The Benefits of Daily Use of Compression Therapy for Lymphedema

by Joachim E. Zuther
Compression Therapy Compression therapy is an essential part of lymphedema management. Without the benefits provided by either specialized short-stretch bandages or elastic support garments, successful treatment of lymphedema would be impossible. For good patient compliance – another important part in lymphedema management – it is necessary to understand how compression therapy affects lymphedema and how bandage materials and compression garments complement each... Proteins and substances too big to move through the walls of veins can be picked up by the lymphatic vessels because they have thinner walls. Edema may occur when there is an increase in the amount of fluid, proteins, and other substances in the body tissues because of problems in the blood capillaries and veins or a blockage in the lymphatic system. Lymphedema may be either primary or secondary. Primary lymphedema is a rare inherited condition in which lymph nodes and lymph vessels are absent or abnormal. Secondary lymphedema can be caused by a blockage or cut in the lymphatic system, usually the lymph nodes in the groin area and the armpit. Blockages may be caused by infection, cancer, or scar tissue from radiation therapy or surgical removal of lymph nodes. This summary discusses secondary lymphedema.

Lymphedema differs from accumulation of fluid (edema) due to venous insufficiency, which develops when blood that is backed up in veins causes fluid to leak from blood vessels into tissues. Edema may be caused by heart failure (see Heart Failure ), but lymphedema is not. Lymphedema results when an abnormality prevents lymph from being reabsorbed into the capillaries as it normally is. As a result, the lymphatic system cannot adequately drain lymph from the tissues.

You are at risk for developing lymphedema if you have had lymph nodes removed and/or radiation therapy. What is lymphedema?
Lymphedema is a swelling that is caused by the abnormal accumulation of lymph fluid in tissue, resulting from faulty development of the lymphatic system or blockage of the lymph vessels. These vessels are small channels like blood vessels that contain a clear fluid called lymph. When lymphedema presents at birth or appears in childhood, it is referred to as primary lymphedema. When its cause is attributed to trauma, infection, or surgical removal of lymph nodes, it is referred to as secondary lymphedema. The two types of primary lymphedema seen in the pediatric population are congenital lymphedema and lymphedema praecox (Milroy's disease). Congenital lymphedema, which is the rarest form of primary lymphedema, appears early in life. It generally involves more than one limb, but rarely extends above the knee. Swelling enlarges at a slower rate than body growth. It becomes less pronounced with age, and only supportive therapy is required in two-thirds of cases. The remaining one-third of patients have a poor prognosis and frequently require surgical interventions.

Lymphedema, it seems, is the proverbial red-headed stepchild of breast cancer awareness. Although lymphedema can strike anyone who has lymph nodes that have been damaged or removed. it seems to hit breast cancer patients in higher numbers, especially in those who have nodes removed from the underarm area. A number of organizations are trying to improve public awareness of lymphedema. To assist in this effort, TheBreastCareSite offers the articles below, which are intended to educate and inform on the topic. LYMPHEDEMA

LYMPHEDEMA Lymphedema is a long-term swelling condition that can occur in the adjacent hand, arm, and/or the entire quadrant (chest, side and back) as a result of breast cancer surgery and radiotherapy. The lymphatic system drains excess protein and water from the blood and natural and foreign substances from the tissues. This lymph fluid is filtered by the lymph glands and then returned to the blood system. When the lymphatic system has been damaged or blocked, lymph fluid builds up and causes mild to severe swelling in the hand or arm. This condition reduces oxygen in the tissues, slows healing and creates an ideal environment for infection and serious risk to your general health. Lymphedema can occur at the time of surgery and treatment, or months or years later. It is important that women diagnosed with breast cancer are informed about the causes, symptoms, prevention and treatment of lymphedema.
CAUSES
  • surgical removal of the lymph nodes
  • radiotherapy
  • accidental trauma
    • bruises, cuts, burns, sunburns, sprains, insect bites, pet scratches
