Objective: To determine the prevalence of microsporidiosis in HIV-infected patients with and without diarrhoea and to characterize alterations in mucosal architecture and brush border enzyme activities in patients with microsporidiosis. Patients: A total of 259 HIV-infected patients undergoing oesophagogastroduodenoscopy because of diarrhoea (n = 123) or other symptoms (n = 136) were studied. Methods: Patients were evaluated for the presence of microsporidia by electron microscopy of duodenal biopsies. Brush border enzyme activities were measured by histochemistry and mucosal architecture was determined by three-dimensional morphometry in biopsies from patients with microsporidiosis and compared with biopsies from a subgroup of HIV-infected patients with or without other enteropathogens. Results: Enterocytozoon bieneusi was detected in 17 patients and Encephalitozoon intestinalis was detected in two patients. Microsporidiosis
Immunosuppressed humans have been sentinels of microsporidial infection, with enteric, neurologic, ocular and pulmonary manifestation being recognized ( Microsporidia ). Their symptomatology, pathology and differential diagnosis, based on ultrastructure and the polymerase chain reaction, has been stated by many authors. Microsporidiosis appears to be a common asymptomatic infection, that is not clinically recognized; about 10% of animal handlers were reported to have antibody to Encephalitozoon sp. The organism grows intracellularly, destroying the infected cells. There is little inflammation. The Brown-Brenn stain (Gram stain for tissues), basic fuchsin, toluidin blue, Azur II-eosin, the Warthin-Starry silver impregnation and polarization facilitate recognition of microsporidia in tissue sections. Tissue imprints (smears), dried, fixed and stained as for blood smears, are useful for diagnosis of corneal and conjunctival lesions. Stool and sputum smears can be stained with a modified trichrome stain employing chromotrope 2R or with a fluorochrome chitin stain, such as Calcofluor.

Definition: Infections with protozoa of the phylum MICROSPORA. Notes: do not confuse with Microsporum infections, one of the DERMATOMYCOSES (see note at MICROSPORUM) Microsporidiosis Categories. Encephalitozoonosis - Infection with protozoa of the genus ENCEPHALITOZOON. Lesions commonly occur in the brain and kidney tubules. Microsporidiosis is a disease that is due to infection with microscopic organisms called microsporidia. These are small simple single-celled (protozoan) parasites that form spores. These parasites must live within other cells and are found in the intestine, liver, kidney, cornea, brain, nerves, and muscles. These spores spread to other cells or are eliminated in the stool or urine. Microsporidia have not been studied very thoroughly as agents of disease, partly because they are quite small. They are difficult to diagnose without the aid of an electron microscope, a piece of equipment that is not in a routine diagnostic microbiology laboratory. Objective: To report a new manifestation of microsporidial infection presenting as nodular skin disease with underlying osteomyelitis. Design: Case report. Setting: Tertiary-care military medical center in Washington, D.C.
