INSECT SCIENCE AT BOSTON COLLEGE WHAT IS LEISHMANIASIS? Leishmaniasis is a group of diseases caused by intracellular protozoan parasites of the genus Leishmania. Twenty-one species of the genus infect humans, and all are spread by bites of infected sand flies of the subfamily Phlebotominae. There are two major forms of leishmaniasis: cutaneous leishmaniasis and visceral leishmaniasis. Cutaneous leishmaniasis, which newly afflicts 1.5 million people globally, causes sores on the skin, which can be disfiguring. Cutaneous leishmaniasis can spread to mucus linings of the nose and throat to cause a less common disease known as mucosal leishmaniasis. In contrast, visceral leishmaniasis causes a disruption of the immune system, resulting in fever, enlargement of the liver, enlargement of the spleen, anemia, and loss of weight. There are half a million new cases of visceral leishmaniasis in the world each year. If untreated, visceral leishmaniasis is fatal. HOW DO PEOPLE CONTRACT LEISHMANIASIS?

DESCRIPTION: An infective condition caused by several species of the protozoan Leishmania. Four major clinical syndromes are recognized.
  • Visceral leishmaniasis (Kala-azar, Black fever) may be endemic, epidemic or sporadic. Different forms are:
    • Mediterranean or infantile Kala-azar: Seen primarily in the Mediterranean area, China and Latin America. Dogs, jackals, foxes and rats are potential reservoirs. The strains responsible for the Mediterranean and American disease are sometimes referred to as L. infantum and L. chagasi respectively.
    • Indian Kala-azar: Age and sex distribution similar to African Kala-azar. Humans are the only known reservoir and transmission is by anthropophilic sandflies.
  • Cutaneous leishmaniasis of the old and new worlds. Characterized by one or more localized lesions on exposed areas. These ulcerate centrally and spread centrifugally. Spontaneous healing less common in new world disease. Satellite lesions and lymphadenopathy may or may not occur.
  • Mucocutaneous leishmaniasis (espundia). One or more lesions on the legs which may ulcerate. In 2-5% of patients, metastatic lesions appear in the nasopharynx after months or years, and may result in painful mutilating erosions of soft tissue. Lesions may occur rarely in perineum.
  • Diffuse cutaneous leishmaniasis. Extensive skin lesions , but no visceral lesions. Clinical picture may be similar to lepromatous leprosy.

(There are no reports of and Canadian veterns being infected). An epidemic of Leishmaniasis (kala-azar or black fever) is reported from the Northeast Indian state of Bihar with spread into neighbouring Nepal. In May 1993 WHO rpeorted an outbreak of Visceral Leishmaniasis in Southern Sudan and in the western Upper Nile Province. The Ministry of Health in the State Capital Patna have reported 27,000 cases in 1992 and WHO estimates that there have been 10,000 deaths, with the possibility of dramatic increases in these figures. The causative protozoan infection is transmitted by sandflies. Travellers to rural areas where this disease is being reported should take very strict precautions to avoid being bitten by sandflies because no vaccines or prophylactic drugs are available. Preventive measures for the individual traveller are aimed at reducing man-fly contact. Outdoor activities should be avoided when sandflies are most active (dusk to dawn). Although sandflies are primarily nighttime biters, transmission of leishmaniasis may occur during the day if resting sandflies are disturbed. Sandfly activity in an area may easily be underestimated, because sandflies are noiseless fliers and occasional bites may not be noted.

Leishmaniasis refers to several different illnesses caused by infection with an organism called a protozoan. Description
Protozoa are considered to be the most simple organisms in the animal kingdom. They are all single-celled. The types of protozoa which cause leishmaniasis are carried by the blood-sucking sandfly. The sandfly is referred to as the disease vector, simply meaning that the infectious agent (the protozoan) is carried by the sandfly and passed on to other animals or humans in whom the protozoan will set up residence and cause disease. The animal or human in which the protozoan then resides is referred to as the host. Once the protozoan is within the human host, the human's immune system is activated to try to combat the invader. Specialized immune cells called macrophages work to swallow up the protozoa. Usually, this technique kills a foreign invader, but these protozoa can survive and flourish within macrophages. The protozoa multiply within the macrophages, ultimately causing the macrophage to burst open. The protozoa are released, and take up residence within other neighboring cells. At this point, the course of the disease caused by the protozoa is dependent on the specific type of protozoa, and on the type of reaction the protozoa elicits from the immune system. There are several types of protozoa which cause leishmaniasis, and they cause different patterns of disease progression.

Foxhounds are the most prevalent breed affected in the United States, but other breeds of dogs have been identified with no apparent history of travel. As a result of the diverse manifestations of clinical signs and the lack of suspicion of this previously exotic illness, veterinarians need to have better awareness in order to achieve a clinical diagnosis. Etiology Leishmaniasis is caused by diphasic protozoa of the genus Leishmania and Viannia The disease is endemic in many parts of the world including Central and South America, Africa, India, and the Mediterranean basin. In many of the endemic areas, dogs are considered the major reservoir for human disease while in other regions people are the principal reservoir for further human spread.

Leishmaniasis is a parasitic disease transmitted by the bite of a sandfly that is infected with Leishmania parasites. Currently 350 million people in 88 countries around the world are threatened, and 12 million people are affected by leishmaniasis. Of the 1.5 – 2 million new cases of leishmaniasis estimated to occur annually, most occur in the tropics and subtropics, including the Middle East. In 2002, leishmaniasis reached epidemic levels in Afghanistan, with the World Health Organization calling for more funding and research for the disease.

Leishmania spp. Leishmaniasis
has the potential to infect around 350 million people in eighty-eight countries in the tropics and subtropics. Leishmania epidemics have been reported in Northern India, the junction of Eritrea, Ethiopia and Sudan, and southern Europe. Current World Health Organization (WHO) estimates report 12 million people are infected with an annual incidence of two million. Leishmania can be subdivided into three categories: Cutaneous, Mucosal and Visceral Leishmaniasis. Visceral Leishmaniasis has been associated with HIV and is fatal if left untreated. These parasites are transmitted by sandflies and also display antigenic variation. Vaccination efforts have been unsuccessful and drug resistance is mounting. Organism Leishmania spp.
