View Current Signatures Sign the Petition To: United States Department of Health and Human Services, National Institutes of Health, and Centers for Disease Control We, the undersigned, respectfully demand that Myalgic Encephalomyelitis (ME), an internationally recognized neurological disease, be also formally recognized as a distinct clinical entity in the United States by the National Institutes of Health and the Centers for Disease Control. It is time to make the case definition/diagnostic criteria explicit in the name.
Currently, most patients are diagnosed with Chronic Fatigue Syndrome, a poorly defined, misunderstood, controversial syndrome which remains underfunded and underresearched. The name Chronic Fatigue Syndrome itself has trivialized the illness and further subjected these patients to incredulity and ridicule. The CFS rubric has not only neglected and failed to help those with ME, it has harmed them. ME patients will not improve with cognitive behavioral therapy (CBT) or graded exercise (in fact, exercise will make them worse) - yet these are considered treatments for CFS. Simply "renaming" CFS will not solve this problem.
ME should not be considered a subset of CFS, for the hallmarks of ME will not be present in the population of CFS patients once the Ramsay criteria are established in the US. Patients with Myalgic Encephalomyelitis under the CFS criteria must wait for six (6) months before acquiring a diagnosis, a critical period in which possible recovery is diminished by lack of proper treatment. CFS/ME is a condition where you have long-term disabling tiredness (fatigue). Most people with CFS/ME also have one or more other symptoms such as muscular pains, joint pains, disturbed sleep patterns, poor concentration, headaches, etc. The cause is not known. Treatments which may help in some cases (but not all) include a programme of graded exercise, cognitive behaviour therapy, and 'pacing'.
What is CFS/ME? CFS/ME is a condition that causes marked long-term fatigue and other symptoms (which are not caused by any other known medical condition).
  • CFS stands for chronic fatigue syndrome. Chronic means persistent or long-term.
  • ME stands for myalgic encephalomyelitis. Myalgic means 'muscle aches or pains'. Encephalomyelitis means inflammation of the brain and spinal cord.
However, there is controversy about the nature of this condition. There is no test to diagnose the condition. The diagnosis is made in people who have a certain set of symptoms (which can vary in their type and severity). There is even controversy about what to call this condition.
  • The term CFS is often used by doctors. This is because the main symptom is often fatigue and the condition is chronic (persistent). Also, there is no evidence of inflammation in the brain or spinal cord which is implied by the term encephalomyelitis.

What is fatigue?

CFS/ME is a major world-wide public health problem. There are considerable difficulties in establishing good figures for the number of people affected, but accepting that the figures are approximate, the following generalisations can be made regarding the UK:
  • CFS/ME affects between 2 and 4 people per 1000 of the population it affects people of all ages, including children as young as five years old the commonest age of onset is between the early twenties to mid-forties CFS/ME is diagnosed about twice as often in women compared to men it affects all social classes.
  • There are therefore as many as 200,000 people with CFS/ME in the UK alone  a staggering number for a condition that has attracted so little serious enquiry until recently. Definitions
    From the point of view of conducting research on a condition, it is essential that agreement exists on what it is that is being studied, or conclusions made in one centre will have no relevance to anywhere else. Several groups around the world have therefore worked to produce definitions of CFS/ME that attempt to encapsulate the necessary features of the condition. The most widely used are those of the Centre for Disease Control (1994) in the USA, which are:

    Individual treatment is essential in CFS/ME, making it difficult to present a set scheme that will work for everyone. Although the points listed below are the main ones you will need to consider, exactly how you do this will be up to you. How to manage CFS
  • Accept the illness. Accept the equal importance of mind and body. Get connected to other people with CFS. Get a good doctor. Make sure family and friends understand CFS. Map out what you could do before CFS; compare it to what you can do now. Set priorities for improvement. Make a recovery plan. Sort out employment issues. Sort out pain control. Look at your illness beliefs and reactions. Allow time for relaxation. Consider complementary treatments.

Some other names for this illness are Chronic Epstein-Barr Virus Disease, Chronic Fatigue, and Immune Dysfunction Syndrome (CFIDS), and Post-Viral Fatigue Syndrome (PVFS). The emphasis is put on 'fatigue'. Chronic means ongoing and therefore refers to a fatigue that is long-lasting. The term Chronic Fatigue Syndrome is used primarily in North America. Myalgic Encephalomyelitis (ME) is used in most other countries. It can be best understood when broken down into its individual parts.     itis - inflammation Neither term is satisfactory. People with CFS have many other symptoms contributing to their disability besides fatigue. Seldom is an infection in the brain found in those who have ME as was once suspected. CFS/ME affects thousands of men, women and children. The majority of sufferers seem to be women between the ages of 30 and 50. Generally these people are high functioning, productive individuals. There are some differences in how children of different age groups are affected by the illness. This topic is dealt with later.

    Myalgic Encephalomyelitis (ME)
    A medical condition effecting the immune and central nervous systems. The cause is not clear, but onset is linked in most cases to an acute infection, although some people experience a slow insidious onset, and there is some evidence implicating various common viruses in the development of the syndrome. It is thought to effect 2-3 per 1000 of the population. Onset can occur at any age, but it is most common in persons aged 20-40 years, and in those whose employment exposes them to infection, such as health professionals.
    What are the symptoms?
    The person with ME experiences a disabling, exercise-induced muscle fatigue, muscle pain (myalgia), often severe, a 'flu-like' malaise, an abnormal exhaustion unrelieved by sleep, and various symptoms which indicate a disturbance in brain function (encephalitic symptoms), including loss of concentration and short term memory, dyslexia, nausea, clumsiness and disturbed balance, sensitivity to light and other problems with vision, sensitivity to noise, misjudgement of distance, and sometimes depression, mood swings, and problems with bladder control and bowel disturbance.
