Age-related macular degeneration (AMD) is a disease that affects your central vision. It is a common cause of vision loss among people over age of 60. Community Education MACULAR DEGENERATION Macular degeneration impairs your ability to look straight ahead. There is a blind spot directly in front of you. It is always there and it never goes away. It is almost as if somebody were holding a silver dollar directly in front of your face, about six inches in front of you. You can see around it, but you cannot see through it, and it cannot be moved from the center of your vision. THE NORMAL EYE The human eye works like a photographic camera. The big difference is that the camera has a layer of film in the back while the human eye has a layer of nerve tissue in the back, called the retina, located where the film would normally be. The image focused on the retina is converted into a digital image (like an electric message) that is transmitted through the optic nerve to the brain where it is perceived as sharply focused vision. In a photographic camera, the lens focuses light rays in order to form an image on the photographic film. In the eye, two structures share this responsibility. The cornea and the lens of the human eye focus the light rays into an image on the retina, which lines the back of the eye. When a clear image is focused on the retina, we call this emmetropia, which means that a person can see without glasses. (See FIGURE No. 1)

Macular degeneration is the leading cause of blindness in America. It results from changes to the macula, a portion of the retina that is responsible for clear, sharp vision and is located at the back of the eye.
As macular degeneration advances, a distorted, dark, or empty area often appears in the center of vision. Most people with macular degeneration have the dry form, for which there is no known treatment. The less common wet form may respond to laser procedures, if diagnosed and treated early.
Some common symptoms are a gradual loss of ability to see objects clearly, distorted vision, a gradual loss of color vision and a dark or empty area appearing in the center of vision.

SIGNS AND SYMPTOMS

Some of the clinical retinal signs of dry AMD include drusen of the posterior pole, granular clumping and disorganization of the RPE in the macular area, macular RPE hyperplasia and degeneration of the outer retinal layers with circumscribed areas of geographic atrophy of the RPE. Some of the clinical retinal signs associated with wet AMD include hard and soft drusen, subretinal thickening secondary to classic or occult choroidal neovascularization (producing a grayish-green subretinal hue), subretinal, intraretinal or vitreous hemorrhage, subretinal and intraretinal exudation with serosanguinous fluid accumulation and fibrovascular scar formation (disciform scarring). PATHOPHYSIOLOGY
MANAGEMENT

Begin managing patients with potential or diagnosed AMD by recognizing the associated risk factors and providing patient education. The disease is more common in individuals who have a family history of AMD, are of light complexion, in those who have a cardiovascular history, history of previous lung infection, hyperopia or decreased hand grip strength. It is typically more progressive in males. Smoking is a significant risk factor. The management of patients with dry AMD begins with biannual eye examination, with dilated funduscopy. Home therapy, aimed at early detection, using a home Amsler grid may work to monitor the stability of suspicious or involved maculae. The elimination of potentially harmful ultraviolet light using UV coatings on spectacles and sunglasses may also reduce the risk of photochemical /oxidative damage to the retina for all patients.

Aging and Vision Loss: Macular Degeneration Macular degeneration is the leading cause of low vision and legal blindness in people over the age of 50. By age 75, 1 in 20 seniors will have developed symptoms. Macular degeneration results from damage to the macula, which is the central part of the retina (the nerve tissue that lines the back of the eye). The macula is responsible for central vision and the ability to see detail. When the macula is damaged, the eye loses its capability to identify fine detail, read small print, and recognize faces. Damaged regions can cause localized areas of vision loss, making objects in your field of vision fade or disappear. Straight lines or edges may also appear wavy. Macular Degeneration

Since the macula is only a very small part of the retina, (side) vision is usually maintained. There are many types of macular degeneration. The major type, age-related macular disease, ordinarily occurs after the age of 60 in one of several forms. The most common is the dry (or atrophic) form, which results from the gradual breakdown of vision cells in the retina. There is no medical or surgical treatment. In the wet form of macular degeneration, fluid or bleeding from new blood vessel growth can cause more damage than the degenerative process.

Overview Age-related macular degeneration is a chronic eye disease that occurs when tissue in the macula, the part of your retina that's responsible for central vision, deteriorates. The retina is the layer of tissue on the inside back wall of your eyeball. Degeneration of the macula causes blurred central vision or a blind spot in the center of your visual field. The first sign of macular degeneration may be a need for more light when you do close-up work. Fine newsprint may become harder to read and street signs more difficult to recognize. Eventually you may notice that when you're looking at a grid, some of the straight lines appear distorted or crooked. Gray or blank spots may mask the center of your visual field. The condition usually develops gradually, but may sometimes progress rapidly, leading to severe vision loss in one or both eyes.

AGE RELATED MACULAR DEGENERATION New Knowledge, New Treatments, New Hope William R. Freeman, M.D. Age Related Macular Degeneration is the most common cause of vision loss in developed countries. Other causes of vision loss such as cataract and glaucoma can be successfully treated in many cases but until recently, treatment was possible in only as small proportion of patients with age related macular degeneration. The prevalence of the disease increases with age. By definition, the disease is rare prior to the age of 55 . In individuals between 55 and 65, the disease causes loss of vision in only a small percent of individuals. By age 70-80, the disease affects up to 20% of Americans. There are two types of age related macular degeneration, the wet or exudative or bleeding type and the dry type. The dry type is more common but does not cause as severe vision loss as the wet type. The dry type of macular degeneration is characterized by deposits called drusen which form under the retina and which slowly enlarge and disturb the rods and cones causing vision loss. Wet macular degeneration occurs in patients who previously had drusen. It is more common in eyes with many large drusen. Thus in most patients who develop severe visual loss, this visual loss is due to wet macular degeneration and those patents previously had dry macular degeneration. The estimates vary but probably 20 percent or so of patients with dry age related macular degeneration may develop the wet form.
