    A variant of the GUILLAIN-BARRE SYNDROME characterized by the acute onset of oculomotor dysfunction, ataxia, and loss of deep tendon reflexes with relative sparing of strength in the extremities and trunk. The ataxia is produced by peripheral sensory nerve dysfunction and not by cerebellar injury. Facial weakness and sensory loss may also occur. The process is mediated by autoantibodies directed against a component of myelin found in peripheral nerves. There is no specific test or combination of tests that can establish the diagnosis of AOSD and patients may present with other systemic involvement including neurological manifestations in 7–12% of cases.     Guillain-Barre Syndrome And The Miller Fisher Variant
    Christopher P. Sobczak, M.D., Safwan Jaradeh, M.D. Medical College of Wisconsin, Milwaukee, WI Discussion: Miller Fisher syndrome is a variant of Guillain-Barre Syndrome involving the triad: ataxia, ophthalmoplegia, and areflexia. The pathophysiology of the syndrome can be explained through GQ1b ganglioside autoantibodies, which are seen in 96 percent of cases of Miller Fisher Syndrome. These autoantibodies recognize epitopes expressed specifically in the nodal regions of oculomotor nerves and cerebellar neurons. Organisms such as Mycoplasma Pneumoniae and Campylobacter jejuni contain cell walls that are capable of inducing the production of these antoantibodies. Treatment is thus involved in immune modulation, through the use of IVIg, plasma exchange, and steroids. Our patient required intubation and wan treated with all three modalities over a three-month hospitalization. He eventually recovered to be discharged to long term rehabilitation.     A 62-year-old man presented with an acute onset of symmetric upper and lower extremity paresthesias. Neurological examination showed normal motor strength throughout with areflexia and reduced vibratory and proprioceptive sense in all four extremities. A lumbar puncture performed 6 weeks after onset demonstrated elevated cerebrospinal fluid protein with no cells. Electrodiagnostic testing 7 weeks after onset revealed a primarily sensory neuropathy with normal motor conduction and needle electromyographic studies. Symptoms stabilized within 3 weeks of onset and improvement began within 2 months. Except for the absence of motor disturbance, this case appears to resemble Guillain-Barr© syndrome. Guillain-Barr© syndrome (GBS) is typically characterized by an acute onset of bilateral, symmetric, rapidly progressive weakness of the limbs, and sometimes the trunk and cranial innervated muscles, associated with absent or reduced muscle stretch reflexes and an elevated cerebrospinal fluid (CSF) protein without an increase in cells.

    Thank you for providing this forum.
    I was recently hospitalized and diagnosed with Miller Fisher syndrome. It began one day with distorted vision and by the next morning I could not control my motor functions and my vision was double. The neurologist ordered every tetst imaginable, blood, CT, tensilon, cranial and spinal MRIs, lumbar puncture, EMG etc.
    My dilemmas is, nothing unusual has shown up in the diagnostic work. All of the tests were withing the normal range. There was a very small brain lesion on the MRI but I was told, not in an area likely to cause the type of motor problems I was having. They initially began treating me with steroids for MS, but based on the presence of ophthalmoplegia and lack of reflexes, changed the diagnosis to Miller Fisher. I was given 5 days of IVIg and released. I am still weak - but can walk - although the double vision persists.
    During a followup appointment last week, my neuro told me that a blood antibody test for Miller Fisher had just come back "negative" but they are standing by the diagnosis based on my symptoms and the response to IVIg.

    What are the symptoms of a Campylobacter infection? Diarrhea is the most consistent and prominent manifestation of campylobacter infection. It is often bloody.[5] Typical symptoms of C. jejuni infection also include fever, nausea, and vomiting, abdominal pain, headache, and muscle pain. A majority of cases are mild and do not require hospitalization and may be self-limited. However, Campylobacter jejuni infection can be severe and life-threatening. Death is more common when other diseases (e.g., cancer, liver disease, and immuno-deficiency diseases) are present. Children under the age of five and young adults aged 15-29 are the age groups most frequently affected. The incubation period (the time between exposure on onset of the first symptom) is typically two to five days, but onset may occur in as few as 2 days or as long as 10 days after ingestion.[5] The illness usually lasts no more than one week; however, severe cases may persist for up to three weeks, and roughly 25% of individuals experience symptom relapse.
