    Therapy and the Role of Clinical Diagnostic Criteria
    In my own face-to-face and online therapy practice , I do not treat clients as manifestations of mental illnesses in need of medical fixing. No one is merely a case of bipolar disorder , an obsessive-compulsive , or a paranoid schizophrenic . In my view, formal diagnostic categories hold some value in terms of contextual information, but they play a much less important role than the specific personal circumstances of a given client. If you are a client, you are first and foremost a specific, unique individual. Nonetheless, in my experience of working with clients who have received formal psychiatric diagnoses from other mental health professionals, I have often found that clients feel they lack adequate information about what these diagnostic labels actually mean. The entries in this section aim to provide the basic facts about each of several diagnostic labels. Husband with Narcissistic Personality Disorder
Nov 2004 It's very hard to overcome the influence of Narcissistic Personality Disorder. It's my understanding that that's one reason that many people leave the NPD spouse - to provide an entirely healthy environment for the child(ren). I completely sympathize with your desire to avoid unsupervised time for your husband with your kids: I didn't ID the problem until post- divorce, but I do remember that nobody believed that my ex- husband was anything but charming and loving, even when there was ample evidence of neglect (ignoring the child's needs in order to fulfill the adult's need for constant ''supply''). So maybe waiting awhile is a good idea . . . but not a long time. The thing is, your kids will model your behavior (in response to your husband) without understanding the reason for it - that you're compensating for an ill partner. That, combined with the NPD itself, gives them nothing healthy to imitate. To the extent possible, you need to give them a healthy emotional model, something positive enough and consistent enough to counter the pervasive influence of the NPD. That will give them a base from which to understand that their dad isn't entirely well, that his perspective isn't the only valid world view, and that his behaviour isn't necessarily the way that they should behave themselves.

    The figures seem to indicate that a minimum of 1% (probably 3% and perhaps up to 5%) of the population above the age of 10 are narcissists. Now, factor in the parents, spouses, colleagues, friends, children, the children's families ... This is the biggest underdiagnosed mental health pathology ever.     Narcissistic personality disorder (NPD) is a (Inflexible and maladaptive patterns of behavior) personality disorder that is characterized by extreme feelings of self-importance, a high need for admiration, and a lack of empathy. NPD can be considered as a pathological form of (An exceptional interest in and admiration for yourself) narcissism . It is estimated that 0.7-1% of the general population are afflicted with NPD. Most people discover this personality or behavioral disorder the hard way through the living hell it makes of one's life. Referred to as Narcissistic Personality Disorder (NPD) by most in the medical and social fields, the nature of this disorder is difficult to diagnose and to date, impossible to cure.

    Narcissism refers to collection of personality/behavior disorders that if you discover it, just walk away. Narcissists cannot love like normal people, but they can sap all the love out of everyone around them and create a living nightmare for all within their reach. For myself, I first discovered the devastation narcissism imparts on the self-esteem of those unlucky enough to find themselves in a relationship with this type of personality during college.       Hospitalization
      The hospitalization of patients with severe Narcissistic Personality occurs frequently. For some, such as those who are quite impulsive or self-destructive, or who have poor reality-testing, this is the result of Axis I symptoms which are overlaid upon the personality disorder. Hospitalizations should be brief, and the treatment specific to the particular symptom involved. Another group of patients for whom hospitalization is indicated, provided long-term residential treatment is available, are those who have poor motivation for outpatient treatment, fragile object relationships, chronic destructive acting out, and chaotic life-styles. An inpatient program can offer an intensive milieu which includes individual psychotherapy, family involvement, and a specialized residential environment. The structure is physically and emotionally secure enough to sustain the patient with severe ego weakness throughout the course of expressive, conflict-solving psychotherapy. Small staff-patient groups within the wards, as well as large community meetings, at which feelings are shared and patients' comments taken seriously by staff, and constructive work assignments, recreational activities, and opportunities to sublimate painfully conflictual impulses make the hospital a "holding" environment rather than merely a containing one. The ultimate goals are of effecting a better integrated internal world, more cohesive and modulated self-object representation, and a self-concept less vulnerable to narcissistic injury.

    Vulnerability in self-esteem makes individuals with this disorder very sensitive to "injury" from criticism or defeat. Although they may not show it outwardly, criticism may haunt these individuals and may leave them feeling humiliated, degraded, hollow and empty. They may react with disdain, rage, or defiant counterattack. Interpersonal relations are typically impaired due to problems derived from entitlement, the need for admiration, and the relative disregard for the sensitivities of others.
