Web posted at: 2:13 p.m. EDT (1813 GMT) (CNN) Migraine headache sufferers with heart disease should avoid certain anti-migraine medications, according to a recent study published in Circulation, a journal of the American Heart Association. Migraine headaches are caused by a dilation of blood vessels in the head that causes a severe, pounding pain. Medications to treat the headache cause the blood vessels to constrict or narrow. "Unfortunately, these medications aren't smart enough to exert their effect just on the head vessels," said Jonathan L. Halperin, a cardiologist at Mount Sinai Medical Center. "Sometimes they exert the constrictor effect on the arteries that feed the blood to the heart muscle." The low blood flow can cause chest pain and possibly a heart attack.

A migraine is much more than a bad headache. A migraine headache is a recurrent, throbbing headache usually felt on one side of the head. Migraines usually begin in early childhood, adolescence, or young adult life. Migraine pain can be excruciating, and may incapacitate you for hours or even days. Migraine headaches may be preceded by a warning sign. Warning signs may include: flashes of light, dizziness or numbness. The signs are often accompanied by severe nausea, vomitting, and extreme sensitivity to light and sound. What causes it? Migraine headaches are caused by a rapid widening and narrowing of blood vessel walls in the brain and head. This causes the pain fibers in the blood vessel wall to become irritated. Blood vessels in the scalp are often involved. Some of the events that have been reported to causes migraine headaches are: hunger, cheese, changes in weather, nuts, fatigue, avocados, oral contraceptives, chocolate, menstrual periods, food cured with nitrates (e.g., hot dogs), emotional stress, meat tenderizers (e.g., MSG), and alcoholic beverages. It is not known why some individuals have migraines and others do not. What are some common symptoms?

Migraine Headaches
What most people think are migraine headaches are really tension headaches. "Real" migraine headaches are usually associated with nausea or vomiting and tend to be preceded by seeing flashes of light, zigzags, blind spots, or stars. Doctors call these preceding visual symptoms "auras," which are not nearly as much fun or as practical as what New Age people refer to as auras (and the nausea is no fun at all). Migraines are often triggered by psychological stress, but distinct from tension headaches, migraines tend to begin after a stressed person is finally able to relax; then that "relaxing" weekend or vacation becomes relaxation hell. During a migraine headache, blood vessels initially become overly constricted and then abnormally widened. You usually experience this pain on one side of the head, which can make you feel lopsided. New drugs for migraines studied
From Toria Tolley CNN (CNN) Kiva Schindler is one of the 28 million Americans who suffer from migraine headaches. As often as five times a week, she gets a pulsating feeling accompanied by pain, nausea and light and noise sensitivity. When a migraine hits, her work and home life come to a halt. "I usually have to get someone, my mother or my sister-in-law or somebody, to come and actually get my children ... because they are truly not something that you can just go on with your day to day routine," she said. RESOURCES National Headache Foundation As a woman, she is far from alone. According to the National Headache Foundation, nearly one in five women suffer from migraine headaches, compared with about one in 20 men. Why the difference? Dr. Marshall Nash, a headache specialist and researcher, says there are several biological and environmental reasons why some women get more headaches than men. He points to "a genetic predisposition to have shifts in the chemical called serotonin," in the brain, which plays a role in pain and depression. Serotonin levels also change when estrogen levels drop. Schindler has another possible explanation.

CHICAGO, Illinois (Reuters) Researchers have identified brain lesions in migraine victims, a finding that could indicate the severe headaches are a progressive brain-damaging disease in some cases, new research suggests. The research, which has possible implications for treatment, involved 295 Dutch adults aged 30 to 60, some of whom had migraines with visual disturbances and some migraines without eye problems. They were compared to 140 similar people who were migraine-free. Using magnetic resonance images, the researchers found that for patients with both migraines and visual problems the risk of cerebral infarction tissue which has died due to lack of oxygen when a blood clot blocks an artery was 13 times higher than the group which had no migraines at all. The problem increased with the frequency of migraine attacks.

Bandolier has been stimulated by a number of papers on different treatments for migraine to try and put together a league table of relative effectiveness. In an ideal world this would be created from all the available evidence from systematic reviews. The problem is that the systematic reviews either have not yet been done, or anyway Bandolier couldn't find any.
In these circumstances the choice is either to forget it, or to press on with what evidence is available. Bandolier , you will not be surprised to learn, chose the latter course. So this section comes with the health warning that it is not complete - it is a "Saturday night special" review. Perhaps migraine treatment, which is now a big industry, is one of those topics about which people should write to ask for a Health Technology Assessment.

What is a migraine headache?
A migraine headache is a severe pain felt on one, and sometimes, both sides of the head. The pain is mostly in the front around the temples or behind one eye or ear. Besides pain, you may have nausea and vomiting, and be very sensitive to light and sound. Migraine can occur any time of the day, though it often starts in the morning. The pain can last a few hours or up to one or two days.
  • Most often, migraine affects people between the ages of 15 and 55. Many people have a family history of migraine. They are more common in women. Migraine often becomes less severe and frequent with age.
What causes migraine?
One theory about the cause of migraine is the blood flow theory, which focuses on blood vessel activity in the brain. Blood vessels either narrow or expand. Narrowing can constrict blood flow, causing problems with sight or dizziness. When the blood vessels expand, they press on nerves nearby, which causes pain. Another theory focuses on chemical changes in the brain. When chemicals in the brain that send messages from one cell to another, including the messages to blood vessels to get narrow or expand, are interrupted, migraines can occur.

Migraines are disabling headaches that most likely stem from problems with the nerves and blood vessels in the head. Migraine headaches typically last from 4-72 hours. They may occur as often as several times a week to only once a year. People who have migraines are called migraineurs. Migraine headaches affect about 15% of the population. Three times as many women as men have migraines. Over 80% of migraineurs have family members who have migraines. The following types of migraine headache have been identified:
  • Migraine without aura (common migraine): This type accounts for 80% of migraine headaches. There is no aura before a common migraine.
    Migraine with aura (classic migraine): This type is usually preceded by an aura and is usually much worse than a common migraine. Most often, an aura is a visual disturbance (outlines of lights or jagged light images).
    Status migrainosus: This is the term used to describe a long-lasting migraine that does not go away by itself.
