Definition: Murmurs are blowing, whooshing, or rasping sounds produced by turbulent blood flow in or near the heart. Often, they are caused by turbulent blood flow through the heart valves.

Considerations: A doctor can evaluate heart sounds by listening with a stethoscope. They can be further evaluated with an echocardiogram to see what the exact cause of the murmur is. The heart has four chambers two upper chambers (called atria) and two lower chambers (ventricles). The heart has valves that temporarily close to permit blood flow in only one direction. The valves are located between the atria and ventricles, and between the ventricles and the major arteries from the heart. Normal heart sounds are called S1 and S2. They are the "lubb-dupp" sounds that are thought of as the heartbeat. These sounds are produced when the heart valves close.

What is an innocent heart murmur?
Heart murmurs are extra noises heard with each heartbeat. Some murmurs occur even though the heart and blood vessels are completely normal. These murmurs are called innocent murmurs. There are several different types of innocent murmurs that occur in childhood. Some occur when the child is very young, and others are heard later in childhood.
What causes innocent heart murmurs?
The heartbeat has two distinct sounds that you can hear with a stethoscope. These two thumping sounds are the sounds of the heart valves closing. A heart murmur is heard in between these two normal valve sounds. The murmur sound is caused by blood flowing turbulently through the heart, like the sound of a stream flowing into a whitewater pool. A murmur can occur in several different parts of the heart and blood vessels even though the heart is completely normal.

Innocent murmurs occur in about half of all children at some time during childhood.

Depending on the confidence of the physician, some practicing pediatricians refer all murmurs while others refer only a few children. In residency it is important to become proficient in evaluating murmurs so that you can be fairly confident of what to do. Clearcut Indications for Referral to the Pediatric Cardiologist:
  • Any child with signs of heart failure, cyanosis, tachypnea, significant murmurs, questionable PEs in the nursery. All children with Down syndrome, chromosomal abnormalities, multiple anomalies. The 32-year-old, one of a handful of Chinese literary talents who write in a foreign language, has won awards for every one of her four novels written in French. Her best-known work, The Girl Who Played Go , has been translated into 17 languages. It won the student's Gon-court Prize in France and the Kiriyama Prize in the United States. She's also a painter and has pub-lished a book on the subject. One of her solo art shows a few years ago was attended by several French ministers. A French-educated Beijing native, Shan Sa is something of a panda in French-Chinese diplomacy, a darling export and a favorite figure in the China-dazzled Western media. Back in the motherland, she is portrayed as a nationalistic triumph - the "pretty Chinese girl who has conquered the arrogant French,'' as one media report said. Hype aside, she's even charmed French President Jacques Chirac, who wrote a personal letter praising her works and inviting her to attend a banquet he hosted for Chinese President Hu Jintao a few years ago.

    Although a heart murmur is an important presenting feature of a cardiac disorder in infancy and childhood, innocent murmurs are very common, occurring in up to 80% of children at some time or other. These murmurs are frequently detected during a febrile illness and are also exacerbated by nervousness or on exercise. It is important to distinguish between innocent and pathological murmurs and to arrange more detailed evaluation of the child if there is any doubt. Children should be routinely screened for heart murmurs and other evidence of cardiac disorder between 6 and 8 weeks of age and at subsequent examinations during childhood. Serious cardiac pathology may exist without symptoms. Innocent murmurs The commonest innocent murmur in children (usually heard at age 3-6 years, although also occasionally in infants) is the parasternal vibratory ejection systolic murmur ( Still's murmur which has a very characteristic low-frequency 'twanging' or musical quality.
