General Discussion Multiple system atrophy (MSA) is a rare progressive neurological disorder characterized by a varying combination of symptoms. Affected individuals may experience symptoms similar to those found in Parkinson’s disease (parkinsonism); cerebellar signs such as progressive impairment of the ability to coordinate voluntary movements (cerebellar ataxia); and impaired functioning of the portion of the nervous system (autonomic nervous system) that regulates certain involuntary body functions (autonomic failure) such as heart rate, blood pressure, sweating, and bowel and bladder control. The exact cause of multiple system atrophy is unknown.

The above symptoms occur because SDS damages the autonomic nervous system, the part of the nervous system which controls autonomous functions that are essential to survival. These functions include regulation of heart rate and breathing, in addition to control of intestinal, urinary, and sexual functions. The autonomic nervous system also controls skin and body temperature, and the body's response to stress. Because the autonomic nervous system also controls constriction and dilation of the pupils of the eye, as well as focusing the eye, SDS patients sometimes complain of blurry vision. Diagnosis is sometimes aided by testing autonomic nervous system function, for example, by measuring blood pressure and heart rate with the patient laying down compared with standing up. A polysomnogram, or sleep study, can document sleep apnea (gaps in breathing during sleep). Brain imaging with CT and MRI are usually helpful in differentiating typical Parkinson's disease from atypical parkinsonism.

Shy-Drager Syndrome (SDS) is a form of MSA, termed MSA-P (P indicates that parkinsonism is the most prominent clinical feature), originally named after the two physicians who first described it in 1960: Dr. Milton Shy from the National Institutes of Health and Dr. Glenn Drager from the Baylor College of Medicine. Patients with MSA-P often experience symptoms of fainting or dizziness upon changing position (such as sitting up or standing up from a laying down position), impotence (loss of erectile function in males), and urinary incontinence. The lightheadedness and fainting (passing out) result from a drop in blood pressure that occurs upon standing, which in turn decreases the amount of blood flowing to the brain. When patient collapses to the floor, the amount of blood reaching the brain increases as the blood pressure rises, and the patient regains consciousness. Diagnosis is aided by testing autonomic nervous system function, for example, by measuring blood pressure and heart rate with the patient lying down compared with standing up and documenting drop in blood pressure while standing (orthostatic hypotension). A sleep study (polysomnogram) can document sleep apnea (gaps in breathing during sleep). Brain imaging, particularly MRI, is usually helpful in differentiating typical PD from MSA.

Abstract
Multiple system atrophy is a rare disorder involving pyramidal, extrapyramidal, cerebellar and autonomic nervous systems. Here we report a case which presented with sexual dysfunction to start with, followed by other system involvements.

Introduction
Multiple system atrophy (MSA) is a term introduced by Graham and Oppenheimer in1969 to describe a group of patients with a disorder of unknown cause affecting extrapyramidal, pyramidal, cerebellar and autonomic pathways. MSA included the disorders previously called Striato Nigral Degeneration (SND), sporadic Olivo Ponto Cerebellar atrophy (OPCA), and the Shy-Drager syndrome (SDS) 1. The clinical syndrome can include various combinations of symptoms of autonomic dysfunction [ orthostatic hypotension, impotence, bladder and bowel dysfunction and defective sweating] as well as additional symptoms of nervous system such as rigidity tremor and loss of associated movements. Symptoms usually develop over a period of five years. Here we report a case having features of degeneration of cerebellar, extrapyramidal, pyramidal, and autonomic nervous systems which initially presented as sexual dysfunction.

Help! My best friend is becoming progressively weaker, can't walk,is in constant pain. His neurologist has been reluctant to diagnose but keeps hinting about multiple system atrophy. Where can I find information about this? He's also getting transient cramping all over his body. He's been to Mass. General and is now slated to go to B.U. Hospital.We're so frustrated. Help. A.T. I'm sorry to here about your friend . (I'm not from the U.S. so I don't know about he hospitals you mentioned above) But I know a little about the disease called "multiple system atrophy" ... (I also looked on the internet for some clues ...and pasted it on the bottom of this mail) As far as I know, multiple system atrophy, is a nerve cell disease in which some area's of nerve cells in your brain die. And we still don't know what causes it or what to do about it ... A degenerative disorder characterized by progressive damage to the autonomic nervous system (the portion of the nervous system that controls involuntary functions such as blood pressure, heart rate, digestion and sexual function), muscle tremor and rigidity, slow movement, and other widespread neurologic losses.
Alternative Names: Shy-Drager Syndrome; Neurologic orthostatic hypotension; Shy-McGee-Drager syndrome; Parkinson's plus syndrome
Causes, incidence, and risk factors: Multiple system atrophy (MSA) is a rare degenerative condition. It is a condition that appears similar to Parkinson's disease in that patients may be slow moving, tremulous and have a shuffling gait but with more widespread neurologic damage and damage to the autonomic nervous system (the portion of the nervous system that controls involuntary functions).
The cause is unknown. There is progressive damage (degeneration) of the nervous system, with damage to all parts of the nervous system. The disorder develops gradually. It is most often diagnosed in men over 60 years old.

The term multiple system atrophy has generated significant controversy and confusion in the medical literature. The term now encompasses three conditions once thought to be separate disorders, specifically Shy-Drager syndrome, striatonigral degeneration, and sporadic olivopontocerebellar atrophy. Additionally, there is a hereditary form of olivopontocerebellar atrophy that is not part of the multiple system atrophy spectrum.

Parkinsoninan-like mild cognitive impairment can be observed, but no dementia.

3 . Course :
Orthostatic hypotension and symptoms of autonomic failure such as constipation, impotence in men, and urinary incontinence usually predominate early in the course of the disease. Constipation may be unrelenting and hard to manage. Shy-Drager syndrome may be difficult to diagnose in the early stages. For the majority of patients, blood pressure is low when the patients stand up and high when the patients lie down. Other symptoms that may develop include impaired speech, difficulties with breathing and swallowing, and inability to sweat.

yes no Multiple System Atrophy: MSA; Shy Drager syndrome Multiple System Atrophy is a neurological disorder caused by degeneration of cells in certain areas of the brain. These control a number of different body systems; hence the name. They include functions of the autonomic nervous system (such as the control of blood pressure, sweating, bladder function) and the motor systems (such as muscle activation, movement and balance). MSA affects both men and women. Symptoms usually start between 40 and 60 years of age. The cause of MSA is not known, but is neither inherited nor contagious. MSA can manifest in a variety of different ways and progresses in an unpredictable sequence and time scale for each patient. It is important to understand that having a diagnosis of MSA does not mean that you will experience all the symptoms from the range below. The initial symptoms may resemble Parkinson's disease with slowness of movement, stiffness of limbs and occasionally tremor. Some patients may have been given anti-parkinsonian drug treatment for these problems (with mixed results). A poor response to these drugs and the appearance of some of the following symptoms are indicators of MSA:
