Genetic Disorders of the Cardiovascular System, Dr. Brenda Gerull (Helmholtz fellow)
TTN encodes for the largest known protein, titin. Titin serves as a scaffold in the sarcomere, plays a role in myofilament turnover, and probably, in myocyte signal transduction. More than 360 exons code for multiple, alternatively spliced isoforms of approximately 1-3MDa in size which are expressed in cardiac and skeletal muscle. A role for titin in non-muscle tissue (where it may be expressed at low levels) is less clear. Several inherited human muscle disorders have been mapped to the TTN locus on chromosome 2q31. One form of dilated cardiomyopathy (CMD1G), tibial muscular dystrophy (TMD), and proximal myopathy with respiratory failure (all autosomal dominant disorders) have been mapped to the TTN locus on chromosome 2q31. For CMD1G and TMD, TTN mutations have been identified by us and others. One of the CMD1G mutations is a complex mutation causing a frame shift in A-band titin (thereby introducing a premature stop codon) and, depending on muscle tissue, different posttranscriptional modifications of the truncated protein. In skeletal muscle, the truncated titin (expected size approximately 2MDa) is proteolytically digested to a 1,14MDa protein containing Z-disc and (partial) I-band titin. The truncated titin, however, is not present in a cardiac biopsy sample from an affected family member suggesting differences in posttranscriptional modifications. The question of whether the human cardiac phenotype in CMD1G is due to haploinsufficiency (as suggested by the Western blot results) and why no clinical phenotype of skeletal muscle is observed remain uncertain.

The muscle biopsy also helps to confirm some abnormalities of the lipid metabolism. In those cases the muscle biopsy is essentially normal except for the presence of abnormal lipid deposits in the majority of the fibers with oil-red-O stain. In both glycogen and lipid storage myopathies, quantitative measurement of the deficient enzyme will confirm the diagnosis.
There is no specific treatment for many of the metabolic muscle disorders. Once a particular defect is identified, attempts are made to instruct the patient to avoid precipitating factors. In some disorders, such as carnitine deficiency, a lipid myopathy, attempts have been made with variable success to treat the condition with oral carnitine. Common problems are aches and pain in the neck and shoulder region and disorders in the hands or the wrists. The European project PROCID aimed at providing a scientific base for prevention measures against such problems. A key element in medical and physiological studies of such issues is the analysis of electromyograms (EMG), i.e., the recordings of the electrical signals that are associated with all muscle activity. Ideally, the medical researcher would like to see the activation pattern of each individual motor unit (muscle fibre group). However, the measured electrical signal is the superposition of the signals from a large number of such of motor units, the 'signature' of which is not known a priori and may even change over time. Project-Number : BBW 97.0047-1 Project Partners : Coordinator: Lindholmen Utveckling AB (S) Abstract This Concerted Action was set up to co-ordinate research efforts to advance the scientific base needed to prevent muscular pain related to the use of computer input devices, and to formulate recommendations for the technical and organisational design of work involving computers. The scientific hypothesis of the project was that in computer work, low threshold motor-units in muscles of the neck-shoulder-arm- hand area may be active over long periods of time, not allowing physiological restitution and recovery. Indeed, using advanced electrophysiological techniques, continous firing of some motor units was demonstrated. Such activation was shown to be provoked by biomechanical loading, psychological or mental stress, or a combination thereof.
