Background: No treatment is currently available for intestinal microsporidiosis due to Enterocytozoon bieneusi , a cause of chronic diarrhea, malabsorption, and wasting in immunocompromised patients. Methods: We conducted a randomized, double-blind, placebo-controlled trial of fumagillin (60 mg/day orally for 2 weeks) in patients with chronic E. bieneusi infection. Efficacy was assessed primarily by the clearance of microsporidia from stools. Patients who did not clear microsporidia from stools received 2 weeks of open-labeled fumagillin. Follow-up stool examinations were performed monthly in patients with parasitic clearance to detect a relapse. Results: 12 patients were enrolled in this study, 10 had acquired immunodeficiency syndrome (AIDS), and 2 were organ transplant recipients. Clearance of microsporidia from stools was obtained in 6 of 6 patients in the fumagillin group vs of 6 in the placebo group (p=0.002). Treatment with fumagillin was also associated with significant increases in D-xylosemia (p=0.003) and Karnofsky scores (p

ABSTRACT Background Intestinal microsporidiosis due to Enterocytozoon bieneusi is a cause of chronic diarrhea, malabsorption, and wasting in immunocompromised patients. Currently, there is no effective treatment. Methods We conducted a randomized, double-blind, placebo-controlled trial of fumagillin (60 mg per day orally for two weeks) in patients with chronic E. bieneusi infection. Efficacy was assessed primarily by the clearance of microsporidia, as evidenced by analysis of stool specimens. Patients in whom microsporidia were not cleared received treatment for two weeks with open-label fumagillin. After clearance of the parasite, follow-up stool examinations were performed monthly to detect relapses. Results Twelve patients were enrolled in this study, 10 with the acquired immunodeficiency syndrome and 2 who had received organ transplants. Clearance of microsporidia occurred in all

To the Editor: Molina et al. (June 20 issue) report a two-week study confirming the activity of fumagillin against intestinal microsporidiosis in 12 immunosuppressed adults, 10 of them with advanced human immunodeficiency virus (HIV) infection. Ten patients had partial clinical responses after a median of 10 months; histologic improvement was not documented. It is not certain, however, that these prolonged responses were a result of fumagillin therapy. As the authors note, highly active antiretroviral therapy for HIV infection is also effective against intestinal microsporidiosis (and cryptosporidiosis), probably because it improves immune function.

B. The Johns Hopkins Hospital (Information provided by Dr. Kim M. Ruska, Pathology Resident) Clinical Information
This patient was a 37 year-old man who was found to be HIV-positive during blood donation in 1985. He has been on several different antiviral regimens since 1992, most recently AZT/3TC, but his CD4 count continued to decline, dropping below 200 in May 1994. He has remained relatively asymptomatic, with the exception of some dermatologic problems, thought to be related to his obesity, and one episode of prolonged diarrhea. A biopsy of a rectal nodule at colonoscopy in 1994 revealed a lymphoid nodule, and upper endoscopy showed mild gastritis. The patient presented to the Moore Clinic on 3/25/96 with complaints of the recent onset of diarrhea. White blood cell count on that day was 4300 with a differential of 31% PMNs, 55% lymphs, 12% monos and 2% eosinophils. CD4 count was 123. A stool specimen was positive for
