Ménières syndrome
Ménières syndrome is due to endolymphatic hydrops (glaucoma of the ear!) and is characterized by a tetrad
  • pressure, discomfort, fullness in the ear
  • fluctuating hearing loss
  • fluctuating tinnitus
  • episodic vertigo. The syndrome may be idiopathic, in which case it is called "Ménières disease", or secondary to various processes that lead to interference with the normal resorption of endolymph, e.g. neurosyphilis (and possibly Lyme disease), following viral infections (sometimes years later), after trauma, with congenital anomalies, etc. Transient ischemic attacks microvascular compression syndromes , thyroid disease, autoimmune processes (es pecially sarcoid, periarteritis, giant cell arteritis, Susac's and Cogan's syndromes) otosclerosis, perilymphatic fistula , multiple sclerosis, diamox responsive recurrent vertigo and ataxia, vestibular epilepsy and < a href="../disorders/migraine.html"> vestibular migraine are also in the list of differential diagnoses. There is a particularly high incidence of migraine in patients with Ménières disease so that treatment may have to be directed at both entities. In most instances, full blown attacks of Ménières are probably caused by an increase in endolymphatic pressure that leads to a break in the membrane separating perilymph (extracellular, potassium poor) and endolymph (intracellular, potassium rich) . The p otassium rich endolymph then bathes the vestibular nerve leading to a depolarization block and a transient loss of function, creating an acute vestibular imbalance, until the membrane is repaired and normal K Na relationships are restored. A physical distention leading to a mechanical disturbance of the utricle or saccule may also produce symptoms, for example, causing the

    What is Meniere's disease? Meniere's disease is a condition of the inner ear. About 1 in a 1000 people develop Meniere's disease. It can affect anyone at any age, but it most commonly begins between the ages of 20 and 50. One ear is commonly affected at first. The other ear also becomes affected at some stage in about 4 in 10 cases. What are the symptoms of Meniere's disease? Symptoms which 'come and go' Meniere's disease typically comes in 'attacks' of the following symptoms:
    • Dulled hearing in the affected ear(s). The amount of hearing loss varies.
    • Vertigo. This is dizziness with a spinning sensation. It can be quite severe and make you feel sick or vomit. Vertigo can develop with little or no warning. You may feel very dizzy and ill. You may need to go to bed until it passes.
    • Tinnitus.

    By Professor Bill Gibson AM MD FRACS FRCS Biographical details In the Beginning There are three main stages through which the disease may progress. The rate of progression from one stage to the next varies greatly between individuals. Stage One After the attack the ear returns completely to normal. By the time the patient sees a specialist, no signs remain and the specialist may be unable to detect any abnormality even using the most sophisticated tests. About fifty percent of patients can expect a remission of symptoms which may be permanent or last for many years even after several troublesome periods of attacks. Stage Two At this stage the hearing loss and tinnitus has become permanent. The hearing usually fluctuates together with increases in the feeling of fullness and the tinnitus. For some patients, these fluctuations may give warning of the onset of an attack of vertigo. The attacks of vertigo tend to occur in clusters which can last several months. The dizziness is still severe but as the patient becomes more accustomed to the vertigo, the vomiting may be avoided.

    Description
    In 1861, French physician Prosper Meniere described a condition that now bears his name. Meniere's Disease (MD) is a disorder of the inner ear associated with a change in the volume of fluid inside a portion of the inner ear called the labyrinth, which includes the membranous and bony labyrinth. It causes episodes of vertigo, tinnitus (a constant noise in one ear, often described as a buzzing or humming sound), a feeling of fullness or pressure in the ear, and fluctuating, progressive low-frequency hearing loss. A typical attack of Meniere's disease may be preceded by fullness or aching in one or both ears. Hearing fluctuation or tinnitus may also precede an attack. A Meniere's episode generally involves severe vertigo (spinning), imbalance, nausea, and vomiting. The average attack lasts two to four hours. After a severe attack, most people find that they are extremely exhausted and must sleep for several hours. In some people, Meniere's episodes may occur in clusters; that is, several attacks may occur within a short time. In other cases, weeks, months, or even years may pass between episodes. Between the acute attacks, most people are free of symptoms or note only mild imbalance and tinnitus.

    Meniere's Disease and Allergies Update: September 26, 2002 : I am still doing quite well if I avoid excessive wheat. The most peculiar development this summer is that I no longer get car sick. I discovered this by accident while driving across the U.S. and back. I used to get horribly car sick but not sea sick. Now I'm wondering if I will get sea sick. Update: April 18, 2003 : I am still doing quite well if I avoid excessive wheat. I get allergy shots every two weeks now. If I follow Dr. Derebery's instructions I stay level and balanced. I am not a physician. I have been diagnosed with bilateral Meniere's disease that has its etiology in food allergies. I am a lucky sufferer because my physicians at the House Ear Clinic in Los Angeles, California were able to diagnose and prescribe a treatment and diet plan that has kept me virtually symptom free since the fall of 1996. There are many Meniere's patients who do not have the successful outcome that I am enjoying. I might add that I do have a graduate degree in developmental neurophysiology and that makes it easier for me to read and attempt to understand the medical research articles. Dr. M.
