The changing pattern of megaloblastic anemia: megaloblastic anemia in Israel

The causes of megaloblastic anemia were studied in a survey of patients admitted to six Israeli hospitals over a period of 15 yr. Among the 203 patients identified, 69% had pernicious anemia, 12% had gastrointestinal disease, 9% had primary nutritional deficiency of whom only 1% were associated with pregnancy, and 7% had selective vitamin B12 malabsorption with albuminuria. Comparison with previously published surveys showed, that in contrast with earlier studies where primary nutritional deficiency was the cause of megaloblastic anemia in about 70% of cases and pernicious anemia in only 20%, in more recent studies the proportion of cases with

Depressed cell-mediated immunity in megaloblastic anemia due to folic acid deficiency

Cell-mediated immunity has been studied in patients with 1) megaloblastic anemia of folic acid deficiency, 2)megaloblastic anemia of pregnancy, or 3) iron-deficiency anemia. Using dinitrochlorobenzene skin tests, phytohemagglutinin-stimulated lymphocyte transformation, and rosette inhibition by antilymphocyte globulin, we have shown that cell-mediated immunity is depressed in megaloblastic anemia due to folate deficiency; this depression was reversed by folate treatment.     Abstract:
    We present an eight-year-old boy who initially presented to us with megaloblastic anemia and subsequently developed dislocation of lens. The child had a positive sodium nitroprusside test and homocystinemia. He was diagnosed to have homo cystinuria type I. His anemia improved on oral pyridoxine and folic acid therapy. Homocystinuria should be remembered as a cause of megaloblastic anemia.

    VITAMINS AND IRON IN ANEMIA
    General Concept
    The maintenance of normal red cell mass and the synthesis of hemoglobin are normally adjusted to take care of the physiological loss of the blood elements. Anemia results when there is excessive loss or diminished replacement of red cells. Most anemia's are deficiency diseases resulting from inadequate tissue concentrations of iron, vitamin B12, or folic acid. Correction of the deficiency is highly successful provided an accurate diagnosis is made. Erythropoietin is a useful stimulator of bone marrow in certain diseases.
    Iron Iron is contained in the body in various forms, principally as hemoglobin. Normal blood contains about 15 g of hemoglobin/100 ml, and each gram of hemoglobin contains 3.4 mg of iron. It may be calculated then that the total normal blood volume contains about 2.6 g of iron. In addition to hemoglobin, iron is contained in ferritin, the storage form for iron in the tissues, and in the serum attached to the carrier substance, the globulin transferrin. Minute quantities are also present in the cytochrome enzymes and myoglobin of muscle. Quantitatively, hemoglobin and ferritin contain the bulk of the iron in the body, amounting to a total of about 4 to 5 g. Under normal circumstances red cells are broken down at a steady rate, their lifespan being on the order of 120 days. Most of the iron released from the breakdown of hemoglobin is reutilized. As a consequence, the daily iron requirement in a normal adult is quite low, about 1 mg. Growth, menstruation, and pregnancy increase the iron requirement.
