Clinically the limb is stiff and painful, there may be thickening of the overlying skin and soft tissue contractures and fibrosis. Radiographs show dense cortical hyperostosis along long bones with a "dripping candlewax" appearance which may appear to flow from one bone to the next ( Fig.1 ). In a child the hyperostosis appears predominantly endosteal as opposed to an adult in whom it is usually more dramatic and causes an increase in the width of the bone. Epiphyses, carpal or tarsal bones tend to show more discrete rounded sclerotic foci (similar to osteopoikolosis) and flat bones may show radiating sclerotic patches. Radiographs may also show soft tissue calcification or ossification and joint ankylosis.

Eventually profound muscle contractures, tendon and ligament shortening, and soft tissue involvement with severe growth disturbances may ensue. Scoliosis, joint contracture and foot deformities may be seen." Encyclopaedia of Medical Imaging Volume III:1 Melorheostosis usually presents in a sclerotomal distribution. Early clinical findings include joint swelling, limitation of motion and pain. Contractures, ligamentous and tendinous shortening and even soft tissue involvement may ensue.( Radiographic findings are often limited to a single limb where more than one bone is often involved. The lower extremity is more often affected than the upper extremity. The axial skeleton is often spared. The classic finding is that of "peripherally located (cortical) hyperostosis...in one bone or a series of bones. The appearance of the osseous excrescences extending along the length of the bone simulates that of candle wax flowing down the side of the lit candle"

This 26 y/o male came to the ER complaining of increasing arm pain. An x-ray was performed. A selected image shown above demonstrates findings of melorheostosis. Rotate the image (or your monitor) to the left (or your head to the right) and you will see the "flowing wax" appearance better.

    Melorheostosis, a rare mesenchymal dysplasia, is one of the sclerosing bone disorders. It is a developmental error, with a sclerotomal distribution, involving frequently one limb. It may be asymptomatic, but pain, stiffness with limitation of motion, leg-length discrepancy and limb deformity may occur. On plain radiographs, melorheostosis shows as cortical hyperostosis with thickening, resembling flowing candle wax. Soft tissue calcification and even ossification may rarely be seen. In some rare and complicated cases, corrective surgery may be required. Until very recently, the etiology of melorheostosis was unknown, but now, it has been established that melorheostosis is due to a loss-of-function mutation in LEMD3 gene (also called MAN1), which encodes an inner nuclear membrane protein. *Authors : Prof M.E Azouz and Prof A.     Name: Monica (child), Lyn (mother) Child: 3 years old City and state: St. Louis, MO Areas of involvement: bilateral, upper extremities Personal History: I am the mother of a 3-year old girl, Monica, who was diagnosed with melorheostosis approximately a year-and-a-half ago. Monica is our seventh child. We are "older" parents (I will be 47 in March), and with the exception of one child who died in utero, our six other children have been pretty healthy. Subsequently, we were referred to Shriner Hospital in St. Louis where the hand surgeon suggested osteotomies on the third and fourth fingers of Monica's right hand. We did not fully accept the surgeon's recommendation because we felt that he and his staff were unreceptive to our questions and input. Specifically, I was concerned about research showing that surgery on children with melorheostosis has a history of poor outcomes. I was particularly concerned about the need for a repeat of surgery due to the high recurrence of soft tissue deformity. We decided to seek a second opinion. This past June, my husband and I went to Mayo's Clinic in Rochester, Minn. We saw two doctors there: Dr. Anthony Stans, an orthopod who has seen or is seeking 5 other children with melorheostosis, and Dr. Julia Katarincic, a hand surgeon. Their opinion was that Monica did need surgery within the year on both hands: osteotomies on two of her fingers on the right hand and an osteotomy on her left index finger. They also recommended an MRI of Monica's left hand because she has a palpable mass (thickness) in the palm below her left index finger which is the same one that is rigid and paralyzed. They also told us that they are sure that the surgery will need to be repeated since soft tissue deformity recurrence is part of the disease, especially in children. Therefore, they said it would be okay for us to wait about one more year before having the surgery done since Monica will undoubtedly have to undergo it a second time.
