Management of the anemia caused by ineffective erythropoiesis is difficult. In patients with myelodysplastic syndromes and symptomatic anemia, we evaluated the safety and hematologic activity of lenalidomide, a novel analogue of thalidomide. Methods Forty-three patients with transfusion-dependent or symptomatic anemia received lenalidomide at doses of 25 or 10 mg per day or of 10 mg per day for 21 days of every 28-day cycle. All patients either had had no response to recombinant erythropoietin or had a high endogenous erythropoietin level with a low probability of benefit from such therapy. The response to treatment was

One of the most challenging problems in hematology is the heterogeneous group of disorders that were formally defined as myelodysplastic Group in 1982. This set of disorders includes idiopathic conditions as well as the secondary or therapy-related forms that develop after exposure to alkylating agents, radiation, or both. Idiopathic myelodysplastic syndromes occur mainly in older persons: the incidence of these syndromes is about 5 per 100,000 persons per year in the general population, but it increases to 20 to 50 per 100,000 persons per year after 60 years of age. Myelodysplastic syndromes (MDS)
DESCRIPTION: A heterogeneous group of acquired hematopoietic stem cell disorders, characterized by cytologic dysplasia in the bone marrow and blood and by various combinations of anemia, neutropenia, and thrombocytopenia
  • There is a natural progression of disease between categories as cellular maturation becomes more arrested and blast cells accumulate. There is a great deal of overlap between arbitrary diagnostic subgroups.
  • Refractory cytopenia - same as RA but with leukopenia or thrombocytopenia without anemia
  • Refractory cytopenia with multi-lineage dysplasia (RCMD) - marked trilineage dysplasia but without excess of blasts
  • Acute MDS with sclerosis - RAEB with marked myelosclerosis
  • Therapy-related (t-MDS) - seen 3-7 years after treatment with alkylating agents and/or radiation therapy. Evolves to acute myeloid leukemia (AML) over about 6 months.

System(s) affected: Hemic/Lymphatic/Immunologic
Genetics: Most are clearly clonal neoplasms by cytogenetics, G6PD isoenzyme analysis, or RFLP analysis. Mutations in RAS oncogene have been reported.

Myelodysplastic syndromes, sometimes called "pre-leukaemia" are a group of diseases in which the bone marrow does not produce enough normal blood cells. Common symptoms are anaemia, bleeding, easy bruisability, and fatigue. These Myelodysplastic syndromes can occur in all age groups but are more common in people aged over 60. Myelodysplastic syndromes may develop spontaneously or be secondary to treatment with chemotherapy / radiotherapy. There is an association with Myelodysplastic syndromes and acute myeloid leukaemia.

Myelodysplastic Syndromes/Myeloproliferative Disorders
Myelodysplastic syndromes (MDS) and myeloproliferative disorders are diseases of the blood and bone marrow. The National Cancer Institute (NCI) says myelodysplastic syndromes are the result of not enough healthy blood cells being produced. Myeloproliferative disorders are the result of too many blood cells being produced.
Myelodysplastic syndromes (MDS)
The American Cancer Society (ACS) estimates that each year 10 to 15 thousand people are diagnosed with MDS. Most of them are over 60 years of age and more men than women are affected. MDS has not always been considered a cancer, according to ACS but is now being considered a form of cancer. ACS also says about 30 percent of MDS cases progress to become acute myeloid leukemia. (Read about " Leukemia ") NCI says basically the bone marrow does not make enough healthy blood cells. The key word there is healthy. Many of the cells that are created are abnormal. Normally, the bone marrow produces stem cells (immature cells) that develop into mature blood cells. There are 3 types of mature blood cells:
  • Red blood cells that carry oxygen and other materials to all tissues of the body

    Myelodysplastic Syndromes
    Group of acquired clonal disorders affecting the haemopoietic system involving a defect in an early progenitor cell and therefore MDS can affect many cells. Previously have been referred to as pre-leukaemias, smouldering leukaemias and refractory anaemias. In many forms of MDS there are chromosomal abnormalities particularly including deletions of chromosomes 5 and 7 and trisomy 8. Primary acquired sideroblastic anaemia caused by disruption in haem synthesis (associated with consumption of alcohol, isoniazid or pyrazinamide) in a particular form of MDS. Epidemiology

    Blood film and FBC (useful to see results over time), all cases show anaemia either normocytic or macrocytic. Blood film characteristically shows dysplasia as erythrocytes with anisocytosis (varying sizes), poikilocytosis (abnormal shape) and basophilic stippling. Polymorphs are commonly hypolobulated and may show characteristic bilobed appearance. May also be hypogranulation and abnormal chromatin.

Leukemias Myelodysplastic Syndromes (MDS) Myelodysplastic syndromes (MDS) are a group of diseases that cause immature blood cells (called blasts) to accumulate in the bone marrow leading to a shortage of mature blood cells, including red blood cells, white blood cells, and platelets. Furthermore, the mature blood cells that are made can also be defective. MDS are not cancer themselves, but about 30% of the time the syndromes are a precursor to leukemias such as  acute myeloid leukemia (AML)  and  chrome myelomonocytic leukemia (CMML) Although MDS can affect people of all ages, the median age of onset is 65. MDS affects less than 15,000 people each year in the United States, and includes the following syndromes:
    • 5q syndrome (named after the chromosomal defect that causes the symptoms) Hypoplastic MDS MDS with myelofibrosis MDS with prominent eosinophilia or monocytosis
    Causes of MDS
    People who have received radiation therapy, chemotherapy with alkylating agents (such as chlorambucil, cyclophosphamide, and melphalan), or who have been exposed to industrial solvents (such as benzene) have a higher risk of developing MDS than people who have not had these exposures. Rarely, genetic disorders are responsible for the disease. Nevertheless, in 60% to 70% of MDS patients, no specific cause can be identified.

    The Stanford MDS Center has been recognized by the MDS Foundation as an MDS Center of Excellence. In addition, physicians in the Center participate in developing national treatment guidelines, serve on the boards of MDS organizations, and are actively involved in clinical trials for MDS.
