Extraordinary progress has been made in understanding the physiopathological mechanisms of skeletal muscle disorders aided by the application of molecular genetics. It was recently enhanced by application of advanced technologies in mRNA expression profiling, and bioinformatics analysis, to characterize gene involvement in individual patients with acquired or inherited myopathies. DNA microarray technique is one of the keys for deciphering the information content of the genome, measuring the expression levels of single genes or thousands of genes simultaneously. THE CLINICAL PICTURE
  • You finally finish tying the last pack while your friend has been circling on his four year old filly. This is their first mountain trip and the nervous filly dug a big hole at the hitching rack while the packs were being tied. The string has just left the trailhead when your friend calls out that his filly is sweating and is walking funny behind. It has been an uneventful trip into the wilderness taking most of the day. The horses and mules were turned loose in the meadow while you enjoyed the first camp meal. At dusk you stroll out into the meadow and find your saddle horse standing away from the other stock that are quietly grazing. As you approach, you notice he seems distressed, has flared nostrils, and is quivering. When you try to move him, he will not walk. The last day of the trip you leave the canyon floor at midday and work your way up to the trailhead. It is warm and the creeks you crossed last year are dry. By late afternoon you notice the third pack horse is lagging behind so you change his position and take him on lead. At the trailhead you unsaddle the stock and offer them a drink in the packing corral. The horse you led won't drink, is breathing fast, and seems depressed. The first stop on the trip is about two hours out. The trail is mostly downhill and the going is easy. The horses get restless after having drank at the spring and you tell your friends to get aboard to continue. As you leave the spring one rider falls behind. You stop to see if there is a problem and he says his horse is acting strange and doesn't want to walk.

    Carnosine - Muscle ageing, muscle disorders
    When man ages from 20 to 70 years of age, the lean body mass (muscles) decline by 20 %, and muscular strength and endurance decline likewise. The carnosine concentration and the antioxidant effect of carnosine decreases by half as age increases. This marked reduction in muscle carnosine concentration may be a cause of the age-related decline in muscle mass, strength and endurance.
    Carnosine has a role in the following neuromuscular disorders, too:
    • ALS (amyotrophic lateral sclerosis) myastenia gravis (MG), polymyositis medicine-related muscle diseases (e.g., statins) late-onset-mitochondrial myopathy.
    The role of carnosine in neuromuscular diseases has been investigated scientifically, and the results suggest supplementation with carnosine. While carnosine does not cure these grave disorders, it halts the oxidative stress and may increase the contractility of the muscles and add some strength and endurance.

    The Muscle Centre at Newcastle upon Tyne is one of three centres in England for the diagnosis and management of inherited muscle diseases which are supported by the British Muscular Dystrophy Campaign . The Muscle Centre runs clinics in Newcastle and at various locations across the North of England. Professor Kate Bushby is the Director of the Centre and holds a joint appointment between the University of Newcastle upon Tyne and the NHS . Her time is split between research and clinical commitments, both of which are mainly focused on muscle disease.
    Muscle Clinics
    The muscle team runs children's and adult clinics throughout the North of England. The main base for the children's clinic is Newcastle General Hospital, and for adults the International Centre for Life. Children's clinics are also held at a range of other locations throughout the region, in conjunction with local physiotherapists and other local staff. In addition to the clinics at the International Centre for Life, adult clinics are held in Middlesborough General Hospital and the Central Clinic Carlisle. We try to ensure that all clinics are multidisciplinary and attended by all members of the team.

  • What Are Metabolic Diseases of Muscle?
    Metabolic diseases of muscle can affect all the body's voluntary muscles, such as those in the arms, legs and trunk. Some can also involve increased risk of heart or liver diseases, and the effects can damage the kidneys. Metabolic diseases of muscle were first recognized in the second half of the 20th century. Each of these disorders is caused by a different genetic defect that impairs the body's metabolism, the collection of chemical changes that occur within cells during normal functioning. Specifically, the metabolic diseases of muscle interfere with chemical reactions involved in drawing energy from food. Normally, fuel molecules derived from food must be broken down further inside each cell before they can be used by the cells' mitochondria to make the energy molecule ATP. The mitochondria inside each cell could be called the cell's "engines." The metabolic muscle diseases are caused by problems in the way certain fuel molecules are processed before they enter the mitochondria, or by the inability to get fuel molecules into mitochondria.
