Abstract:
We describe our work on HEPAR II, a probabilistic causal model for diagnosis of liver disorders. The model, a Bayesian network capturing the causal interactions among various risk factors, diseases, symptoms, and test results, is based on expert knowledge combined with clinical data captured in medical records. The main applications of HEPAR II are assistance in diagnosis and training of beginning diagnosticians. We outline the principles of the applied approach, present a brief description of the model, and report its diagnostic performance. Keywords: Medical decision support systems, Bayesian networks.

Liver Disease
Liver disease is a term for a collection of conditions, diseases, and infections that affect the cells, tissues, structures, or functions of the liver.
What is going on in the body?
The liver is an important organ in the body located in the upper right quadrant of the abdomen. It is responsible for: filtering the blood making bile, a substance that aids in digestion and helps rid the body of harmful substances processing fats and sugars, helping the body store energy for later use making important proteins, such as those involved in blood clotting metabolizing many medications, such as barbiturates, sedatives, and amphetamines the storage of vitamins A, B12, D, and several of the B-complex vitamins. The liver also stores iron and copper. If the liver becomes inflamed or infected, its ability to perform these functions may be impaired. Liver disease and infections are caused by a variety of conditions including viral infections, bacterial invasion, and chemical or physical changes within the body. The most common cause of liver damage is malnutrition, especially that which occurs with alcoholism. Symptoms of liver disease may be acute, occurring suddenly, or chronic, developing slowly over a long period of time. Chronic liver disease is much more common than acute. The rates of chronic liver disease for men are two times higher than for women. Liver disease may range from mild to severe depending on the type of disease present.

More than 15 million people in the United States suffer from liver and gallbladder diseases, according to the American Liver Foundation. And, more than 43,000 die of a liver disease each year. The liver is the largest organ in the human body. It is also one of the most important ones. The biliary system consisting of the bile ducts and the gallbladder and the pancreas are all closely associated with the functioning of the liver. Some liver, biliary, and pancreatic diseases are congenital. Others can be prevented. In any case, whether these diseases are congenital, injury-related, viral-induced, or alcohol-induced, they can be devastating to a person's health and require professional care.

The Center for Liver Disease and Transplantation is a multidisciplinary treatment center offering comprehensive medical, surgical and radiologic therapy for disorders of the biliary system in both adults and children. A team of outstanding clinical professionals has been recruited for the new Center. Fulfilling their joint vision of a comprehensive liver treatment center, Drs. Jean C. Emond and Robert S. Brown, Jr., have renewed their past clinical association by becoming co-directors of the Center for Liver Disease and Transplantation. The CLDT was fully approved as a liver transplant center in July 1997 after extensive review and consideration by the New York State Department of Health. Statistics complied already compare favorably to established programs elsewhere. A strong background for a new program is provided by New York Presbyterian Hospital and the research expertise of two world renowned medical teaching institutions.
