The lymphatic system helps the body fight infection. There are two main types of cancer associated with the lymphatic system: Hodgkin's Disease and Non-Hodgkin's Lymphoma (NHL). Both are rare in children aged under 3, and are more common in older children and adults. Disease Background
Description of the Disease Lymphomas are cancers affecting the lymphatic system, the network of vessels and nodes that carry infection-fighting white cells, called lymphocytes, throughout the body. Lymphomas are generally classified as Hodgkin's disease and non-Hodgkin's lymphoma. In both types of cancer, the lymphatic cells begin to grow abnormally. Hodgkin's disease follows a more predictable pattern, and tends to have a more limited spread. It is typically treated with radiation or chemotherapy. Non-Hodgkin's lymphomas, on the other hand, may begin in a variety of places in the body, since lymphatic tissue is present in many different organs. LYMPHOMA - Received a call today from an oncologist who evaluated a 40 yr old male who had a localized 1 cm area of induration of scalp excised by his GP (good grief). They could find no evidence of systemic involvement. The oncologist (not able to get authorization yet from the patient's HMO GP) called and asked me what to do. Local radiation tx? Nitrogen mustard? PUVA? No treatment until new lesion? It is not a T cell lesion which was confirmed by appropriate immunotyping etc. Robert M. Peppercorn, M.D. x-ray therapy would seem to be the leading candidate for localized B-cell lymphoma on the skin. I dare say you''ll havle difficulty finding anything in print describing success rates with any other modality. Mark Valentine Having the experience of seeing two cases of b-cell lymphoma presenting in the skin I would offer the following observations. If the lesion is truly isolated and there can be no nodal involvement demonstrated I have one patient, a local internist by the way, who had the area irradiated. Her workup and radiation was done at the NIH. She is at least 15 years post at this point and disease free as far as I know. My second patient presented when she as about 70 with diffuse nodular involvement of the skin. Her nodules did not respond well to electron beam because of their depth and she did not have demonstrable internal involvement for 10 or more years. When she did get internal involvement it did not seem to slow her down and she died with heart problems at 85 years of age. She also had a few attempts at systemic chemo with very little effect on her disease. Some of her lesions were intensely pruritic and this was resolved by my local injections of triamcinolone 2.5 mg/cc. The nodules also went away after injection so when she was getting them on her face she ask me to inject those because of their rather unsightly appearance. CASE The patient is a 44 year old man who first noted decreased vision in the left eye in October 93, then in the right eye in January 94. He was examined by his ophthalmologist, who told him that he had "retinal inflammation and swelling." He was treated with prednisone 60 mg a day, and prednisolone acetate 1% every hour in both eyes. His vision improved minimally with the treatment. He also had numerous blood tests and a chest X-ray, which reportedly were all within normal limits. He was examined by a neuro-ophthalmologist, and had an MRI of the brain, which was also normal. Figure 1 These lesions were also found scattered nasally and inferiorly to the optic disc. (Fig. 2) Figure 2 Figure 3 Figure 4 Fluorescein angiogram of the right fundus showed areas of hyperfluorescence in the central macula and superior to the disc, which corresponded to some of the lesions noted on exam. (Fig. 5) Figure 5 Late frames of the angiogram showed persistence of the hyperfluorescence.

This radiograph represents the pretreatment image of a patient with lymphoma manifest as a huge anterior mediastinal mass. The lung fields otherwise are generally clear. The post-treatment x-ray shows calcified lymph nodes in the right hilum with reduction in the anterior mediastinal mass. The cell-type was Hodgkin's. CT scan better defines the relationship of the mediastinal mass with the cardiac silhouette and great vessels.

Bath Cancer Research provides consultant haematologists/ oncologists an international service for the diseases above. Cells from their patients are tested for ex vivo drug response to a panel of cytotoxic drugs (up to 25 drugs). A report is sent to the physician to aid choice of optimum therapy. This information helps maximise the likelihood of response (and therefore survival) and reduces the risk of causing patient toxicity with no clinical benefit. Some assays are chargeable.
