Microsporidiosis of queen scallops.
Scientific name or taxonomic affiliation
Unidentified microsporidian. The overall dimensions of mature spores with evidence of a diplokaryon and polar tubules consisting of eight coils in a single row, and the presence of this microsporidian in an invertebrate host suggested affiliation with the genus Pseudopleistophora (Lohrmann et al. 2000).
Geographic distribution
Several coastal sites around the United Kingdom.
Host species
Aequipecten opercularis
Impact on the host
Pathogenicity on wild population of A. opercularis remains unknown despite high prevalences of infection (up to 20%) in some locations. No host reaction to this microsporidian was observed and infected scallops showed no evidence of being in poor condition. Only spore stages (immature and mature) were found. The effect of this parasite on cultured scallops is unknown.
Diagnostic techniques
Histology: Observed only in the connective tissue of the digestive gland. Aggregates of immature spores (2.3 by 1.3 µm) within extremely hypertrophied host cell (up to about 300 µm in diameter) and surrounded by a thin layer of fibroblast-like cells. Mature spores (2.3 by 1.2 µm) clustered in lumen of blood vessels.

Geographic distribution
Foveaux Strait, New Zealand
Host species
Tiostrea chilensis Ostrea lutaria
Impact on the host
Induces formation of oval cysts (20-70 µm) in connective tissue surrounding the gut. These cysts, only visible in histology, contain numerous spores surrounded by more or less concentric layers of fibrous elements infiltrated by haemocytes.
Diagnostic techniques
Histology: Cysts, as described above, contain numerous oval and refractive spores (5.2 µm × 2.57 µm in frozen sections and 4.7 µm × 2.39 µm in plastic embedded sections). The spore has a polar cap at the apex with an attached polar filament of 19-22 coils and a single nucleus. Sporogenesis and number of spores per pansporoblast are not known.
Methods of control
No known methods of prevention or control.

Malaria Drug Fumagillin Is Effective Against AIDS-Related Microsporidiosis
Fumagillin, a drug once used to treat malaria, is an effective treatment for intestinal microsporidiosis caused by Enterocytozoon bieneusi, according to a report published in the June 20th issue of The New England Journal of Medicine
The intestinal disorder is known to cause malabsorption, chronic diarrhea, and weight loss in immunocompromised patients. While the diagnosis is easily made with stool analysis, no treatments that effectively eradicate E. bieneusi had been identified.
Ten patients had AIDS and two had undergone organ transplantation. The patients were randomized to receive fumagillin 60 mg or placebo daily for 2 weeks.
After the randomized phase, fumagillin was offered to patients in the placebo group. All of these patients cleared E. bieneusi from their stool.
While three patients in the fumagillin group demonstrated laboratory abnormalities, none required hospital admission. In contrast, one patient in the placebo group required hospitalization for severe diarrhea and weight loss. Several minor adverse events occurred, but it was unclear whether these were due to the fumagillin therapy or to underlying conditions.

Researchers from United States, Russia and Azerbaijan plan joint work on HIV/AIDS related infections
July 9, 2004 A workshop sponsored by the U.S. Civilian Research and Development Foundation (CRDF) and hosted by the Institute of Cytology in St. Petersburg, Russia recently brought together researchers from the United States, Russia and Azerbaijan studying infections associated with HIV/AIDS. Participants in the workshop compared and discussed best practices in their respective countries for diagnosing and preventing cryptosporidiosis and microsporidiosis, two hazardous, yet poorly investigated opportunistic infections.

    Microsporidiosis
AETIOLOGY Microsporidia are widespread organisms in nature, but they have been considered for a long time to have a low impact on animal or human health. With the advent of HIV and AIDS however, significant numbers of human cases of microsporidiosis have been reported and the zoonotic impact of this infection has recently been confirmed. Microsporidia are small, unicellular parasites. Their transmission between infected and healthy organisms occurs by means of highly resistant spores that contain the infective material called "sporoplasm". Currently about 1000 species have been described but it is generally accepted that many others are present in nature (Canning, 1993). EPIDEMIOLOGY Some aspects that remain largely obscure with respect to the biology of these parasites are: the epidemiological patterns, the susceptibility of different vertebrate organisms to the parasites and the routes of transmission between susceptible hosts. Recent studies (van Gool et al., 1997) reported that a high proportion of immunocompromised persons harbour antibodies against Encephalitozoon spp. compared with healthy individuals. These findings can account for the wide distribution of microsporidian in the environment. They also suggest that without an impairment of the immune system they do not represent a serious health risk because they are unable to induce infection.

      Micosporidiosis Pathogen:
Several species of microsporidia Enterocylozoon bieneusi, Encephalitozoon hellem cuniculi, Septata intestinalis, Sites of Infection:
      E. bieneusi is found in the small intestine (and has been reported as a cause of sinusitis). Encephalitozoon hellem, cuniculi and Septata intestinalis have produced disseminated infections with symptoms mainly in the sinus or eyes. A recent report (Schwartz et al.) describes several patients with symptomatic and asymptomatic pulmonary microsporidiosis resulting from E. hellem, and suggests that disseminated microsporidia infections are increasing in significance. Pol et al. report that microsporidia may be a frequent cause of otherwise unexplained cholangitis in HIV-infected patients. Bile samples were examined from a series of eight patients with unexplained AIDS-related cholangitis; all eight were found to have biliary microsporidiosis. The presence of E. bieneusi was confirmed by electron microscopy. Symptoms:
Diarrhea and weight loss are thought to be associated with microsporidia infection. However, a recent case-control study was unable to demonstrate such an association. Rabeneck et al. studied 106 HIV-infected men, 55 with and 51 without chronic diarrhea. E.bieneusi was detected with equal frequency in duodenal biopsies of men from the two groups. E. bieneusi was found in 18/55 (33%) of men with diarrhea and 13/51 (23%) of men without diarrhea (RR 1.42, 95%; CI 0.61 - 3.31). The difference remained non significant after controlling for CD4 count and the presence of other enteric pathogens.

Case 127. Microsporidiosis (22 y-o M) Biopsied multifocal white matter lesions, up to 1 cm in size, seen in an HIV-negative patient who often eats raw cow liver Key words : microsporidiosis, Encephalitozooon A necrotic lesion with dense infiltration of lymphoid cells and macrophages is presented. In the cytoplasm of glial cells, basophilic granules are observed infrequently (HE). Cytoplasmic basophilic granules (short rods) in the glial cell is observed by oil immersion (HE). The basophilic short rods are focally clustered within a cytoplasmic vacuole in the other astrocyte (HE, oil immersion). Microsporidiosis, probably infection of Encephalitozoon sp., is strongly suspected.
