Diagnosing Munchausen Syndrome by Proxy

Summary:
Munchausen Syndrome by Proxy is a condition where a parent, usually a mother, deliberately makes their child sick. Helen Hayward-Brown believes the diagnosis of M-S-B-P is flawed in many cases. Her doctoral thesis details the experiences of parents who have children with illnesses that have been difficult to diagnose. Of these a significant number were wrongfully accused or suspected of Munchausen Syndrome by Proxy. Dr Jon Jureidini argues the child's safety has to be the first priority.

Smothered With Something That Looks Like Love, But Isn't: Munchausen Syndrome by Proxy
by Monica Zangwill, MD, MPH By the time Cora W. was eight years old she took six different medicines everyday. She saw eleven different doctors to monitor her condition. And she slept every night attached to a machine to monitor her breathing. But when she was admitted to the hospital, doctors could find nothing wrong with her. Cora was healthy, but she was an unfortunate victim of Munchausen syndrome by proxy. Munchausen syndrome by proxy is a serious situation but, contrary to popular belief, it is not a mental health disorder. Munchausen syndrome by proxy is a form of maltreatment or child abuse. While doctors often detect it, it is not a medical diagnosis.
Then, What Is Munchausen Syndrome by Proxy?
Munchausen syndrome by proxy is named after Munchausen syndrome , a mental disorder where a person makes up symptoms or illness in his or her self. In contrast, in Munchausen syndrome by proxy, "A perpetrator feigns or induces illness in a child in order to garner internal satisfaction," says Marc Feldman, MD, co-editor of The Spectrum of Factitious Disorders and professor of psychiatry at the University of Alabama in Birmingham. (KYW) PHILADELPHIA A Delaware woman was charged Thursday with trying to poison her young son. Police say she could have a rare mental disorder where parents purposely make their children sick. Medical Reporter Stephanie Stahl reports it is called Munchausen Syndrome by Proxy, a form of child abuse that almost always involves a mother intentionally making a child sick.

The Munchausen Syndrome by Proxy In 1951, Richard Asher used for the first time the term of “Munchausen syndrome” to describe a psychological disorder in which the patient fabricates or induces an illness to play the sick role. The choice of “Munchausen” was made after a German man, the Baron Von Munchausen who used to tell fake and exaggerated stories to everybody who would listen to him. About twenty years later, in 1977, Roy Meadow talked of another form of this disease : “Munchausen Syndrome By Proxy” (MSBP). This psychological disorder can be defined as following : “The intentional production or feigning signs and/or symptoms in another person who is under the individual’s care for the purpose of assuming the sick role by proxy”. The aim’s perpetrator is to be looked by others as a very devoted and brave person since he/she undergoes all these sufferings, along with “the need to be in a perverse relationship with a doctor or a hospital staff”. In more than 98 % of the cases, the perpetrator is a female and in about 95% of the cases, it’s the mother. The physician or the hospital staff become unknowing accomplices. This is possible because the perpetrator is an intelligent person who has, most of the time, medical knowledge and, as Marc Feldman, a psychiatrist who

Frequently asked questions
  • "Have I Munchausen's?"
The only one who knows for sure the diagnosis is the patient her/him self. She/he is the only one who could know if she/he intentionally, consciously produced the signs and symptoms in order to be cared as patient. In this case she/he knows that she/he suffer from Munchausen syndrome (factitious disorder).
  • "Can those people ever been cured?"
There is limited information on the best methods to treat factitious disorders. However, there are many reports of various therapies that worked in individual cases. There are also some personal stories of ex - Munchausen patients who recovered (with or without professional help). (On " internet resources " section you'll find links to two of those stories) Finally, "statistically speaking", this pathology seems to become rarer in the old age so, maybe, the natural course is toward recovery. The single most important factor for cure is, in my opinion, the patient's willingness to undergo therapy and liberate her/him self of the disorder. For Munchausen by proxy syndrome data are even scarcer and things are further complicated by the risks for the child (which imposes precaution in assessing individual outcome) and the involvement of the legal system (which make the evaluation of the patient's willingness to cure difficult).

Doctors, emergency medical services (EMS) personnel, members of protective service agencies, and law enforcement officers may unwittingly participate in MSBP when they fail to recognize MSBP behavior, treat the offender, and create a favorable outcome for the child. Protecting America's children is immeasurably important; therefore, law enforcement personnel and EMS providers need to know the significance of behavioral artifacts in the recognition, investigation, and prosecution of MSBP offenders.
