Small bowel lymphangioma
The patient was an 8-year old boy suspected of having appendicitis. The tumor of the bowel was exteriorized and resected. The pathology showed acute inflammation in the dilated lymphatic channels.
Lymphangioma of the mesentery is a rare benign cystic tumor of the lymphatics of the bowel.
It arises from developmental defects in the mesenteric where fluid is being collected in small closed spaces.
Most of the times this tumors are multiloculated, have an endothelial lining, and contain a not very clear fluid similar to chyle.
As many others benign tumors of the mesentery it is found peripherally, near the intestine. Malignant tumors such as lymphoangiosarcoma, malignant teratoma, liposarcoma etc., are found near the root of the mesentery.
This type of “benign condition” can have local invasion with mechanical compression of adjacent viscera. Therefore, surgical excision of the tumor including the segment of the bowel involved is the only treatment for this kind of condition.

Introduction Lymphangiomas are benign lymphatic tumours typically composed of dilated lymphatic channels, which often occur subcutaneously in the head and neck region. Tonsillar lymphangiomas have been reported rarely in the literature and on clinical examination are indistinguishable from malignant tonsillar tumours. Histological confirmation is essential for diagnosis. We report a case of lymphangioma of the tonsil in an otherwise well 22 year old man and review the clinical and histological features of this rare tumour. Case Report A 22 year old man presented with a 2 month history of recurrent odynophagia, associated with left-sided otalgia and intermittent low grade fevers. He had been treated with several courses of oral antibiotics prior to his referral with no improvement. He reported constant left sided pharyngeal discomfort and described several episodes in the previous week of spitting out fresh blood in his saliva. He was otherwise in excellent health, being a professional sportsman, with no other symptoms to report. He was a nonsmoker with less than 10g of alcohol consumed per week.

For a general description, see lymphangioma Usually lymphangiomas occur in skin or subcutaneous tissue, but they may also occur in bone, usually in children and adolescents. The tumour tends to be a multiloculated osteolytic lesion in the medullary portion of the tibia, humerus, ilium, skull, mandible, vertebrae or small bones of the hand. This condition may be part of a spectrum consisting of haemangioma lymphangioma , cystic angiomatosis , haemangiomatosis and massive osteolysis ( Gorhams disease

The oesophagus, stomach and duodenum are only rarely involved. Lymphangiomas represent, for instance, only 0.5% of all benign tumours of the stomach. They are often multiple and may be lobulated and are commonly responsible for obstruction. Haemorrhage is very rare. Intestinal lymphangioma associated with haemangioma is seen in Maffuccis syndrome. Radiologically lymphangiomas have the same features as other small intramural benign lesions of mesenchymal origin. They have smooth borders, are round or ovoid, and present as pliable polypoid masses with a broad base, changing shape with compression at fluoroscopy. Also, see lymphangioma colon and haemangioma gastric

Cystic Lymphangioma Of The Retroperitoneum

Sir,
FIG. 1: Sonography showing multiloculated cystic mass lying anterior to left kidney. FIG. 2: Computed Tomography showing hypodense multiloculated mass with nonenhancing septations. Lymphangiomas are uncommon retroperitoneal masses and are always benign. They are developmental malformations following the failure of lymphatic tissue to establish normal communication with the remainder of the lymphatic system. Pathologically, they are classified into simple, cavernous and cystic types (1). Histologically, they appear as well circumscribed lesions composed of cysts of varying sizes and lined by flat endothelium. The adventitial layer may have fascicles of poorly developed smooth muscle and scattered aggregates of lymphoid tissue (2). Lymphangiomas are most commonly reported in the head and neck region, followed by trunk and extremities. All other sites, including the abdomen, account for less than 5% of cases. Retroperitoneal lymphangiomas are even rarer than abdominal lymphangiomas of mesenteric origin (3). Lymphangiomas tend to occur more commonly in the younger age group, with a slight male preponderance. They are frequently symptomatic. The symptoms may be pain, tenderness, fever, fatigue, weight loss and abdominal distension (2,4). The common complications associated with these tumors include infection and hemorrhage. These cysts may be associated with ascites(4).

Introduction Lymphangiomas are benign congenital malformation of lymphatic system and are common in pediatric practice specially involving neck and axillary soft tissue. However it may present at few uncommon and rare sites including spleen, mediastinum, retroperitonum, ovary and mesentery [1]. Clinical presentation is variable and may be misleading; therefore complex imaging studies are required in evaluation. We present a case of rare retroperitoneal lymphangioma with an uncommon presentation. Methods and Materials An eight month old male child presented in our pediatric hospital with the complaint of gradually increasing abdominal mass since last four to five months. On clinical examination the lesion was huge extending from epigastrium to hypogastrium and laterally to flanks. On close interrogation parents revealed that the small hump was present since birth, which remained unnoticed because it was asymptomatic. Plain film of abdomen showed a huge soft tissue mass lesion occupying almost whole abdomen, displacing the stomach bubble and bowel loops on the left side. Both hemi diaphragms were pushed upwards.

porkila I must apologize, but nothing in your detailed description reminds me of any condition with which I am familiar. Neither the clinical findings nor the laboratory values suggest any diagnosis I know about. I'm afraid I have nothing to contribute, and all I can suggest is that you get an opinion from a dermatologist at an academic medical center. Be sure to bring along all prior tests and results, so they won't have to be repeated.
Sorry.

I am dissapointed of course, but understand your response. I do feel that, however I have not recieved a response which warrants a cost of the $20.00 I forwarded. Could you please consider refunding this amount to my credit card, as my medical expenses leave us in hard financial struggles. I appreciate your understanding.
Yours sincerely,
Larissa Tyler.
