General Discussion Transverse myelitis is a neurological disorder of the spine caused by inflammation across the spinal cord. It is sometimes associated with the term myelopathy, which refers to any disorder of the spinal cord. However, transverse myelitis is a more specific term for inflammation (myelitis) across the width of the spinal cord (transverse) that results in changed function below this level while function remains normal above. Symptoms are related to movement and sensory functions. This disorder occurs in both adults and children, and typically begins with a rather rapid development of symptoms over the course of several hours, days, or weeks. Symptoms may include lower back pain, weakness in the legs and arms, sensory disturbance, spasms leading to gradual paralysis, and bowel or bladder disfunction.

Transverse myelitis
Definition
Transverse myelitis (TM) is an uncommon neurological syndrome caused by inflammation (a protective response which includes swelling, pain , heat, and redness) of the spinal cord, characterized by weakness, back pain, and bowel and bladder problems. It affects one to five persons per million. Description
TM affects the entire thickness of the spinal cord, producing both sensory and movement problems. It is believed to be linked to the immune system, which may be prompted to attack the body's own spinal cord. Striking rapidly without warning, its effects can be devastating.
Transverse myelitis has many different causes, often triggered by a variety of viral and bacterial infections (especially those associated with a rash such as measles or chickenpox). Once the infection subsides, the inflammation in the cord begins. About a third of patients experience a flu-like illness with fever about the time they develop symptoms of TM. Sometimes, there appears to be a direct invasion of, and injury to, the spinal cord by an infectious agent (such as herpes zoster or the AIDS virus).

Transverse myelitis is a neurologic disorder caused by a loss of the myelin encasing the spinal cord , also known as demyelination . This demyelination arises idiopathically following infections or vaccination , or due to multiple sclerosis . One major theory of the cause is that an immune-mediated inflammation is present as the result of exposure to a viral antigen The lesions are inflammatory , and involve the spinal cord on both sides. With acute transverse myelitis, the onset is sudden and progresses rapidly in hours and days. The lesions can be present anywhere in the spinal cord, though it is usually restricted to only a small portion. In some cases, the disease is presumedly caused by viral infections or vaccinations and has also been associated with spinal cord injuries, immune reactions, schistosomiasis and insufficient blood flow through spinal cord vessels. Symptoms include weakness and numbness of the limbs as well as motor, sensory, and sphincter deficits. Severe backpain may occur in some patients at the onset of the disease. Treatment is usually symptomatic only, corticosteroids being used with limited success. A major differentiation or distinction to be made is a similar condition due to compression of the spinal cord in the spinal canal, due to disease of the surrounding vertebral column.

Subject: Acute Transverse Myelitis or GBS?
How do neurologists distinguish between the symptoms/test results of acute transverse myelitis as opposed to the acute onst of GBS? What "pictures" should the MRI's, spinal tap or EMG show to distinguish between the two? What about the presence or absence of deep tendon reflexes or Babinsky sign? Can GBS spinal tap results also have high cell count as well as high protein count? What if parasthesias were in the back, not the legs? I really appreciate all the info this BB has provided, although often it has led to other questions? The doctors who respond have been great!
In comparing the two conditions it may be less obvious in the early stages
of Transverse myelitis but as the process evolves things become more clear cut.
Reflexes : Brisk in transverse myelitis, absent in GBS
