Multiple Chemical Sensitivity Syndrome

Multiple chemical sensitivity (MCS) is a syndrome in which multiple symptoms reportedly occur with low-level chemical exposure. Several theories have been advanced to explain the cause of MCS, including allergy, toxic effects and neurobiologic sensitization. There is insufficient scientific evidence to confirm a relationship between any of these possible causes and symptoms. Patients with MCS have high rates of depression, anxiety and somatoform disorders, but it is unclear if a causal relationship or merely an association exists between MCS and psychiatric problems. Physicians should compassionately evaluate and care for patients who have this distressing condition, while avoiding the use of unproven, expensive or potentially harmful tests and treatments. The first goal of management is to establish an effective physician-patient relationship. The patient's efforts to return to work and to a normal social life should be encouraged and supported. M ultiple chemical sensitivity (MCS) has been described under various names since the 1940s.

If you don't know what the illness is, then any name will do (with apologies to Alice's Adventures in Wonderland, Lewis Carroll). In this issue of American Family Physician, Magill and Suruda use the term "multiple chemical sensitivity" to identify a baffling constellation of symptoms without a clear origin other than the sufferer's declaration that it is caused by exposure to chemicalsmost commonly man-made chemicals. However, at least 20 other monikers have been given to this clinical phenomenon. Another name, recently suggested by the American Academy of Allergy, Asthma, and Immunology, can be added to the list: "idiopathic environmental intolerance." This newly named clinical phenomenon has three defining characteristics: (1) it is an acquired disorder with multiple recurrent symptoms; (2) it is associated with diverse environmental factors tolerated by the majority of other people; and (3) it is not explained by any known medical, psychiatric or psychologic disorder. The reader can appreciate that if a clinical condition has a multitude of names, a pantheon of symptoms, numerous pathophysiologic explanations and numerous nontraditional treatment regimens, then confusion abounds. So as not to further contribute to the confusion of names, I will use the term "multiple chemical sensitivity."

A Spurious Diagnosis
Stephen Barrett, M.D. The expression "multiple chemical sensitivity" ("MCS") is used to describe people with numerous troubling symptoms attributed to environmental factors. Many such people are seeking special accommodations, applying for disability benefits, and filing lawsuits claiming that exposure to common foods and chemicals has made them ill. Their efforts are supported by a small cadre of physicians who use questionable diagnostic and treatment methods. Critics charge that these approaches are bogus and that MCS is not a valid diagnosis.
What Is MCS?
The concepts underlying MCS were developed by allergist Theron G. Randolph, M.D. (1906-1995), who asserted that patients had become ill from exposures to substances at doses far below the levels normally considered safe. In the 1940s, he declared that allergies cause fatigue, irritability, behavior problems, depression, confusion, and nervous tension in children. In the 1950s, Randolph suggested that human failure to adapt to modern-day synthetic chemicals had resulted in a new form of sensitivity to these substances. His concern with foods then expanded to encompass a wide range of environmental chemicals. Over the ensuing years, the condition he postulated has been called allergic toxemia, cerebral allergy, chemical sensitivity, ecologic illness, environmental illness (EI), immune system dysregulation, multiple chemical sensitivity, total allergy syndrome, total environmental allergy, total immune disorder syndrome, toxic response syndrome, 20th century disease, universal allergy, and many other names that suggest a variety of causative factors. These labels are also intertwined with Gulf War syndrome, sick building syndrome, toxic carpet syndrome, and other politically controversial diagnoses.

An Analysis of the National Environmental Justice

Resolution #21 on Multiple Chemical Sensitivity
Stephen Barrett, M.D. The U.S. Environmental Protection Agency (EPA) is considering a proposal from the National Environmental Justice Advisory Council (NEJAC) that advocates the political agenda of MCS proponents. NEJAC was chartered in 1993 to provide independent advice to the the EPA Administrator on matters related to environmental justice. Most of NEJAC's members are from community-based groups; industry and business; academic and educational institutions; state and local governments, federally recognized tribes and indigenous groups; or other nongovernmental and environmental groups.. In May 2000, NEJAC's enforcement subcommittee proposed that MCS be recognized as a disease and that people who claim to have it should be given special accommodations. NEJAC has accepted the resolution and forwarded it to EPA for review. This proposal flies in the face of scientific knowledge and can have serious legal and economic consequences. The full text is reproduced below with my comments interspersed. WHEREAS, people throughout the world have developed a chronic condition, Multiple Chemical Sensitivity (MCS). in response to a single massive exposure or repeated low level exposures to one or more toxic chemicals and other pollutants in the environment; and

Multiple Chemical Sensitivity (MCS) is basically a subset of Environmental Illness (EI), which is caused by living in a toxic world. The chemicals that were synthesized after World War II (including, pesticides, synthetic fragrances, cleaning products, detergents, etc.) are mostly "petro-chemicals" (petroleum based) and are quite toxic to humans. There have been virtually no studies done on the majority of these chemicals to see how they affect humans - the industry just placed the chemicals in the environment with the assumption that they are "safe, till proven toxic", instead of the other way around.
