Viral Meningitis
(nonbacterial meningitis) What is viral meningitis? Viral meningitis is an infection of the meninges (a thin lining covering the brain and spinal cord) by any one of a number of different viruses. It is a fairly common disease; 500-700 cases are reported each year in New York State. Almost all of the cases occur as single, isolated events. Outbreaks are rare. Who gets viral meningitis? Anyone can get viral meningitis but it occurs most often in children. Which viruses cause this form of meningitis? Approximately half of the cases in the United States are due to common enteroviruses (intestinal). Occasionally, children will have viral meningitis associated with mumps or herpes virus infection. Mosquito-borne viruses also account for a few cases each year. In many cases, the specific virus cannot be identified. How are the viruses that cause viral meningitis spread?

What is meningococcal meningitis?
Meningococcal disease is a severe bacterial infection of the bloodstream or meninges (a thin lining covering the brain and spinal cord) caused by the meningococcal germ.
Who gets meningococcal disease?
Anyone can get meningococcal disease, but it is more common in infants and children. For some adolescents, such as first-year college students living in dormitories, there is an increased risk of meningococcal disease. Every year in the United States, approximately 2,500 people are infected and 300 die from the disease. Other persons at increased risk include household contacts of a person known to have had this disease, immunocompromised people, and people traveling to parts of the world where meningococcal meningitis is prevalent.

Michigan authorities seek partygoers exposed to meningitis
LANSING, Michigan (CNN) State health authorities are enlisting the help of universities, doctors, local health departments and local media to track down as many as 1,000 people who may have been exposed to meningitis during a "rave" party last weekend. Health officials are concerned that the disease could have been passed among the partygoers when many of them shared a pacifier that had been dipped in the drug Ecstasy.

But the Centers for Disease Control and Prevention's Advisory Committee on Immunization stopped short of recommending the vaccine be required. The panel decided it's not cost-effective to require the $60 shot for meningitis, still considered a rare disease. Also, the vaccine doesn't always work. "Our data indicate college students and their parents should be educated about the signs and symptoms of the disease and to seek medical attention immediately if these symptoms are experienced, and that there is an effective vaccine available," said Dr. Nancy Rosenstein of the CDC.

Meningitis is inflammation of the membranes (meninges) covering the brain and the spinal cord. Although the most common causes are infection (either bacterial or viral), chemical agents and even tumor cells may cause meningitis. Encephalitis and brain abscess can complicate infective meningitis. Major bacteria that cause meningitis are Streptococcus pneumoniae, Haemophilus influenzae, staphylococcus and meningococcus. Meningitis caused by Haemophilus influenzae is known to occur in outbreaks, examples include outbreaks in dorms and military outbreaks. Community acquired cases of meningitis are usually caused by Streptococcus pneumoniae, seen in elderly patients. Listeria monocytogenes and Neisseria meningitis are also other causes of community acquired meningitis. On the other hand, viruses are the most common cause of aseptic meningitis. 20,000 to 25,000 cases of bacterial meningitis are seen in the United States every year. Mostly adults are infected, where it can be community acquired or nosocomial. Vaccination against Haemophilus influenzae has reduced the incidence in children. The classical symptoms of meningitis are headache, neck stiffness and photophobia (the trio are called "meningism"). An altered level of consciousness or other neurological deficits may be present depending on the severity of the disease. A lumbar puncture to obtain cerebrospinal fluid (CSF) is usually indicated to determine the cause and direct appropriate treatment.

Meningitis
Meningitis is one of the most serious infections you can have. It is also one of the scariest understandably, since untreated some forms of meningitis can cause death or lasting impairment. The meninges are membranes that enclose the brain and spinal cord. There are actually three layers of membrane: the "dura", which is a tough outer layer; the "arachnoid", which is a lacy, web-like middle layer; and the "pia", which is a delicate, fibrous inner layer containing many of the blood vessels that feed the brain and cord. Meningitis, strictly speaking, is an inflammation of the meninges. (The suffix "-itis" stands for "inflammation"; inflammation of the brain tissue itself is called "encephalitis", where "encephal-" refers to the brain tissue.) There are many causes for inflammation of tissue, and the meninges are no exception. However, the most common cause of meningeal inflammation is irritation caused by infection with bacteria or viruses. These organisms usually enter the meninges through the bloodstream from other parts of the body. As a matter of fact, many meningitis-causing bacteria are carried in the nose and throat, often without the carrier having symptoms. Viral meningeal infections are usually (but not always) less severe than bacterial infections. This is quite fortunate, since there are no antibiotic treatments available for most viruses and we must therefore let viral meningitis run its course by itself. Bacterial meningitis, on the other hand, must be treated with antibiotics in most cases to avoid severe consequences. Unfortunately the only way to confirm that meningitis is not bacterial is to culture the spinal fluid (actually the cerebrospinal fluid, since it bathes both the spinal cord and the brain) and see if there are bacteria in it. Overview Meningitis is an infection and inflammation of the membranes (meninges) and cerebrospinal fluid surrounding your brain and spinal cord. Before current vaccines, most meningitis cases occurred in children younger than 5 years, but the incidence of the disease has increased among young people between the ages of 15 and 24. Older adults also tend to have a higher incidence of meningitis. Viruses, bacteria and fungi can cause meningitis. The seriousness of the infection and the best treatment depend on the cause of the infection. Bacterial meningitis is generally much more serious than viral meningitis, and quick treatment is necessary. The cause of most cases of meningitis is viruses.

West Nile Virus Overview
West Nile virus is transmitted to humans by a mosquito bite. The virus can cause encephalitis (inflammation of the brain) or meningitis (inflammation of the lining of the brain and spinal cord). West Nile virus was discovered in 1937 in the West Nile district of Uganda. New interest was created when West Nile emerged in the United States for the first time in the New York City area in August 1999. There were 62 confirmed human cases and 7 deaths during this outbreak, creating widespread concern.
  • Since the initial 1999 New York City outbreak, the cases of West Nile encephalitis have been relatively limited. In 2002, there were a total of 480 reported cases in humans and 24 deaths (as of August 28, 2002). The distribution of the virus is spreading across the United States, as determined from surveillance of infected birds by the Centers for Disease Control and Prevention . In a little more than a year, West Nile spread to 11 states along the East Coast. In 2002, the virus spread to Florida, Arkansas, Louisiana, and Texas. Cases are also being seen in the Dakotas, Colorado, Nebraska, and Wyoming. Whether West Nile encephalitis will present a serious health risk to the United States in the future is unknown. Using precautions directed at limiting contact by mosquitoes is the best preventive measure at this time.
