Home Transverse Myelitis is a rare neurological disorder that is part of a spectrum of neuroimmunologic diseases of the central nervous system. Other disorders in this spectrum include, Acute Disseminated Encephalomyelitis (ADEM), Optic Neuritis, and Neuromyelitis Optica (Devic's disease). The membership of The Transverse Myelitis Association includes persons with all of these disorders, their family members and caregivers and the medical professionals who treat people with these disorders. The Transverse Myelitis Association was established in 1994 as an organization dedicated to advocacy for those who have these rare neuroimmunologic diseases. It was incorporated as a not-for-profit organization in 1996.
Mission
The Transverse Myelitis Association facilitates support and networking opportunities amongst families; provides educational information; functions as a clearinghouse for articles and research literature; and investigates, advocates for and supports research and innovative treatment efforts.
Programs
The TMA publishes a newsletter for our members twice a year, which contains articles by physicians and health care providers. There are also articles written by people with the rare neuroimmunologic disorders. A membership directory is published and distributed annually. The TMA assists in the development of local support groups and is involved in various fund-raising efforts, such as Reading for Rachel, to raise awareness and funds for research. The TMA supports and conducts various symposiums and workshops involving both professionals and patients for the exchange of information regarding research and treatment strategies for persons with the rare neuroimmunologic disorders of the central nervous system.

Transverse Myelitis: Symptoms, Causes and Diagnosis

Transverse myelitis (TM) is a neurologic syndrome caused by inflammation of the spinal cord. TM is uncommon but not rare. Conservative estimates of incidence per year vary from 1 to 5 per million population ( Jeffery, et.al., 1993 ). The term myelitis is a nonspecific term for inflammation of the spinal cord; transverse refers to involvement across one level of the spinal cord. It occurs in both adults and children. You may also hear the term myelopathy, which is a more general term for any disorder of the spinal cord. Clinical Symptoms TM symptoms develop rapidly over several hours to several weeks. Approximately 45% of patients worsen maximally within 24 hours ( Ibid. ). The spinal cord carries motor nerve fibers to the limbs and trunk and sensory fibers from the body back to the brain. Inflammation within the spinal cord interrupts these pathways and causes the common presenting symptoms of TM which include limb weakness, sensory disturbance, bowel and bladder dysfunction, back pain and radicular pain (pain in the distribution of a single spinal nerve). Almost all patients will develop leg weakness of varying degrees of severity. The arms are involved in a minority of cases and this is dependent upon the level of spinal cord involvement. Sensation is diminished below the level of spinal cord involvement in the majority of patients. Some experience tingling or numbness in the legs. Pain (ascertained as appreciation of pinprick by the neurologist) and temperature sensation are diminished in the majority of patients. Appreciation of vibration (as caused by a tuning fork) and joint position sense may also be decreased or spared. Bladder and bowel sphincter control are disturbed in the majority of patients. Many patients with TM report a tight banding or girdle-like sensation around the trunk and that area may be very sensitive to touch.

Transverse myelitis is a neurological disorder caused by inflammation across both sides of one level, or segment, of the spinal cord. The segment of the spinal cord at which the damage occurs determines which parts of the body are affected. Damage at one segment will affect function at that segment and segments below it. In people with transverse myelitis, inflammation usually occurs at the thoracic (upper back) level, causing problems with leg movement and bowel and bladder control, which require signals from the lower segments of the spinal cord.

Acute Transverse Myelitis
A syndrome, not a disease, in which acute inflammation affects gray and white matter in one or more adjacent thoracic segments. Often, the cause is unknown, but some cases follow nonspecific viral infection or vaccination, suggesting an immunologic cause; others are associated with vasculitis, use of amphetamines or IV heroin, Lyme disease, syphilis, TB, or parasitic or fungal agents.
Symptoms and Signs
Usually, ascending weakness and numbness of the feet and legs and difficulty voiding develop over a few days; they may progress over several more days to become severe, usually with global sensorimotor paraplegia below the lesion, urinary retention, and loss of bowel control. Occasionally, posterior column functions are spared, at least initially. Local back pain, headache, and stiff neck may be present. The syndrome occasionally recurs.
Diagnosis and Treatment
CSF examination may show monocytes and a slightly increased protein content. MRI can rule out expanding extramedullary lesions. MRI or myelography may show swelling of the cord and occasionally a subarachnoid block at the level of the lesion. If myelography is used, supine and prone visualization is necessary to rule out a vascular malformation. Blood serologies are abnormal in rare cases associated with collagen vascular disease.

The Johns Hopkins Transverse Myelitis Center (JHTMC)
was established in October 1999 as a multidisciplinary center dedicated to the diagnosis and treatment of acute transverse myelitis (ATM) and to better understand the pathophysiology and natural course of the disease.
The Center of Excellence has several explicit goals:
  • To provide expert care and treatment for patients in the acute phase of TM. To extend the spectrum of care through collaborative management of the disease condition by health care providers from multiple disciplines: neurology, psychiatry, urology, physical medicine and rehabilitation, rheumatology, physical and occupational therapy, neuroradiology, and neurosurgery. To develop standard diagnostic criteria and work-up for patients based on clinical research to understand the natural course of the disease.
  • To perform clinical and basic science research in order to comprehend the pathophysiology of TM.

    In keeping with the goals and objectives of the JHTMC, we have numerous research studies that are currently underway. We have evaluated 450 patients since the center was established. An extensive clinical database has been established on all evaluated patients.
    We are studying epidemiological and clinical markers that differentiate pediatric and adult TM . Spinal fluid, DNA, peripheral and CSF leukocytes and serum have been collected and stored on selected patients. We have a clinical repository of blood and CSF of patients both in the acute and convalescent phase.This database also serves as a clinical repository for future clinical studies and trials.
    Since patient outcomes vary widely despite clinically similar acute disability, we have attempted to define biomarkers of disease which could predict sustained disability. We have examined CSF from over 80 patients in the acute phase of TM and found that the presence of 14-3-3 protein strongly predicts a poor recovery. We are currently examining lymphocyte populations and cytokine/chemokine profiles within the serum and CSF of patients with TM and as indicated above, have three submitted manuscripts reflecting this work. Another biomarker of disease activity is the presence of serum SS-A (Ro) autoantibodies, which strongly predicts
