Melanoma
Malignant melanoma is a serious skin cancer which is curable if detected early. Melanoma grows from pigment cells (melanocytes) in the outer layer of the skin and mucous membranes (epidermis). Although melanoma usually starts as a skin lesion, occasionally it occurs in other parts of the body such as the eye, brain, mouth or vagina. Melanoma tends to spread out within the epidermis (radial or horizontal growth phase) before moving into the deeper layer of the skin (the dermis)(vertical growth phase). It can occur in adults of any age. Melanoma is more common in people aged 50-70. Occasionally it can occur in teenagers but it is almost unheard of in children. Unfortunately, about two hundred New Zealanders die each year of melanoma, but many more are treated successfully. About one in fifteen white skinned New Zealanders are expected to develop melanoma in their lifetime.
Causes
The risk factors for developing melanoma include:
  • Sun exposure particularly during childhood Serious sunburn , especially when young - however, melanoma sometimes occurs on areas of the body not normally exposed to the sun Family history of melanoma Fair skin that burns easily - melanoma is common in people with Northern European ancestry, and is not common among dark-skinned people

What is melanoma?
The body is made up of many types of cells. Normally, cells grow, divide and die. Sometimes, cells mutate (change) and begin to grow and divide more quickly than normal cells. Rather than dying, these abnormal cells clump together to form tumors. If these tumors are cancerous (also called "malignant"), they can invade and kill your body's healthy tissues. From these tumors, cancer cells can metastasize (spread) and form new tumors in other parts of the body. By contrast, noncancerous tumors (also called "benign") do not spread to other parts of the body. Return to top

Melanoma: What it Looks Like
While only 4% of diagnosed skin cancer is melanoma, melanoma is the most serious form of skin cancer because of its ability to spread. This is why it is so important to catch melanoma early when the cure rate with dermatological surgery is about 95%.
Where Occurs
Melanoma can appear anywhere on the body — soles, palms, inside the mouth, genitalia, and underneath nails. However, it is most commonly found on the back, buttocks, legs, scalp, neck, and behind the ears.
Warning signs
Melanoma often develops in a pre-existing mole that begins to change or a new mole. It is estimated that 20% to 40% of melanomas arise from an atypical mole. This is why it is so important to be familiar with the moles on your body and perform regular self-examinations of your skin. When looking at moles, keep in mind the ABCDs of Melanoma Detection

    March 2, 1998 Web posted at: 9:07 p.m. EST (0207 GMT) NEW YORK (CNN) Two vaccines to make the body attack the deadliest form of skin cancer, melanoma, are showing early promise in patients. The patients had advanced cancer. While tumors shrank in some of them after treatment, it's too early to know whether the therapy known as immunotherapy will help patients live longer. Generally, there are three methods of treatment for cancer surgery, radiation, and chemotherapy. Now researchers are hopeful immunotherapy can be a viable fourth option. "In immunotherapy, you alter the body so that the body itself can recognize the cancer as foreign and attempt to reject it," said Dr. Steven Rosenberg of the National Cancer Institute. Melanoma vaccines to treat the disease rather than prevent it have long been tried in patients, and some past studies have reported they prolong life. The new work is reported in the March issue of the journal Nature Medicine. One study was done by Rosenberg and colleagues at the National Cancer Institute. Their vaccine used a modified bit of protein the immune system can recognize on cancer cells.

    Epidemiologic studies suggest that exposure to ultraviolet radiation is one of the major contributors to the development of melanoma. Other candidates are mutations in or total loss of tumor suppressor genes Important factors in determining risk include the intensity and duration of solar exposure, the age at which solar exposure occurs, and the degree of skin pigmentation (see skin types Exposure during childhood is a more important risk factor than exposure in adulthood. (This is seen in migration studies in Australia where people tend to retain the risk profile of their country of birth if they migrate to Australia as an adult). Fair and red-headed people are at greater risk.

    SANTA MONICA, California (CNN) A blood test for malignant melanoma, the most dangerous form of skin cancer, is currently being tested at more than a dozen U.S. research centers. Doctors at the John Wayne Cancer Institute, near Los Angeles, developed the test and say it already has proven useful in predicting the course of the disease. The California researchers expect the test to eventually guide patients and doctors in deciding how closely to schedule check-ups and whether to escalate treatment before the cancer can even be seen or felt.

    What is melanoma?
    Melanoma is a type of skin cancer. It begins in certain cells in the skin called melanocytes. Each year more than 53,600 people in the United States find out they have melanoma. To understand melanoma, it is helpful to know about the skin and about melanocytes, what they do, how they grow, and what happens when they become cancerous. THE SKIN
    The skin is the body's largest organ. It protects us against sunlight, injury, and infection. It helps regulate body temperature, stores water and fat, and produces vitamin D. The skin has two main layers: the outer epidermis and the inner dermis. The epidermis is mostly made up of flat, scalelike cells called squamous cells. Round cells called basal cells lie under the squamous cells in the epidermis. The lower part of the epidermis also contains melanocytes. The dermis contains blood vessels, lymphatic vessels, hair follicles, and glands. Some of these glands produce sweat, which help regulate body temperature, and some produce sebum, an oily substance that helps keep the skin from drying out. Sweat and sebum reach the skin's surface through tiny openings called pores.

    This website has been designed to help members of the public and the general medical community increase their awareness of the changes in moles that could indicate the development of cancerous changes known as malignant melanoma.
    Detection, diagnosis, and treatment of cutaneous malignant melanomas at the earliest possible stage will give the highest possible cure rate.
    We know that people with a large number of moles have an increased risk of developing a malignant melanoma. We also know that people with atypical (unusual) moles are more likely to develop new and changing lesions.
    It is important to know the special characteristics of moles that have a higher tendency to become malignant melanomas. The 19 pictures shown in the section "What to watch for" show the ABCD's of changes seen in confirmed malignant melanomas. Each example shows some of the features that can be seen in abnormal or changing moles.
    If you have any moles that you are concerned about please see your physician.
