Causes, incidence, and risk factors Return to top The macula is the part of the retina that allows the eye to see fine details at the center of the field of vision. Degeneration results from a partial breakdown of the retinal pigment epithelium (RPE). The RPE is the insulating layer between the retina and the choroid (the layer of blood vessels behind the retina). The RPE acts as a selective filter to determine what nutrients reach the retina from the choroid. Many components of blood are harmful to the retina and are kept away from the retina by normal RPE. Breakdown of the RPE interferes with the metabolism of the retina, causing thinning of the retina (the "dry" phase of macular degeneration). These harmful elements may also promote new blood vessel formation and fluid leakage (the "wet" phase of macular degeneration).

No Smoking
By Linda Kaspari Cigarette smoking reduces levels of plasma antioxidant, a substance in the blood stream, which protects retinal cells. Smoking causes the protective layer between the retina and blood vessels to erode, resulting in poor circulation, irritation and scarring. Smoking is damaging to the blood supply. Smoking exposes the body to free radicals which causes cellular damage and can cause poor circulation of nutrients to the retina and lens. Carotenoid levels and vitamin C blood concentrations are substantially reduced. Smoking oxidizes cholesterol, causing fat to build up in the arteries. This leads to heart attack, stroke, and probably macular degeneration.
Cataracts
Researchers think smoking contributes to the incidence of some types of cataracts. "What makes us suspect this is the consistency of studies all over the world," says Robert Sperduto, M.D., chief, epidemiology branch of the National Eye Institute. "If there's one thing people can do to improve their general health, including the health of their eyes, it is to stop smoking," says Dr. Sperduto. Carbon monoxide from cigarette smoke creates a hypoxic effect, it is clear that hypoxia will degrade night vision. (Hypoxia is a state of oxygen deficiency in the body which is sufficient to cause an impairment of function. The inadequate oxygen transport or the inability of the tissues to use oxygen causes hypoxia.)

(Reprinted with permission) In many primate studies, blue light has been shown to cause a photochemical reaction that produces free radicals in the RPE and the rods and cones. Researchers believe that these free radicals interact with the high oxygen and lipid content in human rod and cone tips to produce abnormal chunks of metabolized waste that cannot be properly processed by the RPE, clogging up the macula's maintenance system and producing dry macular degeneration. Melanin, the substance that gives eyes their color, protects the macula by trapping light rays so they don't reach the macula and cause damage. People with fair skin and blue or light-colored eyes may be particularly susceptible to macular damage by blue light because they have less melanin in their irises. Their blue eyes transmit up to one hundred times as much light to the back of the eye as dark colored eyes do. Additionally, when the light reaches the choroid and RPE of people with fair skin and blue eyes, there is less melanin there to absorb the radiant energy, leaving these tissues more vulnerable to light damage. Can blue light rays cause macular degeneration? Can you reduce your risk by protecting your eyes from blue light? The answer is maybe.
