Hoofdpijnlijders krijgen van vrienden en familie heel vaak goedbedoelde adviezen. Jammergenoeg lossen die de pijn niet op. Voor de meer dan een miljoen Nederlanders die regelmatig last hebben van hoofdpijn kan een bezoek aan hun dokter geen kwaad. De beste bondgenoot in de strijd tegen je migraine is een dokter die jou kent, die bereid is ruim voldoende tijd te stoppen in diagnose en behandeling. Hij moet bekend zijn met de moderne technieken, inclusief de hoofdpijn-classificatie, diagnoses en behandelingen van hoofdpijn. Hierna volgen enkele aanwijzingen die kunnen helpen bij het eerste bezoek aan de dokter. Wanneer moet ik bellen? Bij iedere soort pijn die u heeft, rijst ogenblikkelijk de vraag: "Moet ik een dokter bellen?" Het is heel eenvoudig. Als u regelmatig ernstige hoofdpijn heeft die uw dagelijks leven belemmert, of als u erg veel pijnstillers slikt, moet u professionele hulp zoeken. Iedere ernstige of chronische hoofdpijn die plotseling opkomt en die gepaard gaat met lusteloosheid, duizeligheid of andere ongewone lichamelijke verschijnselen moet u onmiddellijk aan uw dokter melden. Hou een hoofdpijndagboek bij Bel zonodig uw huisarts. Hij of zij kent u in het algemeen het best en kan daardoor gemakkelijker uw probleem behandelen. Als hij geen of moeilijk een diagnose kan stellen of niet de meest doeltreffende therapie kan voorschrijven, verwijst hij eventueel door naar een specialist. Het onderzoek gaat vlotter en beter als u alles wat u kwijt wilt (desnoods schriftelijk) op een rijtje hebt om diens vragen te beantwoorden. Een belangrijk hulpmiddel daarbij is het hoofdpijndagboek, waarin u de aanvallen en het verloop ervan opschrijft. Het ingevulde dagboek neemt u mee naar uw huisarts of specialist. There are two ways to approach the treatment of migraine headache with drugs: prevent the attacks, or relieve the symptoms during the attacks. Many people with migraine use both approaches by taking medications originally developed for epilepsy and depression to prevent future attacks, and treating attacks when they happen with drugs called triptans that relieve pain and restore function. Hormone therapy may help some women whose migraines seem to be linked to their menstrual cycle. Stress management strategies, such as exercise, relaxation, biofeedback, and other therapies designed to help limit discomfort, may also reduce the occurrence and severity of migraine attacks.

    AASH Index a timely medical meeting newsletter TREATMENT OF MIGRAINE HEADACHE: SUMATRIPTAN (SUMA) D.L. Gutterman, PharmD, Glaxo Wellcome, Research Triangle, NC, presented the latest safety and efficacy data on Imitrex(R) (sumatriptan). The data were collected from controlled trials on more than 25,000 patients treated for over 140,000 migraine attacks. Inclusion of information from postmarketing surveillance studies increased the number of SUMA-medicated patients to over five million. Many years of experience showed SUMA to be highly effective and the standard for other triptans in research. Careful review of all available data found less than one per million treatments with SUMA led to a cardiovascular-related event. Please consult the accompanying information on Imitrex Tablets and Injection Background on sumatriptan (SUMA) The development of this drug is based on serotonin pharmacology and migraine headache pathophysiology. Research since the 1950s identified 3 main families of serotonin (5-HT) receptors designated as 1, 2, and 3 with many subtypes (A, B, C, etc) in each family. Sicuteri launched the 5-HT theory of migraine with the discovery that methysergide (METH), a 5-HT type 2 blocker, helps relieve migraine ( Int Arch Allergy Synthesis of SUMA in the late 1980s corrected the limitations of ergotamine (E) and dihydroergotamine (DHE), both nonspecific 5-HT receptor agonists. As a selective 5-HT type 1 agonist, SUMA does not stimulate other 5-HT receptors which cause nausea, vomiting and diarrhea as seen with METH, E, and DHE. Also, SUMA relieves sensitivity to light, sound, and smell in addition to controlling the nausea and vomiting of migraine.

    • A frightening attack of sudden onset of unilateral weakness (one-sided weakness).
    • Weakness may involve face/arm/leg.
    • If the right side of the body is the affected side, the individual may not be able to speak.
    • May be triggered by mild head injury.
    • There is a familial form, with a first degree relative affected. (i.e., father, mother, uncle, aunt)
    • The headache follows the onset of weakness.
    • The weakness may last one hour to days, but usually resolves in 24 hours.
    • The familial form has been identified on chromosome 19 in 50%-60% of cases.
    • Other causes of acute onset weakness must be considered before the diagnosis is made.

    Migraine: Migraines are a form of chronic recurring headaches. It is mistaken to assume that any form of chronic headache is a migraine, as migraines have very special symptoms , and there are numerous other causes of chronic headache: tension headache , and others; see also the list of underlying conditions for headache in general. Even within migraine there are several subtypes including both with and without a preceding aura, basilar artery migraine , and even headache-free migraine where there is no headache! Migraine: The most common type of vascular headache is migraine. Migraine headaches are usually characterized by severe pain on one or both sides of the head, an upset stomach, and at times disturbed vision. Diagnostically, one must determine whether the dizziness and headaches are independent or related to each other, and in particular, whether they are a manifestation of migraine. Here we will review the association between vertigo and migraine. This subject has also been recently reviewed by Reploeg and Goebel (2002) as well as Radke et al (2002). Epidemiology Nearly 13% of the adult population of the United States has migraine. There is a male-female distribution difference. At all ages, about 5% of men have migraine (Stewart, 1994; Lipton et al, 2002). Women of childbearing age have a much higher prevalence, jumping up to roughly 10% at the onset of menstruation, and increasing to nearly 30% at the peak age of 35 years. At menopause, rates of migraine abruptly decline in women back to roughly 10%. Migraine headaches are often misdiagnosed by patients themselves as sinus headaches. A recent study suggested that 88% of 2991 patients who had diagnosed themselves as having sinus headache, actually had migraine (Schreiber et al, 2004).
