This and other credible information about cancer treatment, screening, prevention, supportive care, and ongoing trials is available from the National Cancer Institute. Lymphedema is the buildup of lymph (a fluid that helps fight infection and disease) in the fatty tissues just under the skin. The buildup of lymph causes swelling in specific areas of the body, usually an arm or leg, with an abnormally large amount of tissue proteins, chronic inflammation, and thickening and scarring of tissue under the skin. Lymphedema is a common complication of cancer and cancer treatment and can result in long-term physical, psychological, and social issues for patients. What Is Lymphedema?
Lymphedema is an accumulation of lymphatic fluid in the interstitial tissue that causes swelling, most often in the arm(s) and/or leg(s), and occasionally in other parts of the body. Lymphedema can develop when lymphatic vessels are missing or impaired (primary), or when lymph vessels are damaged or lymph nodes removed (secondary). When the impairment becomes so great that the lymphatic fluid exceeds the lymphatic transport capacity, an abnormal amount of protein-rich fluid collects in the tissues of the affected area. Left untreated, this stagnant, protein-rich fluid not only causes tissue channels to increase in size and number, but also reduces oxygen availability in the transport system, interferes with wound healing, and provides a culture medium for bacteria that can result in lymphangitis (infection). Lymphedema should not be confused with edema resulting from venous insufficiency, which is not lymph-edema. However, untreated venous insufficiency can progress into a combined venous/lymphatic disorder which is treated in the same way as lymphedema.

Thursday, May 3, 2001 Contact: HCFA Press Office
MEDICARE TO EXPEDITE COVERAGE FOR LYMPHEDEMA PUMPS
BALTIMORE HHS Secretary Tommy G. Thompson announced today that Medicare will expedite coverage of pneumatic compression pumps to make it easier for Medicare beneficiaries with lymphedema to take advantage of the technology. Secretary Thompson announced the expanded Medicare coverage while working at the Baltimore offices of the Health Care Financing Administration (HCFA). The Secretary moved HHS headquarters to the Health Care Financing Administration's offices in Baltimore this week to give HHS and HCFA staff an opportunity to work together to build a stronger HCFA. Medicare's new coverage policy eliminates language that made these compression pumps the treatment "of last resort" for beneficiaries suffering from lymphedema, an accumulation of lymphatic fluid causing abnormal swelling of the arms, legs, breast, neck or head that often develops when lymph nodes are removed during surgery. Breast cancer surgery is the most common cause of the condition in the United States. "It's important to make effective technologies available to Medicare beneficiaries when it helps them the most," Secretary Thompson said. "This coverage decision simplifies Medicare policy to allow older Americans who need these pumps to get them more quickly and easily."

Lymphedema
Description
Lymphedema is a persistent swelling, usually affecting an arm or leg, that sometimes occurs as a side effect of cancer treatment. This condition results from the abnormal accumulation of lymph fluid and tissue proteins. In rare circumstances, lymphedema may affect the head, torso or genitals. The underlying cause in all cases is damage to the lymphatic system, an important part of the immune system that circulates infection-fighting proteins throughout the body. Preventive care to avoid lymphedema and early treatment if swelling does start to occur are extremely important. Left untreated, lymphedema may lead to infections, numbness in the affected limb, inability to move joints, hardening of connective tissue and discomfort or pain. As a result, patients may:

When the amount of fluid is too great or when your lymphatic system is damaged, you get lymphedema. Lymphedema is a collection of protein-rich fluid that can cause edema (swelling) in your extremities (arms and legs). Extremity signs and symptoms of Lymphedema can include:
  • Swelling (edema)
  • Feeling of heaviness
  • Hardness under your skin
  • Frequent infections
  • Difficulty walking (leg swelling only)
What are the risk factors that increase the possibility of developing lymphedema?
  • Surgery involving the lymphatic system including your lymph nodes
  • Radiation Therapy
  • Infection
  • Trauma
  • A congenital defect of your lymph system
Guidelines for extremity care to prevent edema formation.
  • Do not allow blood pressures or injections in the arm that is on the same side as the lymph node surgery.
  • Do not wear jewelry or clothing that is tight on the involved limb.

        Lymphedema Following a Mastectomy
        What is the lymphatic system?
The lymphatic system consists of many vessels that carry lymph (a clear, colorless fluid containing water and a few blood cells that originates in many organs and tissues) throughout the body. The lymphatic system helps to protect and maintain the fluid environment of the body by filtering and draining lymph away from each region of the body. Often during a lumpectomy or mastectomy, some or all of the lymph nodes under the arm may be removed. The lymph nodes under the arm (also called the axillary lymph glands) drain the lymphatic vessels from the upper arms, the majority of the breast, the neck, and the underarm regions. The lymph nodes help to filter excess fluid, bacteria, and by-products of infections.

Lymphedema
Lymphatic fluid, or lymph, contains water, proteins, salts, organic substances, such as urea and glucose, and lymphocytes (these cells are the body's main method to develop immunities). Lymph is formed in small tissue spaces all over the body, collected into small lymphatic vessels that lead to larger lymphatic ducts, and eventually is deposited into the bloodstream. Lymphedema is the abnormal build up of lymphatic fluid in the tissues themselves, which can cause swelling and lead to other problems, including infection. It may develop as a result of surgery, radiation, infection, tamoxifen, or trauma. Find out what causes lymphedema and how to prevent it as well as the symptoms, stages, treatment, and possible complications.

From: hmgold@mcs.com I have a 65 year old obese diabetic woman with chronic stasis dermatitis. The skin of her legs is fibrotic and lichenified with elephantiasis verrucosa nostra changes. She has acute flares of the stasis dermatitis which produce bullae, erosions and ulcers. These have been treated successfully with Unna's boots. She cannot wear graduated pressure stockings because of the cone-like shape of her legs. I have ordered a Lymphapress pump for her to prevent leg edema/lymphatic edema and hopefully prevent ulceration. Does anyone have experience with these pumps? Is one better than another? Is there potential harm in using one on this patient? Harry Goldin, M.D. ======================= From: Marilynne Mckay

  • Evaluation of extremity lymphedema -
0.05 cc injections of Tc-99m Sulfur Colloid (0.22 micrometer filtered, 0.5 mCi per injection) are performed intradermally. 2 injections are performed on the dorsum of the foot or hand. Injection of the contralateral extremity provides a useful comparison. Patient ambulates (~ 2 flights of stairs) to aid lymphatic return. For lower extremity lymphoscintigraphy, sweeps are performed, leaving the injection sites just out of the field of view, up through the chest, in anterior and posterior projections, at a rate of 8 cm per minute. Full body sweep images are obtained every 30 minutes x three, followed by another set of images at three hours, and then as needed. The end point of a normal exam occurs when the clinical question is answered and the liver is visualized.
