    WHAT IS NARCOLEPSY?
    The word narcolepsy comes from two Greek words that can be roughly translated as seized by numbness. The two primary symptoms in narcolepsy reflect this phrase:
    • Excessive daytime sleepiness, with frequent daily sleep attacks or a need to take several naps during the day.
      Temporary and sudden muscle weakness (called cataplexy ), usually brought on by strong emotions.
    Some, but not all patients experience other symptoms:
    • Microsleep episodes, in which the patient behaves automatically but without conscious awareness.
      A sense of paralysis that occurs between wakefulness and sleep (called atonia
      Dream-like states between waking and sleeping (called hypnagogic hallucinations
      Periodic leg movements during sleep ( periodic limb movement disorder
    REM (rapid eye movement) sleep is abnormal in narcolepsy. In fact, narcolepsy is sometimes defined as the loss of boundaries between wakefulness, non-REM sleep, and REM sleep. [For a full definition of REM and nonREM sleep, see box Healthy Sleep.]

    Overview Symptoms Treatment Prevention Definition:
    Narcolepsy is a sleep disorder associated with uncontrollable sleepiness and frequent daytime sleeping.
    Alternative Names: Daytime sleep disorder
    Causes, incidence, and risk factors: The exact cause of narcolepsy is unknown. Studies have indicated that the disorder may be genetic, with studies showing gene markers that may indicate a tendency to develop narcolepsy. A small group of neurons in the brain has been implicated in producing transitions from sleep to wakefulness and vice-versa, and people with narcolepsy may have fewer of these neurons or they may have been damaged. The condition may be aggravated by conditions that cause insomnia , such as disruption of work schedules.
    Narcolepsy is characterized by episodes of frequent, uncontrollable daytime sleeping, usually preceded by drowsiness . The episodes usually occur after meals, but sudden onset of sleep may occur while working or driving a vehicle, having a conversation, or being in any sedentary or nonstimulating situation.

    Narcolepsy is a chronic (long lasting) disease of the brain (central nervous system). Excessive daytime sleepiness (EDS) is the main symptom and is present in 100% of patients with narcolepsy. The other primary symptoms of narcolepsy include loss of muscle tone (cataplexy), distorted perceptions ( hypnagogic hallucinations), and inability to move or talk ( sleep paralysis). Additional symptoms include disturbed nocturnal sleep and automatic behavior. All of the symptoms of narcolepsy may be present in various combinations and degrees of severity. Narcolepsy begins (has its onset) usually in teenagers or young adults and affects both sexes equally. The first symptom to appear is EDS, which may remain unrecognized for a long time in that it develops gradually over time. The other symptoms can follow EDS by months or years.

    During sleep, we usually pass through five phases of sleep: stages 1, 2, 3, 4, and REM (rapid eye movement) sleep. These stages progress in a cycle from stage 1 to REM sleep, then the cycle starts over again with stage 1. We spend almost 50 percent of our total sleep time in stage 2 sleep, about 20 percent in REM sleep, and the remaining 30 percent in the other stages. The neurological disorder is characterized by excessive sleepiness that is typically associated with cataplexy (a sudden loss of muscle tone and paralysis of voluntary muscles that is associated with a strong emotion) and other REM sleep abnormalities such as sleep paralysis, (Imobility of the body that occurs in the transition from sleep to wakefulness.) and hypnagogic hallucinations ( pre-sleep dreams). What are the symptoms?
    • excessive sleepiness or sudden muscle weakness
    • cataplexy (a sudden loss in muscle tone and deep tendon reflexes leading to muscle weakness, temporary paralysis or a complete postural collapse. Cataplexy is usually brought on by an outburst of emotion - notably laughter, anger or startle.)
    • sleep paralysis
    • hypnagogic hallucinations
    • automatic behaviors (like driving home and not remembering how you got there!)

    People with narcolepsy fall asleep anywhere, at any time without warning. It has been linked to hormones and nervous system activity. Researchers from Australia's Flinders Medical Centre found antibodies from narcolepsy patients triggered the disorder in mice. It is hoped The Lancet study may lead to better tests and treatments. The finding of this antibody may be the missing link which explains the cause of narcolepsy
    Dr Andrew Cummin The researchers induced narcolepsy-like symptoms in the muscles of mice by injecting them with antibodies from the blood of nine people with the disorder. Mice injected with antibodies from nine people who did not have narcolepsy did not develop such symptoms. Antibodies are produced by the immune system to attack infections and other foreign invaders in the body.

    People with narcolepsy experience an uncontrollable need to sleep A chemical found naturally in the brain could be used to treat the sleep disorder narcolepsy, US scientists say.
    Researchers at the University of Texas said injecting a chemical called orexin stopped symptoms in mice with narcolepsy. They found the treatment made the mice more alert and reduced other narcoleptic symptoms, such as muscle weakness, called cataplexy. The study appeared in the Proceedings of the National Academy Sciences. Dr Masashi Yanagisawa, professor of molecular genetics, and colleagues studied genetically modified mice which lacked the ability to naturally produce orexin. Orexin are small chains of molecules, produced by nerve cells in the area of the brain called the hypothalamus. No rebound sleep It is thought that people with narcolepsy are unable to produce this chemical, due to cells in the brain that produce it being damaged. The researchers injected the modified mice with orexin peptides. They found it increased wakefulness, regular sleep patterns and stopped cataplectic fits.

    Sleeping is an essential part of everyone's life yet it remains little understood is barely understood. You might think it's a relaxing recharge but in fact your brain is working harder at times overnight than when you're conscious in the day.

    Dr Thomas Kilduff, Stanford Research Institute International Fresh insight into why and how we sleep has come from studying people with sleep disorders, especially sufferers of narcolepsy. The condition means that people fall asleep many times a day, completely out of the blue. A less known symptom is paralysing attacks, that can cause narcoleptics to fall to the ground - unable to move - several times a day. If a way can be found to ease their symptoms, it could open the way to helping any of us to control our sleep patterns and perhaps even to go without rest while staying alert. Gaynor Carr has been nodding off routinely since the age of seven. Her narcolepsy has made holding down a job impossible and made her question the idea of ever having children. Gary Beattie used to work in construction, until he fell asleep 7m up a ladder. He not only loses consciousness, his body becomes paralysed in a so-called cataleptic attack. Both of them say that showing emotion sparks the paralysing attacks and that has forced them to avoid laughing and crying. Bill Baird worked in finance but describes his stockbroking days as a race. The emotion of closing a deal would bring on a fit; he had constantly to hope he could get a client's signature before his almost inevitable collapse. His sleep is restless, with vivid nightmares when he is able to hear his surroundings while seeing terrifying hallucinations.
