Clinical Information:

The patient is a 16 year old white female who had undergone normal development of secondary sex characteristics, yet remained amenorrheic. Her pelvic exam was notable for pain, and the cervix was not visualized on speculum exam. Hormonal tests (estradiol, FSH, LH, TSH, prolactin) were all within normal limits for an adolescent female in the follicular stage. This enabled the physician to rule out androgen insensitivity (testicular feminization), and suggest that a pituitary tumor was not the cause of amenorrhea. An ultrasound exam was attempted (study not available). Transabdominally, the uterus was not identified. An endovaginal US exam could not be performed, secondary to pain. She was referred to MR for evaluation of the pelvic organs. Radiographic Findings:

MRI of the abdomen and pelvis:
T1 axial and T2 axial of the pelvis were obtained, confirming the diagnosis of MRKH syndrome.

What is Mayer-Rokitansky-Kuster-Hauser Syndrome? Mayer- Rokitansky-Kuster-Hauser (MRKH) syndrome is an uncommon variation in the prenatal development of the female genital tract. Its features include an absent or very short vagina and a uterus that can be absent or immaturely formed. Females with MRKH syndrome have functioning ovaries, normal external genitalia and the typical, 46, XX, female chromosome pattern. Breast development and growth of pubic hair are also normal. Associated renal and/or skeletal abnormalities are common. MRKH syndrome is also known as Mullerian (female internal sex organs) Agenesis (no growth) syndrome (a group of related medical findings). How often does MRKH syndrome occur? The incidence of MRKH syndrome is approximately 1 in 4,000 - 5,000 female births. Although it has been determined that the absence of a vagina and uterus is a result of the Mullerian ducts failing to form properly early in embryonic development, its underlying cause is unknown. When is a diagnosis of MRKH syndrome made? The normal external appearance of MRKH females makes it difficult to diagnose until puberty - often when a girl visits a physician because she has not started to menstruate. In some cases, a young woman may have attempted unsuccessfully to have intercourse. The average age of diagnosis is between 15 and 18 years, although occasionally a girl may be diagnosed at birth or during childhood because of other health problems. A pelvic ultrasound may be used to see the presence or absence of the uterus and its condition. Doctors might use laparoscopy, a surgical technique that allows the doctor to view the inside of the abdominal cavity, to determine the presence, absence and/or condition of the internal female sex organs. Blood work is usually done to determine a chromosome pattern and to test for hormonal levels.

Hi. I have been diagnosed with Mayer-Rokitansky-Kuster Hauser Syndrome (MRKH) and I was looking to find some individual who have been diagnosed with the samd disorder or who have been diagnosed with MURCS Association (asociated with Klippel Feil Syndrome (KFS, a similar disorder. Hi, My name is Elizabeth. I have MRKH, a disorder where you dont have a uterus and/or a poorly formed vagina. I'm 16. Anyway, were you given a diagnosis for your over condition-a syndrome or something? I live in VA and go to Johns Hopkins Hospital-great doctors there. I'm having kidney tests done soon. MRKH can affect your kidneys, uterus, vagina, renal system, and even your skeletal system. To my knowledge I have a mild case. Hope to hear from you soon. Elizabeth P.S. Do you have a period? If you don't mind me asking. Bye.

Magnetic resonance imaging in Mayer-Rokitansky-Kuster-Hauser syndrome
L Fedele, M Dorta, D Brioschi, MN Giudici, and GB Candiani Six girls with suspected Rokitansky syndrome underwent magnetic resonance imaging (MRI) and then sonography and laparoscopy to confirm the clinical diagnosis and define the precise characteristics of the malformation. The uterus was missing in all; four had mullerian rudiments in place of the uterus, which were cavitary in two as shown by sonography and in one by MRI. Both ovaries were present in all cases, normally located in four and extrapelvic in two; they were correctly identified by MRI in all cases and by sonography in four. Partially canalized vaginal rudiments were recognized by MRI in two girls and by sonography in one. One patient had unilateral renal agenesis, demonstrated by both MRI and sonography. Magnetic resonance imaging was more precise than laparoscopy and sonography in defining the anatomical characteristics of Rokitansky syndrome, and is less invasive and expensive than laparoscopy.

Department of Otolaryngology, University Hospital Nijmegen, The Netherlands. We describe a 21-year-old woman with congenital unilateral conductive hearing loss and an atypical form of the Mayer-Rokitansky-Kuster-Hauser syndrome. To our knowledge, this is the first reported case of this syndrome in which surgery for congenital stapedial ankylosis was successful. Besides aplasia of the vagina and uterus, the patient also had
