Nasralla International Molecular Diagnostics Inc., Huntington Beach, California Nancy L. Nicolson The Institute for Molecular Medicine, Huntington Beach, California Summary Pathogenic mycoplasmas have been found in the blood or other specimens of patients with a variety of chronic clinical conditions, including respiratory, oral cavity, genital and other infections, autoimmune, inflammatory and immunosuppressive diseases and fatigue syndromes of unknown origin. These small bacterial microorganisms are possible causative agents, cofactors or opportunistic infections in these and other illnesses. Evidence for their association or possible role in various clinical conditions is suggested by their significantly higher incidence or degree of infection in symptomatic patients than in non-symptomatic controls and their gradual suppression by the appropriate antibiotics resulting in gradual patient recovery from clinical signs and symptoms. Although they are not widely appreciated for their pathogenic properties, certain Mycoplasma species and certain other species of bacteria ( Chlamydia Borrelia , etc.) appear play a role in disease progression or patient morbidity in rather large subsets of chronic illness patients.

RESOURCE LIBRARY Mycoplasmal infections Co-Existing Disorders Mycoplasmal infections Author: MA Goldstein and CL Harden The majority of CNS complications of mycoplasmal infection result from Mycoplasma pneumoniae. Neurologic complications of M. pneumoniae can begin days to weeks after the onset of respiratory symptoms. They vary in type and severity. Seizures can complicate mycoplasmal meningitis and meningoencephalitis. M. pneumoniae CNS infection should be considered in the differential diagnosis of seizure and encephalopathy of unclear etiology. Caceres and colleagues reported a case of a 10-year-old patient presenting with seizures of focal onset and rapid secondary generalization, which did not respond to high doses of multiple anticonvulsants and ultimately required a phenobarbital drip. Continuous EEG monitoring demonstrated persistent epileptiform discharges. CSF analysis and brain biopsy studies were unrevealing, but mycoplasma pneumonia titers showed elevated immunoglobulin G (IgG) and immunoglobulin M. High-dose methylprednisolone resulted in marked EEG improvement and rapid clinical recovery.
Diagnosis
The results of CSF analysis in mycoplasmal meningitis reveal:
  • Protein: normal or slightly elevated Glucose: normal or slightly decreased WBC counts: average of 80, with polymorphonuclear leukocytes averaging 25%

Mary Ann Isaksen
From the September/October 1997
Mycoplasma pulmonis , the organism that almost all rats in the general pet population carry is the cause of the majority of respiratory and genital infections in rats. The organism is carried in the upper respiratory system and is transmitted by direct contact between mother and babies, by sexual transfer, through the air over short distances, and can even be passed to the babies while going through the birth canal if the mother is infected.
PREDISPOSING FACTORS
Stress or other illnesses can weaken your rat's immune system and cause your rat to break with an active mycoplasmal infection. If left untreated, this can lead to pneumonia. Therefore, it is very important to ensure a stress-free environment for your pet. This includes providing an uncrowded, well-ventilated, clean cage with a safe bedding product in which to live at all times. This will eliminate any chance for ammonia buildup from urine and feces, or phenols from cedar and pine, to damage the respiratory system's own protective capacity. It is also very important not to expose your pet to other rats who may be sick since a secondary bacterial or viral infection could allow the mycoplasmal infection to accelerate to pneumonia very quickly, possibly causing death.

Mycoplasmal Conjunctivitis Fact Sheet
HISTORY: During the winter of 1993-94, house finches ( Carpodacus mexicanus ) with severe conjunctivitis (swollen, crusty eyes) and impaired vision were reported at bird feeders in Virginia and Maryland. Birds with these symptoms were diagnosed as infected by the microorganism Mycoplasma gallisepticum (MG). Reports of infected house finches have become more frequent and widespread. The disease has been documented in Ohio and states east of the Mississippi River as well as in some states west of the Mississippi River. Finches with symptoms of MG infections were observed in Ohio in the fall of 1994; reports to the Division of Wildlife, nature centers, wildlife rehabilitators, and Cornell Laboratory of Ornithology (organizer of Project FeederWatch) have increased. MG has been cultured from several goldfinches with swollen eyes.

    CVM Update
February 7, 2002 FDA TO HOLD TECHNICAL WORKSHOP ON SWINE MYCOPLASMAL PNEUMONIA The Food and Drug Administration (FDA) is holding a Swine Mycoplasmal Pneumonia Technical Workshop. The topic to be discussed at this public workshop is how to evaluate drug effectiveness against swine mycoplasmal respiratory disease. The public workshop will be held on March 6 and 7, 2002, at the DoubleTree Hotel Kansas City, 1301 Wyandotte St., Kansas City, MO 64105, 816-474-6664. The workshop will be held from 8:30 a.m. to 5:00 p.m. on Wednesday, March 6 and from 7:00 a.m. to 3:30 p.m. on Thursday, March 7. FDA is seeking scientific input from a broad public forum to help the Agency determine an acceptable method, in light of the current state of scientific knowledge, for evaluating drug effectiveness against swine mycoplasmal respiratory disease. Mycoplasma hyopneumoniae is a major pathogen in "porcine respiratory disease complex" (PRDC). PRDC is a significant problem in the swine industry in the United States and abroad. This workshop will provide a necessary forum for leveraging scientific resources, including top experts in swine mycoplasmal pneumonia. The workshop is part of CVM's leveraging initiative aimed at increasing interaction with industry, academia, practitioners, and other government agencies. Registration is required for the meeting, and space is limited. Registration will be on a first come, first served basis, and there is no registration fee.

Immediate family members of veterans diagnosed with Gulf War Illnesses (GWI) often complain of fatiguing illnesses, and upon analysis they report similar signs and symptoms as their veteran family members. Since a relatively common finding in GWI patients is a bacterial infection due to Mycoplasma species, we examined military families (149 patients: 42 veterans, 40 spouses, 32 other relatives and 35 children with at least one family complaint of illness) selected from a group of 110 veterans with GWI who tested positive (~41%) for at least one of four Mycoplasma species: M. fermentans, M. hominis, M. pneumoniae or M. genitalium. Consistent with previous results, over 80% of GWI patients who were positive for blood mycoplasmal infections had only one Mycoplasma species, M. fermentans.

Since we previously found that about one-half of Gulf War Illness (GWI )patients had mycoplasmal infections, such as M. fermentans, patients with Chronic Fatigue Syndrome (CFS) and/or Fibromyalgia Syndrome (FMS) were examined for system-wide mycoplasmal infections by examining their blood leukocytes.
Methods: Patients: Blood samples from 203 patients (148 female, 55 male) diagnosed with CFS or FMS were investigated for Mycoplasma spp. and M. fermentans
