even worse for Bob Cloud, a 58-year old lawyer from Cincinnati, Ohio. It was bad enough falling asleep talking to judges, he says, but one day he went limp while swimming and had to be rescued. At least, he says, that provided a “great educational opportunity” to tell stunned onlookers what was really wrong: A brain disorder called narcolepsy, characterized by extreme daytime sleepiness and caused by low levels of a brain chemical called hypocretin. In many cases, low hypocretin levels also cause cataplexy, sudden loss of muscle tone due to the intrusion of dreaming (REM) sleep in the waking state.

STANFORD RESEARCHERS NAB GENE FOR SLEEP DISORDER
STANFORD - After a decade-long search, a Stanford-led team has identified a gene that causes the sleep disorder narcolepsy a breakthrough that brings a cure for this disabling condition within reach, the scientists say. The researchers, led by Emmanuel Minot, MD Ph.D. , associate professor of psychiatry at Stanford University School of Medicine, used a technique called positional cloning to pinpoint the gene in dogs, one of the few species besides humans known to develop narcolepsy. Minot predicts the finding will not only help roughly 135,000 Americans who suffer from narcolepsy, but in time it will shed light on two of the biggest mysteries in sleep research: how and why we sleep. "This discovery is the exciting culmination of 36 years of research on narcolepsy at Stanford," said William Dement, MD, Ph.D., director of the Sleep Disorders Research Center at Stanford.

    Overview Narcolepsy is a chronic sleep disorder characterized by overwhelming daytime drowsiness and sudden attacks of sleep. People with the disorder often find it difficult to stay awake for long periods of time, regardless of the circumstances. Narcolepsy comes from the Greek word narke , meaning "stupor." Narcolepsy is a neurological disorder, which causes excessive sleepiness. This condition usually develops in younger individuals, and is lifelong. In addition to excessive sleepiness, patients with narcolepsy sometimes experience muscle weakness during strong emotion (cataplexy).
Symptoms:
People with narcolepsy fall asleep unexpectedly many times
during the day. Not only can this be embarrassing, but it can be very dangerous.
The Cause is a nervous system disorder involving the REM phase of sleep. The person may doze on and off throughout the day.
Other symptoms of Narcolepsy
Extreme sleepiness
Hallucinations just before or after sleep
Muscle weakness when emotions are strong
Inability to move for several minutes after waking up.
A Daytime Study (MSLT) called a Multiple Sleep Latency Test

Narcolepsy is a disorder that can be brought on by intense emotion and cause uncontrollable episodes of falling asleep during the normal waking hours. Approximately 125,000 people in the United States are affected by narcolepsy. This disorder usually begins in the teens or early twenties but can be seen in children or the elderly. The main symptoms of narcolepsy are:
  • Excessive daytime sleepiness and "sleep attacks" are the most common symptom of narcolepsy. A sleep attack is suddenly and unexpectedly falling asleep and can last for only a few minutes or up to an hour.
    Cataplexy , a secondary symptom of narcolepsy, is the sudden, temporary loss of muscle strength. Cataplexy can be so severe that it can lead to complete collapse. During a cataplectic attack the person is completely awake and can stay in a collapsed state for 15 to 30 minutes.
    Hypnogogic hallucinations are another symptom of narcolepsy and are similar to nightmares. These hallucinations occur upon awakening and falling asleep. They can be visual, auditory, or tactile and last for a few minutes. Hypnogigic hallucinations occur in only five percent of the narcoleptic population and can be misdiagnosed as schizophrenia.

Narcolepsy is a rare disorder characterized by chronic, excessive attacks of drowsiness during the day, sudden extreme muscle weakness (cataplexy), hallucinations, paralysis while sleeping, and disrupted sleep during the night. Attacks of drowsiness may persist only a few minutes or last for hours, and may vary in frequency from a few incidents to several during a single day. Although the exact cause of narcolepsy is not known, many researchers suspect that genetic factors play a role in the development of the disorder.
