Clinical features include autonomic, cerebellar, and basal ganglia dysfunction. Pathologic examination reveals atrophy of the basal ganglia cerebellum pons , and medulla, with prominent loss of autonomic neurons in the brain stem and spinal cord. M ultiple system atrophy (MSA) is a neurological disorder that has frequently been misdiagnosed as idiopathic Parkinson disease. A proliferation of articles in recent literature has noted that MSA is a disease that primarily affects the functioning of the autonomic, basal ganglia, and cerebellar systems.
The History and Nomenclature of MSA
The recognition of MSA as a unique entity has emerged gradually during the last century. Quinn, in describing several cases occurring in the late 1800s and early 1900s, may have provided the first descriptions of MSA. In 1925, Bradbury and Eggleston wrote the first clear case report of 3 patients with postural hypotension. Today, we know that postural hypotension is a characteristic sign of MSA. Multiple system atrophy
Overview Symptoms Treatment Prevention Alternative Names: Shy-Drager syndrome; Neurologic orthostatic hypotension; Shy-McGee-Drager syndrome; Parkinson's plus syndrome Treatment: There is no cure for MSA, and there is no known means to slow progression. Treatment is aimed at controlling symptoms such as postural hypotension and parkinsonian movements. Anticholinergic medications may be used to reduce early or mild tremors . Levodopa may improve movement and balance. Carbidopa may reduce the side effects of Levodopa and make it work better. However, the response to medications may be disappointing. Many affected individuals respond poorly to treatment with anticholinergics or Levodopa. Medications that may be used to treat low blood pressure include the following:
  • Sympathomimetics Vasoconstrictors Beta-blockers MAO inhibitors Vasopressin 9-fluohydrocortisone
A pacemaker programmed to stimulate the heart to beat at a rapid rate (faster than 100 beats per minute) may increase blood pressure for some people.

The Shy-Drager syndrome (SDS) is a form of progressive autonomic nervous system failure (PAF) with orthostatic hypotension and associated extrapyramidal involvement that is often mistaken for Parkinson disease. SDS includes olivopontocerebellar atrophy and striatonigral degeneration which is attended by PAF. Eight patients with SDS were studied on a 0.5-T superconducting system utilizing T1- weighted inversion recovery (IR) and T2-weighted spin-echo pulse sequences and also on a 1.5-T system using

To the Editor: In reviewing the differential diagnosis in the case of a woman with disturbances in gait, cognition, and autonomic function, Schlossmacher provides a scholarly discussion of multiple-system atrophy (Aug. 26 issue), but he errs by including pure autonomic failure as one of four syndromes subsumed under the diagnosis of multiple-system atrophy. Pure autonomic failure is a distinct and separate clinical entity that affects postganglionic neurons. Autonomic impairment is the principal manifestation; orthostatic hypotension, bladder incontinence, and impotence in men are the major signs. Cerebellar And Spinocerebellar Disorders
Disorders of the cerebellum and its inflow or outflow pathways produce deficits in the rate, range, and force of movement. Anatomically, the cerebellum has three subdivisions. The archicerebellum (vestibulocerebellum) comprises the flocculonodular lobe, helps maintain equilibrium and coordinate eye-head-neck movements, and is closely interconnected with the vestibular nuclei. The midline vermis (paleocerebellum) helps coordinate movement of the trunk and legs. Vermis lesions result in abnormalities of stance and gait. The lateral hemispheres, which make up the neocerebellum, control ballistic and finely coordinated limb movements, predominantly of the arms. Signs of cerebellar disease are listed in Table 179-3.
