modafinil Provigil ', 'Alertec', 'Vigicer', 'Modalert', etc) is a memory-improving and mood-brightening psychostimulant. It enhances wakefulness and vigilance , but its pharmacological profile is notably different from the amphetamines methylphenidate Ritalin ) or cocaine . Modafinil is less likely to cause jitteriness, anxiety, or excess locomotor activity - or lead to a hypersomnolent 'rebound effect' - than traditional stimulants . Subjectively, it feels smoother and cleaner than the amphetamines too. The normal elimination half-life of modafinil in humans is between 12 - 15 hours . So it's worth fine-tuning one's dosage schedule accordingly. Current research suggests modafinil, like its older and better-tested analogue adrafinil , is a safe, effective and well-tolerated agent. It is long-acting and doesn't tend to cause peripheral sympathetic stimulation. Yet its CNS action isn't fully understood. Modafinil induces wakefulness in part by its action in the anterior hypothalamus . Its dopamine -releasing action in the nucleus accumbens is weak and dose-dependent; the likelihood of a euphoric response ('abuse potential'), dose-escalation and tolerance is thus apparently small. We have found the first evidence of neuronal degeneration in narcolepsy.
After hearing the punch line of the joke, the teenager falls to the floor, almost as if actually punched. She remains there, completely unable to move. She hears her parents reassure her friends that they need not worry about her because she will be all right in a few minutes. She is embarrassed and frustrated as the episode continues, and her friends begin to leave. They bid her goodbye, but she is unable to respond. Although she cannot talk or move, she is otherwise in a state of high alertness, feeling, hearing and remembering everything that is going on around her. The episode lasts for five minutes, longer than her typical cataplexieswhich often last only secondsbut shorter than her longest episode, which lasted 25 minutes. Then it ends, almost as abruptly as it began. She gets up from the floor, and her everyday life resumes. Cataplexy, the loss of skeletal muscle tone without loss of consciousness, is one of the defining symptoms of a puzzling neurological disorder called narcolepsy. The cataplectic attacks of narcolepsy are frequently prompted by laughter; other times, embarrassment, social interactions with strangers, sudden anger, athletic exertion or sexual intercourse may trigger an episode.

Winter, 2000 The cause of the death of the hypocretin cells is not entirely clear, but there are two possibilities. One is that a specific "poison" produced by the body or absorbed through the environment kills these cells. Parkinson's disease, another neurological disease caused by the loss of specific brain cells, can be triggered by ingested toxins, for instance. The second and more likely possibility is that the hypocretin cells are killed by the immune system, which confuses these cells with an infectious agent such as a virus. Other cells near where the hypocretin cells should have been appear to be normal; this indicates that whatever the cause of the cell loss, the hypocretin cells are targeted very specifically. What does finding the cause of narcolepsy mean for people with narcolepsy? This discovery ends the 120-year search for the cause of this debilitating disease. It also has important implications for people with narcolepsy: 1. We have clearly shown that narcolepsy is a neurological disease not a psychiatric one. The problem in not "in your mind," but rather is caused by damage to your brain, specifically to the hypothalamus. Many persons with narcolepsy have not only had to suffer from the disease but also from family members, friends and even physicians, who thought of narcolepsy as a motivational problem. Only the ignorant will now consider narcolepsy a psychological problem.

What is narcolepsy?
Narcolepsy is a chronic , or long-lasting, sleep disorder with no known cause. It affects the body's central nervous system, which is made up of nerves that carry messages from the brain to other parts of the body. When a person has narcolepsy, messages about when to sleep and when to be awake can get mixed up. This can cause a person to fall asleep when they do not want to, and often without any warning like feeling drowsy. The desire to sleep can be overwhelming and hard to resist, and can happen to a person several times during the day. Night sleep may also be poor, broken up by waking up often during the night. If not controlled with medication, narcolepsy can cause serious problems in a person's personal, social, and work life. It can also limit a person's activities, such as driving a car, work, and exercising. Studies indicate that narcolepsy may run in families.

Imagine listening to a funny joke, then in the middle of laughing, your legs suddenly buckle and you are unable to move for the next several minutes. Maybe you are in class, but you are incredibly drowsy and finally just can't stay awake even though you had plenty of sleep the night before. These are examples of what people with narcolepsy experience. People with narcolepsy have trouble staying awake. Narcoleptic often feel drowsy and fall asleep, even in the middle of the day, sometime in the middle of activities that produce strong emotions. It is thought that narcolepsy occurs when rapid eye movement (REM) sleep occurs while people are awake. Although the cause of narcolepsy is unknown, it does occasionally run in families. If one person in a family has the disorder, another relative may be susceptible to developing it if certain environmental triggers occur.
Symptoms
  • Excessive daytime sleepiness: people may fall asleep when they are doing a boring task or when they are working alone. These sleep attacks may become more frequent and occur at inappropriate and dangerous times such as when they are driving a car.
  • Cataplexy: sudden loss of muscle tone. Loss of muscle tone in the legs may cause a person to fall down. In people with normal sleep/wake functioning, loss of muscle tone is associated exclusively with REM sleep.

What you've seen in the movies is partly true narcoleptics do suddenly fall asleep, but most are treated with medication so that this doesn't happen. Symptoms of Narcolepsy
  • Excessive daytime sleepiness - the overwhelming urge to fall asleep during the day, even after having plenty of sleep the night before
  • Cataplexy - a sudden, usually brief attack of general muscle weakness related to a strong emotional response (fear, anger, laughter)
  • Hypnagogic sleep paralysis - brief episodes of paralysis that occur when falling asleep
  • Hypnopompic sleep paralysis - brief episodes of paralysis that occur when waking up
  • Hypnagogic hallucinations - vivid, usually visual or auditory hallucinations that occur at the onset of sleep (sometime between falling asleep and the actual sleep state)
