L eishmaniasis is endemic in 88 countries throughout Africa, Asia, Europe, and North and South America. There are an estimated 12 million cases worldwide, with 1.5 to 2 million new cases each year. Although the incidence of leishmaniasis is greater in the Old World than in the New World, the U.S. traveler is most likely to contract this disease in Latin America. Fifty to 100 cases of New World cutaneous leishmaniasis are diagnosed each year in the United States. They are contracted mainly in Peru and Brazil, although the disease is endemic and can be contracted in any country from Mexico to Argentina, except Uruguay and Chile. There also is an endemic focus in Texas. Leishmaniasis is a disease associated with rural areas and poverty, but it has adapted to the urban environment as well.

A Khartoum newspaper reported in late Dec there had been 4000 cases of
leishmaniasis in the Eastern Sudan. On 21 Jan 1998, DPA reported on the
situation, quoting the health minister of the Gadarif state who provided
the same statistic, saying that cases had dramatically increased during
December and was "spreading at an alarming rate" in areas around the Atbara
and Rahad rivers. Leishmaniasis, in the Sudan known in its worst [visceral] form as
Kala-azar, is a parasitic protozoan "wasting" disease, spread by the sandfly, that can affect internal organs. According to the health minister, the medicine for treating the disease is scarce (some say unavailable) within the country. Further, Britain is Sudan's only source for the medicine and does not manufacture it in large quantities. [The current APHA communicable disease manual says that Pentostam is available from the US CDC; the other effective drug is

Leishmaniasis Document Actions Geographic Information Systems (GIS) for an efficient monitoring and control of cutaneous leishmaniasis in the Mediterranean area. ZCL-GIS sub project in EMPHIS aims to enhance current surveillence systems of ZCL by integrating temporal (incidence data) and spatial components (basic administrative unit) represented on computerized maps using a Geographic Information System (GIS). Multiple sources of information related to ZCL (incidence, human population density, rodent activity and vegetation are placed on individual maps and layered to identify overlaps on these maps; thereby, providing spatial relationship and correlative activity. Electronic transmission of generated data and maps will permit better understanding of ZCL risk and targeted control interventions. The ultimate objective (5 to 10 years) is to enhance the surveillance of leishmaniasis by integrating to parasitic disease programs a validated tool that will help managers designing and implementing more efficient control programs of cutaneous leishmaniasis and a risk safe land use.

Leishmaniasis
The leishmaniases are a group of diseases caused by protozoan parasites of the Leishmania complex. The major clinical forms are:
  • Visceral leishmaniasis or kala-azar, affecting an estimated 500,000 people each year, is a deadly disease if left untreated. The parasites colonize the internal organs, in particular the spleen, bone marrow and lymph nodes. (Muco)cutaneous leishmaniasis, affecting approximately 1.5 million people per year. In this case the parasite is confined to the skin where it causes one or more slow-healing sores (oriental sore, Aleppo boil). In the case of mucocutaneous leishmaniasis, the infection spreads to the mucosal membranes, especially those of the mouth and nose, where it may cause extensive damage.

Author details Although cutaneous leishmaniasis is occasionally seen in Australia in overseas travellers and migrants, visceral leishmaniasis has been reported rarely and only in people who were immunocompromised. We describe an 18-year-old immunocompetent man who presented with pancytopenia and a 2-week history of fever and lethargy a year after visiting the Greek Islands. Visceral leishmaniasis was diagnosed after a bone marrow biopsy showed protozoa, and the patient responded well to treatment with liposomal amphotericin. To our knowledge, this is the first case of visceral leishmaniasis in an immunocompetent patient in Australia. T he leishmaniases are a group of infections caused by protozoa belonging to the genus Leishmania . We report an immunocompetent patient with visceral leishmaniasis, probably acquired in Greece. As far as we are aware, this is the first report of visceral leishmaniasis in an immunocompetent person in Australia. Clinical record An 18-year-old man of Greek parentage presented with a 2-week history of dry cough, diarrhoea, lethargy, anorexia and fever. He had a past history of cutaneous vasculitis of uncertain aetiology and acne vulgaris. He was not taking any regular medication.
