in San Francisco, CA, September 26 - 27, 2005! ACCOMMODATING PEOPLE WITH LUPUS

By Linda Carter Batiste, J.D. , and Beth Loy, Ph.D. Preface There are an estimated 1,400,000 to 2,000,000 people with lupus, affecting 1 out of every 185 Americans. Most individuals are diagnosed with lupus between the ages of 20 and 40. Therefore, many individuals with lupus are members of the workforce. These statistics, coupled with the requirements of the Americans with Disabilities Act (ADA) show why knowing about workplace accommodations for people with lupus is important. When considering accommodations for people with lupus, the accommodation process must be conducted on a case-by-case basis. Symptoms caused by lupus vary from person to person, ranging from mild symptoms that affect only a few organs to life-threatening symptoms that can cause death. When determining effective accommodations, the person's individual abilities and limitations should be considered and problematic job tasks must be identified. Therefore, the person with lupus should be involved in the accommodation process.

Discoid Lupus Erythematosus l. Definition: Discoid lupus erythematosus is a chronic skin condition characterized by inflammation and scarring type skin lesions which occur on the face, ears, scalp and at times on other body areas. These lesions develop as an inflamed growth with, scaling and a warty like appearance. The center areas may appear lighter in color surrounded by an area darker than the normal skin. When lesions occur in hairy areas such as the scalp, permanent scarring and hair loss can occur. A small percentage of patients with discoid lupus can develop disease of the internal organs which can make the person sick. II. Workup:
  • If your doctor suspects that you have this condition, a skin biopsy needs to be done to confirm the diagnosis because other conditions can look like discoid lupus erythematosus. If the skin biopsy shows discoid lupus erythematosus, then further testing may be indicated.
III. Causes:
  • The cause is unknown. This condition tends to run in families. Females outnumber males with this condition 3 to 1. In some patients with discoid lupus erythematosus, sunlight may make the lesions come out.

The original range of Canis lupus consisted of the majority of the Northern hemisphere from the Arctic continuing south to a latitude of 20° S, which runs through southern Central Mexico, northern Africa, and southern Asia. However, due to habitat destruction, environmental change, persecution by humans, and other barriers to population growth, gray wolf populations are now found only in a few areas of the contiguous United States, Alaska, Canada, Mexico (a small population), and Eurasia. Discoid Lupus Erythematosus
Discoid lupus erythematosus (DLE) is a chronic skin condition of sores with inflammation and scarring favoring the face, ears, and scalp and at times on other body areas. These lesions develop as a red, inflamed patch with a scaling and crusty appearance. The center areas may appear lighter in color with a rim darker than the normal skin. When lesions occur in hairy areas such as the beard or scalp, permanent scarring and hair loss can occur. A small percentage of patients with discoid lupus can develop disease of the internal organs, which can make the person sick. Children and people with many spots are usually at more risk of this. If a doctor suspects this condition, a skin biopsy needs to be done to confirm the diagnosis because other conditions can look like discoid lupus erythematosus. If the skin biopsy shows discoid lupus erythematosus, then further blood testing may be indicated.
The exact cause is unknown, but it is thought to be autoimmune with the body's immune system incorrectly attacking normal skin. This condition tends to run in families. Females outnumber males with this condition 3 to 1. In some patients with discoid lupus erythematosus, sunlight and cigarette smoking may make the lesions come out.
Cortisone ointment applied to the skin in the involved areas will often improve the lesions and slow down their progression. Cortisone injections into the lesions will also treat discoid lupus and usually are more effective than the ointment form of cortisone. If the lesions are becoming unsightly and one really feel something more needs to be done, a drug named Plaquenil will often improve the condition. Patients on Plaquenil need eye exams once a year to prevent damage to the retina of the eye and periodic blood work. Closely related drugs (Aralen, Quinacrine) may be more effective but have more side effects. Other drugs, such as Accutane and Soriatane, can also be used.

The northern and central stars of Lupus, the Wolf, loop around the picture to the southwest of Scorpius . The constellation is cut off by the horizon, which is bright from distant lamps. Pi Scorpii, the southern star of the three-star head of Scorpius, is the brighter star near the upper left corner. Kakkab, Alpha Lupi, is barely seen on the horizon at the lower right. Though a blue star, it appears reddish here because of absorption of the light by the Earth's atmosphere. Chi Lupi is the second star down and to the right of Pi Scorpii. Northeastern Centaurus is at right. From lower latitudes, central Lupus , including Kakkab, shines much more brightly than it does here.
