    The Definitive Description of ME
MYALGIC ENCEPHALOMYELITIS : A Baffling Syndrome With a Tragic Aftermath. By A. Melvin Ramsay M.D., Hon Consultant Physician, Infectious Diseases Dept, Royal Free Hospital. [Published 1986]
The syndrome which is currently known as Myalgic Encephalomyelitis in the UK and Epidemic Neuromyasthenia in the USA leaves a chronic aftermath of debility in a large number of cases. The degree of physical incapacity varies greatly, but the dominant clinical feature of profound fatigue is directly related to the length of time the patient persists in physical effort after its onset; put in another way, those patients who are given a period of enforced rest from the onset have the best prognosis. A second group of clinical features found in patients suffering from myalgic encephalomyelitis would seem to indicate circulatory disorder. Practically without exception they complain of coldness in the extremities and many are found to have abnormally low temperatures of 94 or 95 degrees F. In a few, these are accompanied by bouts of severe sweating even to the extent of waking during the night lying in a pool of water. A ghostly facial pallor is a well known phenomenom and this has often been detected by relatives some 30 minutes before the patient complains of being ill.

Note on the term “Myalgic Encephalomyelitis” Eileen Marshall      Margaret Williams th October 2004 Despite the relentless financial, psychosocial and political engineering that seems to underpin the current determination to remove the term “myalgic encephalomyelitis” (ME) from the medical lexicon (where, based on accurate published evidence of the nature of the disorder, it has resided for the last half century), the present proponents of its demise have failed to produce any evidence-base to support their clamour for its removal and its replacement by the less specific term “myalgic encephalopathy”. Such intentional dilution of terminology is puzzling, since it is clearly not the case (as they proclaim in support of their favoured term) that there is no published evidence of inflammation of the central nervous system (CNS) in ME / ICD-CFS  - see for example "Prevalence in the Cerebrospinal Fluid of the Following Infectious Agents in a Cohort of 12 CFS Subjects: Human Herpres Virus-6 and 8, Chlamydia Species, Myco[plasma Species, EBV, CMV and Coxsackievirus"   Susan Levine MD. JCFS 2001:9 (1-2):41-51  The abstract states: "Over the last decade a wide variety of infectious agents has been associated with the chronic fatigue syndrome (CFS) as potential etiologies for this disorder by researchers from all over the world. Many of these agents are neurotrophic and have been linked previously to other diseases involving the central nervous system (CNS). Because patients with CFS manifest a wide range of symptoms involving the CNS as shown by abnormalities on brain MRIs, SPECT scans of the brain and results of tilt-table testing, we sought to determine the prevalence of HHV-6, HHV-8, EBV, CMV, Mycoplasma species, Chlamydia species and Coxsackie virus in the spinal fluid of a group of patients with CFS. The popular image of chronic fatigue syndrome (CFS) or myalgic encephalomyeltis (ME) over the past several decades has generally tended to view the condition as being that of a new illness - one that has arisen in the 20th century. This image has been reinforced by the media and some of the popular (and professional) literature on the condition, with headlines and titles such as " ME - The New Plague ." So is the illness that new? Despite this popular belief, it would seem that conditions similar to ME/CFS have been around for considerably longer than is generally assumed. The mixture of a broad array of neurological and muscular symptoms that can occur after an infective illness can be traced back into history. For instance, as far back as 1750, Sir Richard Manningham reported a syndrome referred to as ' febricula' (or 'little fever') that had a large number of symptoms but was hard to objectively diagnose.

Much is written about Myalgic Encephalomyelitis or M.E and Chronic fatigue Syndrome, but few draw comparisons between the condition and anxiety disorders. There have been claims that Myalgic Encephalomyelitis or M.E and Chronic fatigue Syndrome, are, in fact anxiety disorders, but whether this is true in physiological or scientific terms is unclear. Much has been researched though about the link between diet and Myalgic Encephalomyelitis or M.E and Chronic fatigue Syndrome and also between anxiety and diet. Many people with M.E also suffer from food intollerences and allergies which seems also to be true of quite a large number of anxiety disorder sufferers. What Is ME/CFS/PVFS?
So what exactly is ME/CFS? More often than not, if you tried to find the answer in a book or on a web site, you'd probably find the 'medical' definition of this illness. For example: "Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a severe systemic, acquired illness that can be debilitating. It manifests symptoms predominantly based on neurological, immunological and endocrinological dysfunction. While the pathogenesis is suggested to be multi-factorial, the hypothesis of initiation by a viral infection has been prominent. (...) Before acquiring the illness most patients were healthy, leading full and active lifestyles." What is fatigue?
The size of the problem
CFS/ME is a major world-wide public health problem. There are considerable difficulties in establishing good figures for the number of people affected, but accepting that the figures are approximate, the following generalisations can be made regarding the UK:
  • CFS/ME affects between 2 and 4 people per 1000 of the population
it affects people of all ages, including children as young as five years old
the commonest age of onset is between the early twenties to mid-forties
CFS/ME is diagnosed about twice as often in women compared to men it affects all social classes.

General Discussion Until the late 1980s, myalgic encephalomyelitis was thought to be a distinct, infectious disorder affecting the central, peripheral and autonomic nervous systems and the muscles. Its major symptom was fatigue to the point of extended periods of exhaustion. A group of experts studying the Epstein-Barr virus first published strict criteria for the symptoms and physical signs of chronic fatigue syndrome in 1988. This case definition was further refined in 1994. The Fact Sheet for CFS published by the National Institutes of Allergy and Infectious Diseases of the National Institutes of Health states that "[T]oday, CFS is also known as myalgic encephalomyelitis, postviral fatigue syndrome, and chronic fatigue and immune dysfunction syndrome." ME/CFS is now recognized as part of a range of illnesses that have fatigue as a major symptom.

Cite / link Email Feedback chronic fatigue syndrome (CFS), collection of persistent, debilitating symptoms, the most notable of which is severe, lasting fatigue. In other countries it is known variously as myalgic encephalomyelitis, chronic fatigue and immune dysfunction syndrome, and postviral fatigue syndrome. It was first recognized as a syndrome in the 1860s by Dr. George Beard, who called it neurasthenia. He believed it to be a neurosis with a fatigue component. Definitions and theories of its cause have changed over the years; many cases have been misdiagnosed as imaginary because doctors could find no cause. In the mid-1980s it came to the public's attention, as affluent women in their thirties began to seek treatment. (For unknown reasons, more women than men seek treatment for the disease.)
Symptoms and Identification
As currently defined, chronic fatigue syndrome is the presence of severe, disabling fatigue lasting for six or more consecutive months. The fatigue is persistent or relapsing, and is new (i.e., not lifelong), not relieved by rest, not the result of ongoing exertion, and interferes with normal work, social, educational, or personal activities. Diagnosis also requires at least four of the following symptoms, each persistent or recurring and not present before the fatigue: impairment of short-term memory or concentration, sore throat, tender lymph nodes in the neck or axillary region, muscle pain, joint pain, headaches peculiar to the syndrome, unrefreshing sleep, and malaise of more than one day's duration following exertion. Chronic fatigue that does not meet all these criteria is termed "idiopathic fatigue."

Nightingale Research Foundation Paper Presented by Byron Marshall Hyde M.D. - New South Wales, February 1998 Abstract
: At the 1998 M.E. /CFS conference in Australia, both Myalgic Encephalomyelitis and Chronic Fatigue Syndrome were used to describe a chronic illness. This paper is a discussion on the similarities and differences in these two terms that may lead to scientific difficulties. The author suggests that the definitional criteria and epidemic history of Myalgic Encephalomyelitis (M.E.) and the inclusion criteria are significantly different from the CDC definitions and history. The three typical phases of M.E. are discussed. A brief review of some of the known deaths in phase 2 of M.E. are also mentioned. Myalgic Encephalomyelitis (M. E.) This is a term used to describe an epidemic and sporadic disease process that is associated with a chronic debilitating illness of children and adults. Variants of this term M.E. were first used following a series of repeating epidemics starting in May 1955 in the Royal Free Hospital in London England. New outbreaks of this illness continued until 1958 in various London area hospitals. M.E. and these epidemics are well described by A. Melvin Ramsay in his book Myalgic Encephalomyelitis and Post-Viral Fatigue States The characteristic M.E. illness is a disease process that can, in its complete form, be separated into three distinct phases.

Centers for Disease Control (CDC) describes CFS as a distinct disorder with specific symptoms and physical signs, based on the exclusion of other possible causes. Paul Cheney, M.D., Ph.D. will speak on the most significant research breakthrough yet in the area of CFS that CFS is a compensatory defense mechanism against a particular kind of heart problem that is extremely difficult to detect without proper equipment, trained technicians, and a highly knowledgeable doctor to interpret the data.
