Multiple myeloma
Definition: Multiple myeloma is a cancer of the plasma cells in bone marrow.
Alternative Names: Plasma cell dyscrasia; Plasma cell myeloma; Malignant plasmacytoma; Plasmacytoma of bone
Causes, incidence, and risk factors: Multiple myeloma is characterized by the excessive growth and malfunction of plasma cells in the bone marrow. The growth of these extra plasma cells interferes with the production of red blood cells, white blood cells, and platelets . This causes anemia , susceptibility to infection, and increased tendencies toward bleeding. As the cancer cells grow and expand in the bone marrow, they also cause pain and destruction of the bones. If the bones in the spine are affected, compression of the nerves may result causing numbness or paralysis Renal failure (kidney failure) is a frequent complication caused by excess calcium in the blood that results from bone destruction. Multiple myeloma mainly affects older adults. Other risk factors are unknown. This disease is rare and only 3 new cases per 100,000 people per year occur in the US.

Treatment of Multiple Myeloma at Mayo Clinic
(Synonyms: Myeloma, Plasmacytoma, Kahler's Disease) Mayo Clinic has one of the premier myeloma treatment groups in the world, with more than 60 years of experience treating multiple myeloma. Each year more than 1,000 patients with this disorder are treated at Mayo Clinic. Mayo Clinic's treatment approach includes medications, experimental therapies and diet and lifestyle management. Mayo is a leader in research of new treatments on multiple myeloma , prognostic factors and a basic understanding of the biology of multiple myeloma. Mayo Clinic's integrated group practice gives patients rapid access to multiple specialists, as needed. Hematologists provide primary care for multiple myeloma patients and work closely with Mayo's bone marrow transplant teams, which have performed more than 2,000 transplants. Depending on the patient's needs and condition, consultations may also be arranged with other specialists such as an orthopedic surgeon or radiation oncologist.

Multiple Myeloma Roughly 13,000 Americans develop this illness each year. It is more frequent in men than in women and is more commonly seen after age 60. This disease is due to an overproduction of plasma cells from the bone marrow . Under normal circumstances, these cells are in charge of producing immune globulins, the immune proteins that our body needs to fight infection. In the case of multiple myeloma, the abnormal plasma cells produce abnormal and dysfunctional immune globulins. One indicator of the disease is a blood test known as Beta 2 microglobulin . This disease is basically diagnosed by detecting these abnormal immune globulins in the blood or, alternatively, by observing the abnormal plasma cells in the bone marrow. Causes: The cause of this illness is unknown. Signs and Symptoms Patients may not show any signs for quite sometime and myeloma can present itself in many different ways. Approximately 20 percent of patients are diagnosed incidentally and during workup on an unrelated problem. As the disease progresses, it can cause any of the following problems: Diagnosis The diagnosis is established by certain blood tests and also taking a biopsy of the bone marrow.

Tom Courbat
Those terrible first days
Perhaps you've just heard the three most dreaded words (when used together) in the English language, "You have cancer". Worse yet, perhaps you were told you have Multiple Myeloma (MM), a disease you likely had never heard of, since it represents only 1% of the total cancer cases in the U.S., but 2% of the cancer deaths. You may have had the disease for several months (or years) but it escaped a proper diagnosis. Many doctors and oncologists have never treated an MM case, or have very few MM patients, thus it is fair to say that many will not be up on the latest treatments or clinical trials. They may not even be that knowledgeable about what factors are important to track related to MM. Multiple Myeloma is a malignant cancer of the plasma cell, which is a type of white blood cell found in bone marrow and many tissues of the body. Normal plasma cells fight infections, but malignant plasma cells lose this ability and crowd out other infection fighting cells. As the cancer grows, it destroys normal bone tissue, causing pain and crowding out normal cell production. It is considered a bone marrow/blood cancer and is closely related to leukemia and lymphoma. The old statistics indicated a typical lifespan following diagnosis of three to five years, although the new treatments are significantly extending this period. An acquaintance of mine lived 19 years with the disease, in relatively good health, so don't ever fall victim to "statistics" when it comes to Multiple Myeloma. For better or worse, you may be hit by a beer truck and die long before MM causes your demise!

Typically, multiple myeloma occurs in people at least 60 years of age. Although its cause is not certain, the increased occurrence of multiple myeloma among close relatives indicates that heredity plays a role. Exposure to radiation is thought to be a possible cause, as is exposure to benzene and other solvents. A herpesvirus, HHV-8, may play some role in the disease.

Overview
In 2001, about 14,400 people in the United States will be diagnosed with multiple myeloma. The number of new cases diagnosed each year has increased over the past decades, for reasons not well understood. Risk Factors In most cases, patients with multiple myeloma have no known risk factors.

Disease Background
Description of the Disease Multiple myeloma is a cancer of the plasma cells, a type of infection-fighting white blood cells that produce immunoglobins, also known as antibodies. Multiple myeloma occurs when the body begins to overproduce abnormal plasma cells. These abnormal cells can grow to form multiple tumors in the bone marrow. When the cells appear in a number of sites, the condition is called multiple myeloma. There is no cure yet for multiple myeloma using conventional therapy, although lengthy remissions have been achieved with newer experimental therapies. Before the era of chemotherapy, the median survival for people diagnosed with multiple myeloma was less than one year. Now, people often live three to four years, and some patients survive five to 10 years after diagnosis. Back to Top Who is at Risk?

Jean-Luc Harousseau John Shaughnessy Jr. and Paul Richardson Abstract High-dose therapy with stem cell transplantation (SCT) and novel targeted therapies (thalidomide, its more potent analogues, and bortezomib) represent two approaches for overcoming resistance of multiple myeloma (MM) cells to conventional therapies. While it is now clear that dose-intensification improves the outcome in younger patients, long-term remissions are obtained in a minority of patients. Therefore, the impact of novel agents as part of front-line therapy is the objective of ongoing trials. Gene expression profiling (GEP) will help to improve the management of MM not only by identifying prognostic subgroups but also by defining molecular pathways that are associated with these subgroups and that are possible targets for future therapies.
