Findings: This CT of the abdomen demonstrates a very large multiseptated cystic mass involving the majority of the abdomen. Diagnosis: Mesenteric lymphangioma. Discussion: The most common type of mesenteric or omental cystic structure is the lymphangioma, followed by a nonpancreatic pseudocyst, duplication cyst, mesothelial cyst, and enteric cyst. A lymphangioma is a congenital malformation of lymphatics that occasionally occurs in the mesentery but may also occur in the neck, mediastinum, and axilla. Approximately 65% of these patients present at birth with approximately 90% presenting by 2 years of age. The usual symptoms include abdominal distention and small bowel obstruction. Lymphangiomas are large, thin-walled cystic structures that are usually multiloculated. The fluid may be serous chylous, or occasionally hemorrhagic. Sonographically, lymphangiomas are generally multiseptated anechoic masses which may contain internal echoes if hemorrhagic. CT demonstrates a multiseptated mass of fluid (or rarely fat) attenuation. Expected Total Enrollment: 150 Study start: April 2000
PROTOCOL OUTLINE: This is a randomized, multicenter study. Patients are stratified according to prior treatment (no prior treatment vs prior surgical treatment) and geographic area. Patients are randomized to 1 of 2 treatment arms. Arm I (Immediate treatment group): Patients receive an intralesional injection of picibanil with the aid of ultrasonography, transillumination, or fluoroscopy for localization of cysts. Treatment repeats every 6-8 weeks for a total of 4 injections.

SUPERFICIAL LYMPHANGIOMA (LYMPHANGIOMA CIRCUMSCRIPTUM) Multiple dilated lymphatic vessels that occupy the papillary dermis are characteristic of lymphangioma circumscriptum. These are associated with delicate fibrous stroma and, therefore, do not simply represent lymphangiectasis. A deep component may or may not be apparent in a given case. Hyperkeratosis is very uncommon though the location of the vessels is the same as that found in angiokeratomas.
This is from an area within which there were closely grouped, light amber papules that looked almost like blisters. The patient is a 45 year old female.

This lesion may wax and wane in size, especially when the child has an upper respiratory infection. Occasionally these tumors may undergo a rapid expansion in size when there is an acute hemorrhage in one of the tumor spaces forming a large, chocolate cyst. Histopathologically, the lesion is composed of multiple, irregular-shaped lymphatic channels which are lined by flatten endothelial cells (#22126) (#22125). Study Design. A case report. Objectives. To report and discuss a case of lymphangioma presenting as a dumbbell-shaped tumor in the epidural space of the lumbar spine. Summary of Background Data. Dumbbell tumor of the spine is not uncommon. Although the pathogenesis of such tumors varies widely, lymphangioma has not been reported previously, to these authors' knowledge. Methods. A 56-year-old woman with pain in the lower back and left leg was examined by plain radiographs and magnetic resonance imaging before surgery. Treatment consisted of the usual technique of surgical resection of the tumor. Histopathologic study of the resected material confirmed the diagnosis. Postoperative follow-up assessment was performed by magnetic resonance imaging. Results. Magnetic resonance imaging demonstrated a dumbbell tumor in the epidural canal area of the lumbar spine. Histopathologic diagnosis of the resected tumor was cavernous lymphangioma.
