A procedure's serendipitous success hints that some headaches start in the heart
Ben Harder Neurologist Roman Sztajzel received an unexpected letter in 1999 from a patient he had last seen a year and a half earlier. The Swiss woman thanked him for curing her of migraines, which she had experienced frequently into her early 30s. But Sztajzel hadn't treated her for migraines. He'd seen her because she'd had a stroke. Another stroke soon followed. Neither brain attack showed any sign of a typical cause. In search of an explanation, Sztajzel and his colleagues had screened the woman for an abnormal opening between the heart's upper chambers. GAP IN A YOUNG HEART. An opening between the two upper chambers of the fetal heart permits blood to pass from one side to the other. The gap can cause problems if it doesn't close in early life.

At or shortly after birth, tissue flaps in the heart usually fuse and close the hole. But in about a quarter of the U.S. population, complete closure never occurs.

Migraine headaches typically affect one side of the head. They can last anywhere from a few hours to a few days. Some people have them weekly, others have fewer than one a year. Migraines usually begin sometime between the teen years and the age of 40, and can be classified as either ""classic'' or ""common.''
Causes
At the onset of a migraine, the blood vessels in your head first shrink, then swell, causing pain. Tension, bright lights, loud noises, strong smells, weather changes, fatigue, missed meals, and emotional upset all may trigger a migraine. The headaches may also be brought on by many common foods and beverages, including lunch meat, hot dogs, alcohol, beans, coffee or tea, cheese, chocolate, nuts, pickles, raisins, and canned soup. Artificial sweeteners can trigger a migraine. Many women get the headaches before or during their monthly period.
Signs/Symptoms
You may know you are going to have a migraine before the headache starts. Warning signs include nausea, vomiting, and sensitivity to noise, light, or smells.

GROWING STRONG IN SPITE OF MIGRAINE
Migraine is perhaps one of the most misunderstood conditions I know about. It is commonly thought of as a type of headache, and it certainly isn't thought of as disabling. It can be disabling, and it isn't just a headache. I have put this site together to help people understand migraine more fully and learn how to live with it and help loved ones live with it.
The Mystery Symptoms
In 1986, I was chosen to participate in a program for gifted and talented youth at the Texas School for the Blind. At first, I balked, remembering my previous experience there. But perhaps things had changed. In any case, I was older now and more able to speak up for myself. Besides, I could take a cooking class. I learned my way around campus easily using my cane as well as visual landmarks. I traveled from "my house," which I shared with four other girls, to the classroom building, the dining hall, the recreation center, and dormitories where friends were staying. But one afternoon, I encountered a problem while traveling. I froze. If I had become lost, I would have been able to think and retrace my steps. But I could not retrace my stepsand I wasn't lost. I knew exactly where I was. I was approaching the bridge under the trees, on my way to the dorms to see friends. But I couldn't see the shadow of the trees. I couldn't see the contrast between the color of the bridge and the color of the sidewalk. I couldn't see the grass on either side of me. The sun was beating down on me, but I couldn't see it either. I couldn't see anything.

Migraine Gene Discovered
A n abnormally structured gene is the culprit in familial hemiplegic migraine, a rare inherited form of migraine. People with a similar neurological disorder, episodic ataxia type-2, have other alterations in the same gene, researchers found. The gene resides on chromosome 19, the organization and structure of which has long been studied by Lawrence Livermore National Laboratory's Human Genome Center. Three years ago, medical scientists at Leiden University in The Netherlands initiated a collaboration with the LLNL researchers to complete the physical mapping of the chromosome and clone the DNA of the gene for this form of migraine. Armed with the cloned DNA fragments and data from human patients studies, the Dutch researchers discovered the gene that regulates the transport of calcium into specific classes of brain cells. The researchers noted alterations in the DNA sequence in patients from the migraine families. These alterations result in the production of nonfunctional protein, which prevents calcium from being properly transported. The movement of calcium into brain cells regulates the release of neurotransmitters, which are critical elements in the network of communication among cells of the nervous system. While the type of Familial Hemiplegic Migraine associated with this gene is rare, many of its symptoms are similar to those of more common migraines. Scientists believe knowledge gained in studying this form of migraine will help understand the cause of other forms of the disorder.
