If you don't know what the illness is, then any name will do (with apologies to Alice's Adventures in Wonderland, Lewis Carroll). In this issue of American Family Physician, Magill and Suruda[1] use the term "multiple chemical sensitivity" to identify a baffling constellation of symptoms without a dear origin other than the sufferer's declaration that it is caused by exposure to chemicalsmost commonly man-made chemicals. However, at least 20 other monikers have been given to this clinical phenomenon. Another name, recently suggested by the American Academy of Allergy, Asthma, and Immunology,[2] can be added to the list: "idiopathic environmental intolerance." This newly named clinical phenomenon has three defining characteristics: (1) it is an acquired disorder with multiple recurrent symptoms; (2) it is associated with diverse environmental factors tolerated by the majority of other people; and (3) it is not explained by any known medical, psychiatric or psychologic disorder. The reader can appreciate that if a clinical condition has a multitude of names, a pantheon of symptoms, numerous pathophysiologic explanations and numerous nontraditional treatment regimens, then confusion abounds. So as not to further contribute to the confusion of names, I will use the term "multiple chemical sensitivity."

Multiple chemical sensitivity (MCS) has been described under various names since the 1940s.[1,2] MCS syndrome is characterized by the patient's belief that his or her symptoms are caused by very low-level exposure to environmental chemicals. The term "chemical" is used to refer broadly to many natural and man-made chemical agents, some of which have several chemical constituents. Health care professionals who focus on MCS often refer to themselves as practicing "clinical ecology." MCS syndrome has led to great controversy among clinicians, researchers, patients, lawyers, legislators and regulatory agencies. The absence of scientific agreement on MCS has contributed to the development of emotionally charged, extreme and entrenched positions. Gots[2] summarized the controversy as follows:

ABSTRACT
AS RESEARCHERS AND CLINICIANS with experience in the study, evaluation, diagnosis, and/or care of adults and children with chemical sensitivity disorders, we support the stated goal of the National Institutes of Health 1999 Atlanta Conference on the Health Impact of Chemical Exposures During the Gulf War “to fully characterize the nature of multiple chemical exposures within the Gulf War veteran population and to relate this characterization to what is known about Multiple Chemical Sensitivity (MCS) and related conditions and disorders within civilian populations.”(1) Based on research conducted by state and federal government agencies, we already know that MCS is one of the most commonly diagnosed chronic disorders in civilians and the most common—but still largely undiagnosed—disorder of any kind in Gulf War veterans of the United States. In statewide telephone surveys of randomly selected adults, conducted by health departments in California in 1995 and 1996 and New Mexico in 1997, investigators found that 6% of adults in California(2) and 2% of adults in New Mexico(3) indicated that they had already been diagnosed with MCS or Environmental Illness, whereas 16% in both states said they were “unusually sensitive to everyday chemicals.” When randomly selected adults in other states were asked if they were “especially sensitive” (instead of “unusually” sensitive), one-third consistently maintained that they were.(4–6)

by Debra Wood Exposure to small amounts of common chemicals can ruin the lives of chemically-sensitive people, but many doctors deny multiple chemical sensitivity syndrome exists. While debate rages on, environmental medicine specialists are diagnosing and treating the problem. For sufferers of multiple chemical sensitivity (MCS) syndrome, even fleeting contact with cleaning products, pesticides, car exhaust and other inescapable substances in the modern world sets off a series of symptoms that mystify most medical experts. "It's a controversial medical disease," says victim Cynthia Wilson, executive director of the Chemical Injury Information Network, an MCS support organization based in White Sulphur Springs, Montana. She estimates more than 10 million chemically-sensitive people live in the United States. "Our patients come to us very, very ill," says Dr. Richard Walker, an environmental medicine specialist at New York's Healthcare for the 21st Century, one of only six regional treatment centers in the United States. "Many of their physiological systems are being affected by chemicals."
Body Burden
When chemical exposures exceed a person's ability to eliminate these substances, chemical body burden increases, says Walker, who uses a glass of water as an example. If the glass is half full and you add a couple of extra drops, nothing noticeable happens. But if the water level is already at the brim and you add a few drops, the water starts to spill over.

Such chemicals include smoke, pesticides, plastics, synthetic fabrics, perfumes, scented products, petroleum products, fumes, and paints. Persons who experience MCS have a more intense reaction to chemicals than most people, in much the same way some individuals have a far more severe allergic reaction to animals or pollen. 2. How is MCS related to Environmental Illness (EI)? MCS is acute sensitivity to chemical substances. EI includes not only this but also a broad range of conditions triggered by airborne pollution, food, naturally occurring substances such as molds and pollens, and electromagnetic fields. 3. When exposed to the chemicals, people with MCS react with symptoms such as nausea, headache, dizziness, fatigue, impaired memory, rash, and respiratory difficulty. A wide range of household and industrial chemicals, including cleaning products, tobacco smoke, perfumes, inks, and pesticides, have been implicated as triggers for MCS. Many researchers do not regard multiple chemical sensitivity as a medically valid syndrome, believing that the depression that frequently accompanies it is an indication that the symptoms are psychological in origin. Others note that descriptions of the syndrome are largely anecdotal and not proven scientifically, or that the imprecisely defined syndrome is easily abused as a diagnosis, pointing to what they feel is an exaggerated number of worker's compensation cases involving MCS. Nevertheless, many sufferers do seem to improve when they eliminate contact with the chemicals known to trigger their condition; in extreme cases this may mean confinement to specially treated living quarters.

Multiple chemical sensitivity (MCS) is an ailment, or a family of ailments, that has very real consequences for tens of millions of Americans. MCS has been recognized by its symptoms for 50 years because MCS sufferers in many geographical areas, researchers studying them, and doctors treating them, have reported a remarkably consistent picture of disease. However, because MCS sufferers react to chemicals at levels that are hundreds or thousands of times lower than allowable occupational exposures, traditional toxicology dictates that their symptoms cannot be caused by chemical exposures. Nor is MCS a true allergy because there are no IgE-mediated reactions involved, so allergists don't know what to make of it. In sum, because MCS does not fit any of the three currently-accepted mechanisms of disease infectious, immune system, or cancer traditional medicine has not known how to explain MCS, and so has often labeled it "psychogenic" originating in the patient's mind. This has left MCS sufferers in limbo. Told they are crazy, or imagining their disease, or making it up, they find themselves passed from physician to physician without any satisfactory answers and often without relief from their very real distress. (Some MCS sufferers DO have psychological symptoms, but that doesn't necessarily mean their disease ORIGINATES in their mind.) Forty percent of MCS sufferers report having seen more than 10 medical practitioners.

Agency for Toxic Substances and Disease Registry (ATSDR)
An agency of the U.S. Department of Health and Human Services, ATSDR was established to prevent exposure and adverse human health effects and diminished quality of life associated with exposure to hazardous substances from waste sites, unplanned releases, and other sources of pollution present in the environment. Its web site is packed with information - including health alerts and advisories, the HazDat database, ToxFAQs, a science corner, a list of the top 20 hazardous substances, resources for kids, parents and teachers, a newsletter, and more.
Children's Health Environmental Coalition (CHEC)

Introduction Multiple chemical sensitivities (MCS), a condition that probably has been present for hundreds of years in various manifestations, is gaining increasing recognition in medicine (Hileman, 1991). The THEORY behind multiple chemical sensitivity (MCS) is that exposure to environmental chemical fumes (e.g., perfumes, new carpets, pesticides, fresh paint, tobacco smoke, gasoline) leads to symptoms ranging from runny nose to headache to nausea to fatigue to asthma. That's the theory. But what's the reality?
The exposures are commonplace. The symptoms are commonplace. Psychopathology is commonplace among people that claim MCS. There are no tests to determine sensitivity to environmental chemicals. A mechanism of chemical sensitivity is not known.
Does MCS really exist? You would think that this would be the first challenge for a scientist interested in MCS. But here's where science and junk science diverge.

Chemical sensitivity is a loose term describing a group of clinical conditions caused by environmental chemicals. Immunological sensitivity (hypersensitivity or allergy) causes inflammation from exposure to environmental allergens, which are usually natural "chemicals", but may be manmade. Any barrier that prevents access - or precludes the ability to choose to access - is a barrier. Forcibly exposing a chemically sensitive person to the fragrances and chemicals they are reactive to is no different than physically assaulting them. Chemical sensitivities is also known as multiple chemical sensitivities (MCS), chemical injury or illness (CI) and environmental illness (EI).
